Unexplained Loss of Consciousness in a Patient With Klinefelter Syndrome and Brugada Syndrome.

Managing patients with both Klinefelter syndrome (KS) and Brugada syndrome (BrS) is challenging because of the rarity of this unique interaction. A man diagnosed with KS during fertility treatment experienced chronic muscle pain, severe depression, and cognitive difficulties, managed with testosterone therapy and antidepressants. Increased testosterone doses led to recurrent episodes of loss of consciousness, initially considered epileptic. Hormonal assessments, cardiac imaging, Holter monitoring, electrophysiological studies, and genetic testing led to BrS diagnosis. A positive ventricular ta

Acute hydrocephalus triggered by isolated traumatic subarachnoid hemorrhage in the posterior fossa following mandibular fracture in a toddler: a rare case report.

INTRODUCTION: Facial bone fractures triggered by low-height falls are rare in toddlers, while severe intracranial injuries resulting from minor trauma are extremely rare. CASE: Herein, we report the case of a 2-year-old girl who fell from a baby chair, striking her chin, who rapidly developed impaired consciousness 3 h later. The patient subsequently presented with a mandibular fracture and acute obstructive hydrocephalus due to a traumatic isolated subarachnoid hemorrhage in the posterior cranial fossa. She was successfully treated with ventricular drainage, which achieved a favorable outcom

Takotsubo Cardiomyopathy Presenting with QT Prolongation and Torsade de Pointes in a Patient with Coronavirus Disease 2019.

Coronavirus disease 2019 (COVID-19) is associated with cardiovascular complications; however, Takotsubo cardiomyopathy (TCM) with QT prolongation and Torsade de pointes has been reported only rarely. We present a case of TCM after QT prolongation and Torsade de pointes. A 58-year-old woman was admitted because of COVID-19-related pneumonia. Seven days after admission, she developed sudden loss of consciousness without any indication of cardiovascular disease. A monitoring electrocardiogram indicated Torsade de pointes and a prolonged QT interval. Emergency cardiac catheterization revealed TCM.

[Critical care management of patients with spontaneous subarachnoid haemorrhage - a review].

Spontaneous subarachnoid haemorrhage is characterized by extravasation of blood into the subarachnoid space without a preceding trauma. The leading cause is a ruptured intracranial aneurysm. Serious neurologic complications can occur, such as rebleeding, cerebral vasospasm and delayed cerebral ischemia. Subarachnoid haemorrhage is a serious condition with a high mortality rate and those who survive often suffer long-term consequences. Prevention of rebleeding by aneurysm repair is essential and guidelines recommend this procedure should be done as soon as possible or within 72 hours. Managemen

[Study on the operation process and construction standard of an esophageal cancer screening cohort study].

One of the most cost-effective measures in cancer prevention on cancer is to advocate to be aware of the disease, consciously changing negative behaviors, and taking the initiative to participate in regular physical checkup programs. Esophageal cancer is one of the malignant tumors accompanied by a heavy disease burden in China. Routine screening, early diagnosis, and treatment are the critical points of preventing and treating the disease. Cohort studies help understand the natural history and risk factors of esophageal cancer and identify high-risk groups of the disease. This paper intends t

Focal atonic seizures originated in the posterior temporoparietal area: A case report.

Atonic seizures are typically observed in younger children with Lennox-Gastaut syndrome and have been rarely described in adults. Herein we present a case of the adolescent-onset drug-resistant focal epilepsy in a 31-year-old woman with focal atonic seizures originating in the left posterior temporoparietal area and manifesting without aura with abrupt impairment of consciousness and slow falling down. According to the video-EEG monitoring, the seizure began with the medium amplitude spikes principally at T5 area evolving onto the left centroparietal area, which was immediately followed by the

Remote cerebellar haemorrhage: A case report.

Intracranial haemorrhage after supra-tentorial craniotomies can occur in a typical pattern and location which may suggest the diagnosis of remote cerebellar haemorrhage (RCH) which is quite a rare occurrence. The 'Zebra Sign' refers to a pattern of hyperdensity indicative of blood and hypodensity indicative of normal cerebellar parenchyma in a curvilinear, stripe-like fashion along the cerebellar folia and is a characteristic imaging finding in RCH. RCH in general doesn't require surgical treatment, however in cases of significant hydrocephalus or progressive deterioration of consciousness sur

Cerebral Malaria in a Patient with Recent Travel to the Congo Presenting with Delirium: A Case Report.

INTRODUCTION: Cerebral malaria, a syndrome of altered consciousness, is a rare and severe neurologic complication resulting from Plasmodium falciparum. Historically, cerebral malaria has been seen more frequently in children rather than adults. To complicate the diagnosis, cerebral malaria has few specific symptoms and neurologic findings can vary with each case. CASE REPORT: We describe a case of a 61-year-old male who returned from the Democratic Republic of Congo and presented to the emergency department with dehydration, fatigue, and intermittent confusion. He was ultimately diagnosed wit

SEVERE DENGUE IN PREGNANT WOMEN.

Over half of the world's population lives in areas at risk of dengue infection with 70% of overall disease burden in Asia. A shifting in age group of dengue patients towards adulthood has been widely seen in Asia. This will affect an increase in incidence of dengue infection in childbearing age and pregnant women. Two cases of severe dengue in pregnant women admitted to Photharam Hospital, Ratchaburi, Thailand were described. Both of them had dengue shock syndrome with organopathy involving the central nervous system with alteration of consciousness in one. They had uneventful recoveries follo

[Unsuccessful anesthetic management for cesarean section in a patient with primary pulmonary hypertension].

