Feasibility challenges in protein supplementation research: Insights from the convalescence of functional outcomes after intensive care unit stay in a Randomised Controlled Trial.

BACKGROUND & AIMS: Dietary protein supplementation may benefit physical outcomes in post-intensive care unit (ICU) patients suffering ICU-acquired weakness (ICU-AW). This study examines the impact of a six-week protein supplementation compared to an isocaloric carbohydrate on physical functioning outcomes in post-ICU patients with a follow-up of 12 weeks after ICU discharge. This paper presents descriptive data, feasibility outcomes, and the barriers faced while conducting this nutritional intervention study in post-ICU patients. METHODS: This two-arm, randomised, double-blind controlled inte

Zinc during and in convalescence from diarrhea has no demonstrable effect on subsequent morbidity and anthropometric status among infants <6 mo of age.

BACKGROUND: Preventing illness and improving growth in the first 6 mo of life is critical to reducing infant mortality. Zinc given for 14 d at the start of diarrhea has been shown to decrease the incidence and prevalence of diarrhea and pneumonia and improve growth in the 2-3 mo after, but no trial has been done in infants <6 mo of age. OBJECTIVE: This study sought to assess the effect of 14 d of zinc supplementation on subsequent morbidity and growth among infants 1-5 mo of age living in Pakistan, India, and Ethiopia. DESIGN: Infants with acute diarrhea were randomly assigned to receive zin

COVID-19 progression and convalescence in common variable immunodeficiency patients show dysregulated adaptive immune responses and persistent type I interferon and inflammasome activation.

Common variable immunodeficiency (CVID) is the most prevalent primary immunodeficiency, marked by hypogammaglobulinemia, poor antibody responses, and increased infection susceptibility. The COVID-19 pandemic provided a unique opportunity to study the effects of prolonged viral infections on the immune responses of CVID patients. Here we use single-cell RNA-seq and spectral flow cytometry of peripheral blood samples before, during, and after SARS-CoV-2 infection showing that COVID-19 CVID patients display a persistent type I interferon signature at convalescence across immune compartments. Alte…

Widespread Molecular Imprints in the Serum Proteome of COVID-19 Convalescents Uncovering Immune System Sequelae.

Post-COVID-19 sequelae have become an emerging global health issue, but the mechanisms for the sustained susceptibility of convalescents to the sequelae remain poorly understood. Here we report the use of a restricted open-search approach to explore the molecular imprints of SARS-CoV-2 infection left on the proteome of 412 COVID-19 patients and convalescences. A total of 827 non-standard amino acid variations, chemically modified residues as well as post-translational modifications, termed non-coded amino acids (ncAAs), are found spreading over 29,814 sites in patient's serum proteins. Markedl…

C3d Complement Activation and Antibody Immunity in Convalescent COVID-19: Insights From Adults and Children.

Following severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, the complement pathway becomes activated, generating the stable split product C3d. Although C3d can modulate B cell responses through complement receptor 2 (CR2) in experimental systems, it remains unclear how circulating C3d relates to antibody responses after SARS-CoV-2 infection in humans. Here, we examined plasma C3d levels and their associations with SARS-CoV-2-specific antibody responses and clinical characteristics in convalescent adults and children. In adults, older age, male sex, and a history of severe

Convalescent plasma for persistent symptomatic COVID-19 infection in immune-suppressed patients - a case series.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection progresses from an initial direct viral cell injury to an immune-mediated response. In most cases, the immune response results in viral clearance and recovery after a mild disease. Severe infections seem to result mainly from uncontrolled immune-mediated damage. Accordingly, initial treatment involves an antiviral strategy, while late severe infection is treated with anti-inflammatory medication. Immunocompromised patients have reduced vaccine efficacy and reduced ability to mount an appropriate immune response to SARS-CoV-

Blood Banking Capacity in Low-and Middle-Income Countries: Covid-19 Convalescent Plasma in Context.

Blood transfusion capacity in low- and middle-income countries (LMICs), encompassing both the safety and adequacy of the blood supply, is limited. The challenges facing blood banks in LMICs include regulatory oversight, blood donor selection, collection procedures, laboratory testing, and post-transfusion surveillance. A high proportion of LMICs are unable to fully meet clinical demands for blood products, and many do not meet even the minimum threshold of collection (10 units per 1000 population). Suboptimal clinical transfusion practices, in large part due to a lack of training in transfusio

Development and validation of a clinical prediction model for concurrent pulmonary infection in convalescent patients with intracerebral hemorrhage.

