Forty-six children with acute rheumatic fever were admitted to Coronation Hospital, Johannesburg, between April 1981 and December 1984; 4 of them were admitted twice during this period. Their ages ranged from 4.5 years to 12.4 years. Carditis was present in 26 patients, arthritis in 22, chorea in 14, subcutaneous nodules in 3 and erythema marginatum in 2. Various morbidity parameters in Mamre, a village of 4,623 residents in the western Cape, are described. Acute morbidity (either injury or illness) occurring in the 2 weeks before the survey was reported by 4% of respondents. Blodgett Memorial Medical Center began developing an IVF program in September of 1982. It took 10 months to establish animal and human IVF laboratories and an andrology laboratory. All personnel were recruited from within the hospital. The use of radiotelemetry for continuous electrocardiographic monitoring of patients outside of special care units, although common practice in many community hospitals, has not been adequately studied. Two-hundred five consecutive patients placed on telemetry over a 3-month period were prospectively studied to determine the benefits of radiotelemetry. A review was performed of the computed tomograms (CTs) of 500 children which had been reported as showing widening of the supratentorial subarachnoid spaces with normal cerebral substance. A questionnaire and serologic survey of a Pennsylvania community revealed that 23 (54%) of 43 study participants had antibody titers to Toxocara canis (enzyme-linked immunosorbent assay test [ELISA]) in the range defined as positive (greater than or equal to 1:32). Acute epiglottitis is a fulminating pediatric emergency which should be managed with antibiotics and the establishment of an airway. Either tracheostomy or nasotracheal intubation is employed for airway intervention. Because of the morbidity associated with tracheostomy, recent reports have stressed the use of nasotracheal intubation. Experience in 21 cases of acute epiglottitis emphasizes nasotracheal intubation as the treatment of choice in a community hospital.

The authors studied the records of all persons convicted of homicide or found not guilty by reason of insanity during the uears 1963 through 1975, 6 years before and after the opening of the community-based psychiatric facility, in Albany County, New York. Using the Fisher Exact Probability Test, a statistically significant increase was found for those found not guilty by reason of insanity (p = .041) and those with a history of mental illness found guilty (p = .036).
