Chronic pain and major depressive disorder in the general population.

This study aims (1) to assess the prevalence of Chronic Painful Physical Condition (CPPC) and major depressive disorder (MDD) in the general population; (2) to evaluate their interaction and co-morbidity with sleep and organic disorders; and (3) to investigate their daily functioning and socio-professional consequences. A random sample of 3243 subjects (18years), representative of California inhabitants, was interviewed by telephone. CPPC duration was at least 6months. Frequency, severity, duration and consequences on daily functioning, consultations, sick leave and treatment were investigated…

Proton magnetic resonance spectroscopy of late-life major depressive disorder.

The primary goal of this study was to examine the biochemical abnormalities of late-life major depression by using 3-tesla (3-T) proton magnetic resonance spectroscopy ((1)H-MRS). The antidepressant effects on the biochemical abnormalities were investigated as well. Study participants were 27 elderly patients with major depressive disorders (among which 9 were on antidepressant medication) and 19 comparison elderly subjects. (1)H-MRS spectra were acquired from voxels that were placed in the left frontal white matter, left periventricular white matter, and left basal ganglia. Ratios of N-acetyl…

Major depressive disorder in children and adolescents after renal transplantation.

This cross-sectional study evaluated the prevalence of major depressive disorder and depressive symptoms in children and adolescents after renal transplantation. A total of 71 patients who had undergone renal transplantation were interviewed in person using the Farsi (Persian) version of the Kiddie Schedule for Affective Disorders and Schizophrenia and Diagnostic and Statistical Manual of Mental Disorders diagnostic criteria. Major depressive disorder, depressive symptoms, and suicidal behaviors were assessed. The rate of major depressive disorder was 2.8%; two-thirds of the patients had irrit…

Women and major depressive disorder: clinical perspectives on causal pathways.

BACKGROUND AND AIMS: Epidemiological data on the prevalence of mood disorders demonstrate that major depressive disorder (MDD) is approximately twice as common in women as in men and that its first onset peaks during the reproductive years. We aimed to review key social, psychological, and biological factors that seem strongly implicated in the etiology of major depression and to focus on sex-specific aspects of depression, such as the role of a woman's reproductive life cycle in depressive symptomatology. METHODS: A review of the literature, from 1965 to present, was conducted. RESULTS: An …

Similar effects of tramadol and venlafaxine in major depressive disorder.

The analgesic tramadol has many characteristics in common with the antidepressant venlafaxine. The drugs are structurally similar, share both serotonergic and noradrenergic properties, and undergo a similar metabolic fate. In this study, a patient, who developed significant depression following cessation of tramadol after several years of therapy, is described. Her depression was then treated with venlafaxine with excellent response. It appears that tramadol may have provided a prophylactic antidepressant effect in this patient. Because of its similarities to venlafaxine, tramadol may possibly

Brain structural correlates of depressive comorbidity in obsessive-compulsive disorder.

The high comorbidity of obsessive-compulsive disorder (OCD) with major depressive disorder (MDD) suggests common neurobiological substrates. We assessed the contribution of lifetime MDD to brain structural alterations in OCD using magnetic resonance imaging. OCD patients with (n=33) or without (n=39) lifetime MDD, and 72 control subjects were assessed. Comparative region of interest (ROI) analyses assessed the contribution of lifetime MDD to gray matter volume alterations in OCD patients. Interregional correlations of gray matter volume were also examined and voxelwise analyses were performed …

A continuity between bipolar II depression and major depressive disorder?

BACKGROUND: A recent series of studies has questioned the current categorical split of mood disorders into bipolar and depressive disorders. Mixed states, especially mixed depression (i.e., depression plus co-occurring, noneuphoric, hypomanic symptoms) might support a continuity between bipolar II (BP-II) depression and major depressive disorder (MDD). The aim of the study was to assess the distribution of intradepressive hypomanic symptoms rating between BP-II and MDD depressions. A bi-modal distribution would support a categorical distinction, and no bi-modality would support continuity. ME…

Major depressive disorder and health care costs in multiple sclerosis.

OBJECTIVE: Multiple Sclerosis (MS) is associated with elevated levels of depressive symptoms and an elevated frequency of depressive disorders. Depressive disorders, in general, are associated with substantial direct and indirect economic costs, and have been shown to increase the costs associated with the management of medical conditions in a variety of clinical settings. However, the impact of depressive disorders on costs associated with MS have not been evaluated. The objective of this study was to evaluate this association. METHODS: The Composite International Diagnostic Interview (CIDI)…

Incidence of major depressive disorder and dysthymia in young adolescents.

OBJECTIVE: An epidemiological study conducted between 1987 and 1989 in a single school district in the southeastern United States investigated the incidence, transition probabilities, and risk factors for major depressive disorder (MDD) and dysthymia in adolescents aged 11 to 16 years. METHOD: Diagnoses were based on the Schedule for Affective Disorders and Schizophrenia for School-Age Children, which was administered to 247 mother-adolescent pairs at 12-month intervals. RESULTS: One-year MDD and dysthymia incidences were 3.3% (n = 11) and 3.4% (n = 9), respectively. Transition probabilities…

Development of screeners for depressive disorders and substance disorder history.