A 31-year-old woman with primary pulmonary hypertension presented for an elective cesarean section at the 34-week gestation. After monitoring pulmonary artery, systemic artery blood pressures and an electrocardiogram, continuous lumbar epidural anesthesia was performed. Uneventful delivery was followed by a sudden decrease in systemic pressure and loss of consciousness. Her trachea was intubated and administration of epinephrine was started. Nitroprusside and milrinone were infused to decrease pulmonary artery pressure and to maintain systemic arterial pressure. However, she died after 16 hour

[Inhalation induction with sevoflurane in paediatrics: what is new?].

The delay for loss of consciousness can be shortened by using high concentration sevoflurane > 6% and by adding N2O during inhalation induction with sevoflurane in paediatrics. Mean time for tracheal tube insertion is lower than 5 min in the majority of studies. This shorter delay is not associated with any significant increase in clinical side effects. However, recent studies have demonstrated the epileptogenic effect of high effect site sevoflurane concentration (occurrence of spike wave on the EEG). Inhalation induction with high alveolar sevoflurane concentration is questionable mainly whe

[Ventricular arrhythmia in hypertrophic cardiomyopathy. When and how to treat].

Patients with apical hypertrophy have a natural favourable history. Non Specific Ventricular Tachycardia (NSVT) at ambulatory monitoring is more favourable if it is not associated with consciousness disorders. A high rate of NSVT episodes at Holter or the association with syncope can lead to a less favourable prognosis and therefore require pharmacological or electric treatment. The role of ET (electrophysiological test) has not yet been clearly described and is in progress. Recent studies of molecular genetics help to identify high-risk patients. Sustained monomorphic VT is not frequent but w

An assessment of nonconvulsive seizures in the intensive care unit using continuous EEG monitoring: an investigation of variables associated with mortality.

Of 49 patients with nonconvulsive seizures studied with continuous EEG monitoring, the overall mortality was 33% (16/49). Of the 23 patients with nonconvulsive status epilepticus (NCSE), 13 died (mortality 57%). Individual variables significantly associated with mortality were age, presence of NCSE, seizure duration, hospital and NICU length of stay, and delay to diagnosis and etiology (acute illness versus remote symptomatic). With multivariate logistic regression, only seizure duration (p = 0.0057, OR = 1.131/hour) and delay to diagnosis (p = 0.0351, OR = 1.039/hour) were associated with inc

Cortical, subcortical, brainstem and autonomic responses to nociception under total intravenous anesthesia.

BACKGROUND: Physiological responses to nociception are complex and involve intricate associations between the central, peripheral, and autonomic nervous systems. To optimize intraoperative analgesic titration, several monitoring devices have been developed, each targeting specific physiologic variables. However, existing devices primarily focus on isolated components of the nociceptive response, such as autonomic or cortical activity, without integrating these perspectives comprehensively. Our aim was to compare the performance of different nociception monitors in response to standardized teta

How does mindfulness shape affective experience during exercise? A think-aloud study of real-time psychological processes.

Positive affect during exercise is a key predictor of continued participation, yet the mechanisms through which mindfulness-based exercise influences affect remain largely inferred from retrospective, outcome-focused designs. This study used a think-aloud (TA) methodology to capture consciously accessible, moment-to-moment experience during exercise and to test whether a single bout of mindfulness-based exercise alters affect via in-task process shifts. Thirty-six adults were assigned to low or high habitual physical activity groups and completed three counterbalanced sessions of 20-min modera

A survey of anaesthetic management of regional anaesthesia for patients undergoing arthroscopic shoulder surgery in Australia.

BACKGROUND: The Australian and New Zealand College of Anaesthetists recommends standards of monitoring and conduct for regional anaesthesia but does not endorse specific regional techniques. While the interscalene block is the de-facto standard technique, there are numerous established and novel alternatives. Regional block choice and block-related variables (local anaesthetic type, dose, volume, adjuncts) are subject to individual and institutional practices. METHODS: Using a 30-question survey, we aimed to create a snapshot of current regional anaesthesia practice for patients undergoing ar

The use of observational scales to monitor symptom control and depth of sedation in patients requiring palliative sedation: a systematic review.

BACKGROUND: Palliative sedation is the intentional lowering of consciousness of a patient in the last phase of life to relieve suffering from refractory symptoms such as pain, delirium and dyspnoea. AIM: In this systematic review, we evaluated the use of monitoring scales to assess the degree of control of refractory symptoms and/or the depth of the sedation. DESIGN: A database search of PubMed and Embase was performed up to January 2010 using the search terms 'palliative sedation' OR 'terminal sedation'. DATA SOURCES: Retro- and prospective studies as well as reviews and guidelines contain

Noninvasive detection of alarming intracranial pressure changes by auditory monitoring in early management of brain injury: a prospective invasive versus noninvasive study.

BACKGROUND: In brain-injured patients intracranial pressure (ICP) is monitored invasively by a ventricular or intraparenchymal transducer. The procedure requires specific expertise and exposes the patient to complications such as malposition, hemorrhage or infection. As inner-ear fluid compartments are connected to the cerebrospinal fluid space, ICP changes elicit subtle changes in the physiology of the inner ear. Notably, we previously demonstrated that the phase of cochlear microphonic potential (CM) generated by sound stimuli rotates with ICP. The aim of our study was to validate the monito
