Orbitocranial penetration by a fern: case report.

OBJECTIVE AND IMPORTANCE: Cranial and orbitocranial penetration by organic foreign material is not infrequent. It is important to identify whether penetration has occurred and to localize and remove the organic foreign material. CLINICAL PRESENTATION: We report a 15-month-old patient who suffered orbitocranial penetration with the stem of a fern. The stem passed through the orbit, exiting via the superior orbital fissure. It continued through the middle cranial fossa to end in the posterior cranial fossa. INTERVENTION: The patient underwent surgery, and a modified Dolenc procedure was perfor

Unilateral absence of foramen spinosum with bilateral ophthalmic origin of the middle meningeal artery: case report and review of the literature.

Bilateral ophthalmic origin of the middle meningeal artery with an unilateral absence of foramen spinosum has not yet been described. We report on a skull with endocranial meningeal grooves indicating bilateral ophthalmic origin of the middle meningeal artery, however, its branches were normal both in their position and distribution. In addition, a rare venous sinus variation was present unilaterally - a sinus of Hyrtl. Imaging identification of the anomalous origin of the middle meningeal artery is important while planning surgical and endovascular interventions in the middle cranial fossa an

Traumatic middle meningeal artery and fistula formation with the cavernous sinus: case report.

BACKGROUND: In this study, we present a young patient with traumatic middle meningeal fistula treated with detachable balloon through the middle meningeal artery. CASE DESCRIPTION: The case of a 22-year-old man with exophthalmos due to an arteriovenous fistula between the middle meningeal artery and the cavernous sinus at the base of the middle cranial fossa is reported. Complete embolization was performed with a detachable balloon and resulted in complete relief of the symptoms. CONCLUSION: The middle meningeal fistula can be caused by head trauma; selective external carotid angiogram is ne

Anatomy and significance of the temporal fat pad in Apert syndrome.

Abnormal temporal region anatomy in Apert syndrome described in the literature includes inferior and lateral displacement of the middle cranial fossa, effacement of the temporal fossa, and thinning of the temporalis muscle. Four patients with Apert syndrome were noted to have hyperplasia of the bilateral superficial temporal fat pads, which could be contoured surgically. A case is presented with reformatted three-dimensional computed tomography (3-D CT) scans with intraoperative documentation of increased temporal fat as compared to normal controls.

Transmandibular exposure of the skull base.

The combined transmandibular-transcervical approach to the skull base ensures a wide field exposure to the lateral and midline compartments of the middle cranial fossa with attendant vascular control. Splitting the lip and mandible in the midline and dividing the floor of the mouth structures along the lateral border of the tongue exposes the parapharyngeal space, infratemporal fossa (lateral compartment), and clivus, nasopharynx, and cervical spine (midline compartment). A variety of benign and malignant intracranial and extracranial skull base lesions have been treated using this approach.

Basic Imaging of Skull Base Trauma.

Skull base fractures extend through the floor of the anterior, middle, or posterior cranial fossa. They are frequently associated with complex facial fractures and serious complications such as cranial nerve or vascular injury, cerebrospinal fluid leak, or meningitis. Several distinct patterns of skull base fractures have been recognized, each of them associated with different complications. Recognition of, often subtle, skull base fracture is essential to prevent or allow early treatment of these serious complications.

Sphenoidal artery, ramus orbitalis persistens and pterygospinosus muscle--a unique cooccurrence of first branchial arch anomalies in man.

In a 76-year-old Caucasian male, the right middle meningeal was replaced by a branch of the ipsilateral ophthalmic artery (O), i.e. the sphenoidal artery (S); the right foramen spinosum was found to be absent. In the orbit, the right O bifurcated into a medial (merely orbital) and a lateral division, i.e. a meningolacrimal trunk. From that trunk, the S branched off and entered the middle cranial fossa at the superolateral angle of the superior orbital fissure. In addition, an anastomotic channel was formed by a remnant of the ramus orbitalis, connecting the right infraorbital with the deep orb

A systematic approach to the surgical management of acoustic neuroma.

Contemporary otomicrosurgical techniques have made total removal of acoustic tumor with preservation of the seventh and sometimes the eighth cranial nerves possible. The four approaches currently used in acoustic tumor surgery are the middle cranial fossa, the translabyrinthine, the suboccipital, and the combined translabyrinthine-suboccipital. This review examines the surgical results in the removal of more than 600 acoustic tumors and outlines a rationale for the choice of approach. Tumor size on computed tomographic scan and auditory reserve establish the parameters used in planning the sur

Anatomy of the Orbit.

The orbit is a paired, transversely oval, and cone-shaped osseous cavity bounded and formed by the anterior and middle cranial base as well as the viscerocranium. Its main contents are the anterior part of the visual system, globe and optic nerve, and the associated neural, vascular, muscular, glandular, and ligamentous structures required for oculomotion, lacrimation, accommodation, and sensation. A complex stream of afferent and efferent information passes through the orbit, which necessitates a direct communication with the anterior and middle cranial fossae, the pterygopalatine and infrate

The lateral retrocanthal transorbital endoscopic approach to the middle fossa: cadaveric stepwise approach and review of quantitative cadaveric data.

OBJECTIVE: The lateral retrocanthal transorbital endoscopic approach (LRCTEA) facilitates trajectory to the middle fossa, preserving the lateral canthal tendon and thus avoiding postoperative complications such as eyelid malposition. Here, the authors sought to define the surgical anatomy and technique of LRCTEA using a stepwise approach in cadaveric heads and offer an in-depth examination of existing quantitative data from cadaveric studies. METHODS: The authors performed LRCTEA to the middle cranial fossa under neuronavigation in 7 cadaveric head specimens that underwent high-resolution (1-

Selective Microsurgical Disconnection of Greater Sphenoid Wing Dural Arteriovenous Fistula Through the Subtemporal Approach Under Exoscope Magnification.

OBJECTIVE: Sphenoid dural arteriovenous fistulas located on the greater sphenoid wing carry a notable risk of intracranial hemorrhage due to the frequent presence of cortical venous reflux. Because of the challenging embolization procedure, microsurgery remains a valuable option in these cases. METHODS: In most cases, the fistula is located in the anterolateral endocranial surface of the middle cranial fossa just below the sphenoid ridge; however, in a small subset of cases, the fistula is positioned posteriorly and medially in the region between the foramen ovale and rotundum. The former reg

Iatrogenic traumatic brain injury: penetration of Kirschner's knitting needle into the middle cranial cavity.

Traumatic penetrations of foreign objects into the craniocerebral cavity are often encountered in the department of emergency and traumatology. A 5-year-old child was brought to the department of pediatric neurosurgery with a severe headache and fatigue. On admission, the patient had initial neurologic examinations and radiologic scans. The consciousness assessment by Glasgow Coma Scale was 13. Neuroradiologic studies revealed a long hyperdense object extending from the extracranial cavity into the middle cranial fossa. A thorough history was obtained with attention to how and when the injury

Spontaneous tegmen defect and semicircular canal dehiscence: same etiopathogenic entity?

OBJECTIVES: To describe the clinical and radiologic features of associated spontaneous tegmen defects (STDs) with semicircular canal dehiscences (SCCDs) and to postulate a novel etiopathogenic hypothesis of these pathologic conditions. METHODS: Medical records of all patients with surgically confirmed STD between 2001 and May 2010 were reviewed. We excluded all secondary tegmen defects. Clinical, audiological, and radiologic data were analyzed. RESULTS: Twenty-three patients matched the inclusion criteria. Semicircular canal dehiscence was associated to STD in 13 patients. Of these patients,