OBJECTIVES: This study aimed to develop and validate a clinical prediction model for assessing the risk of concurrent pulmonary infection (PI) in patients recovering from intracerebral hemorrhage (ICH). METHODS: In this retrospective study, we analyzed clinical data from 761 patients in the subacute recovery phase of ICH, of whom 504 developed PI and 257 did not. Univariate logistic regression was initially used to identify potential risk factors, followed by variable selection through the Least Absolute Shrinkage and Selection Operator (LASSO) regression. Predictors selected by LASSO were en

Efficacy of convalescent plasma for the treatment of COVID-19 in lung transplant recipients: A multicenter French study.

INTRODUCTION: Lung transplant (LT) recipients are at greater risk of complications from COVID-19. Treatment options are limited partly due to interactions with immunosuppressive agents. Convalescent plasma (CP) is a potential treatment option, but it has not been extensively studied in LT patients. We aimed to assess the efficacy and safety of CP use in France for COVID-19 infected LT patients. MATERIAL AND METHODS: We retrospectively recruited LT patients followed up in the 10 French LT centers, older than 18 years, infected with SARS-CoV-2 between the pandemic onset and July 1, 2023, and tr

Association between oral health status and functional independence measure on admission in convalescent hospitalized patients.

BACKGROUND: Oral health management has become increasingly important for acute inpatients. Older patients often require extended periods of medical care, and oral health management is necessary in the convalescent period following the acute period. During the convalescent period, oral health management remains unclear as convalescent hospitals have limited dental resources, and effective dental care must be provided if the objective of hospitalization is to improve life functions. This study aimed to clarify the relationship between daily functioning and oral health status at the time of admis

An online survey among convalescents 5 months post SARS-CoV-2 infection in China.

The effects of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection persist months and years after recovery. We conducted an online survey to assess the health condition of convalescents approximately 5 months following the primary infection of SARS-CoV-2. The study recruited 5,510 individuals who were primary infected, 626 participants who had experienced reinfection, and 521 participants who were without infective history. The most common disorders after the primary infection group were fatigue (15.18 %), memory issue (13.13 %), post-exertional malaise (PEM, 11.68 %), and b

Convalescent Growth Differentiation Factor-15 and Long-Term Outcomes after an Acute Coronary Syndrome.

BACKGROUND: Growth differentiation factor-15 (GDF-15) has been shown to be associated with adverse clinical outcomes in patients after an acute coronary syndrome when measured soon after an event. Although dynamic in the acute phase after myocardial injury, GDF-15 has been shown to remain stable during convalescence. In this study, we aimed to assess the value of GDF-15 as a long-term prognostic marker for clinical outcomes when measured in the convalescent phase following an acute coronary syndrome. METHODS: GDF-15 concentrations were measured in 1945 patients who were recruited between 2002…

SARS-CoV-2 Persistence in Immunocompromised Patients Requiring Treatment With Convalescent Plasma: A Case Report.

Severe acute respiratory syndrome-2 (SARS-CoV-2) infection in immunocompromised patients presents a challenge, as patients with such conditions may have severe courses. Identifying modalities to shorten the course or lessen the severity of infection could be potentially beneficial. A 76-year-old male with follicular lymphoma on rituximab and lenalidomide presented with COVID-19 pneumonia requiring intensive care unit (ICU) level care for persistent hypoxemia. He was treated with an extended course of remdesivir, as recommended by the Infectious Diseases service, but he maintained a persistentl

Differences in brain activation and connectivity during unaffected hand exercise in subacute and convalescent stroke patients.

Patients experiencing severe hemiplegia following a stroke struggle to rehabilitate their affected limbs. Cross-education (CE) training emerges as a promising rehabilitation method due to its safety, simplicity, low risk, and ability to effectively improve muscle strength in the affected limb. However, controversy surrounds the neural mechanisms and clinical applications of CE. To address this, we employed functional near-infrared spectroscopy to monitor the response of regions of interest (ROI) and functional connectivity in patients with stroke experiencing severe hemiplegia during one sessi
