Synthesis and spectroscopy of a series of substituted N-confused tetraphenylporphyrins.

A series of N-confused tetraphenylporphyrins (H(2)NCTPPs) with substituents on either the para- or the 3,5-positions of the meso phenyl rings were prepared using Lindsey conditions. Both electron-withdrawing and electron-donating groups were chosen in order to probe the effects of peripheral substitution on the properties of the macrocycles. The series includes 5,10,15,20-tetra-(4-R-phenyl) N-confused porphyrins (where R = bromo (1), iodo (2), cyano (3), methoxy (4), 2',5'-dimethoxyphenyl (5), or ethynyl (6)) and 5,10,15,20-(3,5-di-tert-butylphenyl) N-confused porphyrin (7). Absorption and ste

[Confusing clinical presentations and differential diagnosis of bipolar disorder].

An early recognition of bipolar disorders may have an important impact on the prognosis of this disorder according to different mechanisms. … to delimit mood-episodes may help to adjust the diagnosis. Organic disorders may lead to diagnostic confusion, but it is generally proposed that bipolar disorder should be treated the same way, whether or not an organic condition is detected (with special focus on treatment tolerance). Addictive disorders are frequent comorbid conditions in bipolar disorder …

[Rheumatoid arthritis and the kidney. Pinpointing an aspect of confusing contours].

In literature there are only a few papers about renal involvement in rheumatoid arthritis. The scarcity of reports is due to the difficulties of pinpointing this subject; in fact a bloody investigation like kidney biopsy is necessary to obtain an exact diagnosis. Moreover it is often clinically hard to distinguish renal injury provoked by rheumatoid arthritis from nephropathy caused by drugs, either non steroidal antiinflammatory drugs or disease modifying antirheumatic drugs. This topic is perhaps neglected because primary renal involvement in rheumatoid arthritis is not considered to influen

A woman with recurrent acute confusion: a case of cycloid psychosis.

A woman 46 years old presented with recurrent, brief, sudden-onset episodes of confusion, aimless motor overactivity and lability of mood, from which she recovered completely. She was admitted on each occasion to a medical unit with a provisional diagnosis of organic mental disorder and thoroughly examined. No organic etiology was identified. This patient meets the criteria for cycloid psychosis. The implications for diagnosis, investigations and management are discussed.

Diastematomyelia: a congenital anomaly not to be confused with a giant hairy nevus.

The role of the plastic surgeon in the diagnosis and management of congenital anomalies is often a complicated one that involves the integration of input from numerous specialists into the evaluation process. Diastematomyelia is a serious neurosurgical problem that often presents with cutaneous findings in a neurologically asymptomatic patient. Early recognition of these cutaneous manifestations by the plastic surgeon, evaluation with an MRI scan, and consultation with a pediatric neurosurgeon can prevent delayed diagnosis of this surgically correctable anomaly and irreversible neurologic sequ

Extrapleural fat collections: pseudotumors and other confusing manifestations.

Because thoracic fat is mobile and plastic, it can assume a variety of shapes and distributions. Fat collections can be confusing, and some can even mimic tumors. This essay highlights the variable and potentially confusing manifestations of normal thoracic fat on plain radiographs and CT scans. It does not emphasize fatty tumors, ordinary mediastinal lipomatosis, costal fat pads, or diaphragmatic hernias, as these topics have been reviewed recently.

Sweat gland carcinoma: an easily confused skin malignancy.

Sweat gland carcinoma is a rare malignancy of the skin appendage. Surgeons and pathologists have limited experience with such tumor. Various classifications and diagnostic identifications lead to easy confusion with other metastatic lesions. Eight cases of sweat gland carcinoma were collected from 1020 cases of primary skin malignancies seen at the Veterans General Hospital-Taipei over the past 20 years. After a careful review of the clinical history and histopathology files, as well as immunohistochemical stains of CEA and S-100 protein, only three cases were proved to be sweat gland carcinom…

[Risks of confusion in the laboratory detection of drepanocytosis (author's transl)].

A brief review of the most usual methods for detecting hemoglobin S, their principles, their limits and interpretations. The author, then, considers the identification of hemoglobin S either isolated or associated with other abnormal hemoglobins or with thalassemiae. He emphasizes the techniques preventing some confusion in results interpretation. Problems in identifying HbS in new-born baby are also considered.

[From imagination to reality : I-CBT against inferential confusion in OCD].

Inference-Based Cognitive Behavioral Therapy (I-CBT) is an original cognitive approach developed in Canada that provides an effective alternative for treating obsessive-compulsive disorder (OCD). It is better tolerated than Exposure and Response Prevention (ERP), the core CBT technique and gold-standard treatment for OCD. I-CBT targets inferential confusion, a central concept in which the person places more trust in imagined possibilities than in directly observable reality. I-CBT aims to deconstruct this confusion. Key therapeutic steps help the patient step out of the story created by OCD an…