If screeners are going to be widely incorporated in clinical care to increase the detection of common psychiatric disorders, they need to be brief, easy to score, and sensitive across diverse patient populations. Few screeners exist that meet these criteria. This problem is addressed in this study by identifying a subset of questions from the Diagnostic Interview Schedule (DIS), which predicted simultaneously obtained DIS diagnoses. A two-item screener to detect depression or dysthymia within the last year and three-item screeners for lifetime drug disorders and alcohol disorders were tested i

Depressed mood and major depressive disorder in two ethnic groups.

This study examines the relation of depressive mood, measured using the Center for Epidemiologic Studies Depression scale, with major depression, assessed using the Diagnostic Interview Schedule. Data are from 1244 Mexican American and 1149 non-Hispanic white randomly-selected community residents. Major depression is strongly related to depressed mood, irrespective of the persistence or content of mood, with some components of depressed mood (negative affect and somatic disturbance) more strongly associated with major depression than other components (lack of positive affect and interpersonal

Depressive disorder associated with physical illness. The impact of stroke.

The consultation-liaison psychiatrist is confronted time after time with depression in the context of physical illness, and the link between the two is of practical and theoretical interest. In addressing this topic, current classification systems are ambiguous. The advent of research diagnostic criteria and the primary/secondary depression dichotomy have not clarified the problem as was hoped. The conventional view that physical illness only precipitates depression in the genetically predisposed has been challenged by recent studies in patients with stroke. These studies point to a specific r

Psychosocial predictors of outcome in subjects with untreated depressive disorder.

Untreated community volunteers (n = 66) with a significant reactive or neurotic depressive disorder were assessed at interview to determine predictors of improvement at 6 and at 20 weeks. The degree to which subjects improved by 6 weeks could be predicted by their pattern of improvement one week after initial interview and more definitely at the third week. Key baseline predictors of improvement both at 6 and at 20 week were the break-up of an intimate relationship in the preceding 12 months and weight loss, replicating findings in a study of depressive patients receiving psychiatric assessmen…

Atypical somatoform disorder following infection in children--a depressive equivalent?

Thirteen cases are reported wherein, following an upper respiratory infection or gastroenteritis, children developed a syndrome involving predominantly somatic complaints and objective signs of dysphoria. The syndrome picture was strikingly protracted compared to the convalescence period that would be expected following transient infectious illnesses. Psychiatric consultation was requested primarily because of extensive nonconclusive medical evaluation by the primary care physician. Most of the children showed a striking inability to recognize or acknowledge dysphoria. A multifactorial etiolog

The risk for early-adulthood anxiety and depressive disorders in adolescents with anxiety and depressive disorders.

BACKGROUND: Various studies find relationships among anxiety and depressive disorders of adolescence and adulthood. This study prospectively examines the magnitude of longitudinal associations between adolescent and adult anxiety or depressive disorders. METHODS: An epidemiologically selected sample of 776 young people living in upstate New York received DSM-based psychiatric assessments in 1983, 1985, and 1992 using structured interviews. The magnitude of the association between adolescent and adult anxiety or depressive disorders was quantified using odds ratios generated from logistic regr…

Frontal alpha asymmetry across genders as a distinction between major depressive disorder and bipolar disorder.

BACKGROUND: Bipolar disorder (BD) and major depressive disorder (MDD) impose a significant burden worldwide. Misdiagnosis between BD and MDD has long been a challenging issue. Frontal alpha asymmetry (FAA) is one of the most widely studied electroencephalogram (EEG) markers in mental health research. This study utilizes a relatively large sample size to investigate whether FAA can effectively differentiate BD from MDD. METHODS: We enrolled 149 participants aged 18-65 years, including 68 with MDD, 51 with BD, and 30 healthy control participants. Resting-state EEG was recorded for 5 min each un…

Intrusive thoughts and memories in adolescents with major depressive disorder or post-traumatic stress disorder.

OBJECTIVES: Research in adults suggests that intrusive memories and intrusive thoughts (often referred to as intrusive cognitions) are common in members of the general population and are often seen in clinical disorders. However, little is known about the experience of intrusive cognitions in adolescents, particularly in adolescents with major depressive disorder (MDD) and post-traumatic stress disorder (PTSD). The present study sought to gather fundamental data on these phenomena (i.e., frequency, characteristics and appraisals of intrusive cognitions) in adolescents with MDD and PTSD. METHO…

The clinical presentation of major depressive disorder in youth with co-occurring obsessive-compulsive disorder.

BACKGROUND: Major depressive disorder (MDD) is common in youth and among the most frequent comorbid disorders in pediatric obsessive-compulsive disorder (OCD), but it is unclear whether the presence of OCD affects the symptom presentation of MDD in youth. METHODS: A sample of youth with OCD and MDD (n = 124) and a sample of youth with MDD but no OCD (n = 673) completed the Patient Health Questionnaire for Adolescents (PHQ-A). The overall and symptom-level presentation of MDD were examined using group comparisons and network analysis. RESULTS: Youth with MDD and OCD, compared to those with MD…
