Pharmacokinetics of doxepin in subjects with pruritic atopic dermatitis.

BACKGROUND: Doxepin applied topically by itself or in combination with triamcinolone acetonide is a safe and effective treatment for atopic dermatitis. OBJECTIVE: We evaluated the pharmacokinetic profile of doxepin and desmethyldoxepin after topical application of doxepin hydrochloride 5% cream alone or in combination with 0.025% triamcinolone acetonide (doxepin/TAC). METHODS: Twenty-four subjects with atopic dermatitis received either doxepin or doxepin/TAC cream 4 times daily for 7 days in a randomized, double-blind, controlled trial. Serum samples were obtained and pharmacokinetic paramet

Allergic skin disease: atopic dermatitis as a prototype.

The skin is the largest immune organ in the body. It is strategically positioned as an interface between a hostile antigenic world and a complex immune system characterized by inflammatory cells, mediators, lymphocytes, and a panoply of cytokines. These cytokines act as subcellar messengers that direct effector lymphocytes, eosinophils, mast cells, and a host of other cells to target the skin in allergic inflammation. Just as asthmas and allergic rhinitis are prototypes of IgE-mediated respiratory disease, atopic dermatitis is the prototype allergic skin disease. This article focuses on an und

Production of antibodies to staphylococcal superantigens in atopic dermatitis.

Staphylococcal superantigens (SAG) are implicated in the inflammation of atopic dermatitis. As SAG mediated diseases may be modified by specific antibodies, the antibody response to SAG in atopic dermatitis was investigated. Immunoglobulin (Ig) G to staphylococcal enterotoxin A (SEA), staphylococcal enterotoxin B (SEB), and toxic shock syndrome toxin 1 (TSST-1) were measured by sandwich enzyme linked immunosorbent assay (ELISA) in 74 children with atopic dermatitis and 111 controls. Controls had detectable IgG to SEA, SEB, and TSST-1, which increased with age. Atopic dermatitis subjects had an

Hypnotherapy as a treatment for atopic dermatitis in adults and children.

Eighteen adults with extensive atopic dermatitis, resistant to conventional treatment, were treated by hypnotherapy, with statistically significant benefit (P < 0.01) measured both subjectively and objectively, which was maintained at up to 2 years where results were available. Twenty children with severe, resistant atopic dermatitis were treated by hypnosis. All but one showed immediate improvement, which was maintained at the following two clinic appointments. In 12 children, replies to a questionnaire at up to 18 months after treatment, showed that 10 had maintained improvement in itching a

[Clinical implications of sensitization to inhalants in atopic dermatitis].

In 104 children with atopic dermatitis (AD) and RAST tests for 2 inhalant and 9 food allergen mixtures, RAST results, severity of skin involvement and presence of respiratory symptoms by history were analyzed in addition to total serum IgE values available in 66 children. Overall frequency of sensibilization was high (74%) with more positive RAST results for foods than inhalants, although of the latter the only two tested were among the more frequently positive. When the subjects positive for inhalants were considered alone, the frequency of respiratory symptoms was significantly higher in tho

The immunopathogenic role of food hypersensitivity in atopic dermatitis.

Food hypersensitivity is reported to play an immunopathogenic role in atopic dermatitis in approximately one-third of children. In 320 selected children with moderate to severe atopic dermatitis, 63% of children were found to have food hypersensitivity by double-blind placebo-controlled food challenges. Both IgE-mediated mast cell and mononuclear cell activation appear responsible for the eczematous lesions resulting from ingestion of food allergens.

Study of circulating immune complexes in atopic dermatitis.

The authors examined sera from 92 patients (78 adults, 14 children) with atopic dermatitis (AD), for the presence of circulating immune complexes (CIC). Using the PEG precipitation technique they found an increase in CIC total protein. In the quantity of precipitated proteins, elevated IgA and IgG levels and decreased C3 in CIC were measured. Neither the acute nor the subacute stage of the disease, nor skin involvement, correlated with CIC. In increased content of CIC was found in patients who had AD with associated disease recurrent infections, recurrent conjunctivitis, asthma, allergic rhini

Nickel sensitivity and the course of atopic dermatitis in adulthood.

The course of dermatitis was followed in nickel-sensitive and nickel-negative atopic and non-atopic patients. Manifest dermatitis was seen in 70% of the nickel-allergic and in 64% of the nickel-negative female atopic dermatitis (AD) patients. Those atopic subjects who had minor symptoms in their teens suffered more from dermatitis if they had developed nickel allergy (p less than 0.025). Hands and the head region were the most common sites for current dermatitis in both groups.

Behavioural treatment of scratching in patients with atopic dermatitis.

A behavioural method of habit reversal, in combination with a hydrocortisone cream, was compared with the use of cream alone in the treatment of 17 patients with atopic dermatitis. The patients were assigned randomly to two groups, one of which received the combination treatment and the other regular ointment treatment. The patients' skin status was assessed before and after treatment, and the patients recorded their scratching during the study. Both groups improved, but the group which received habit-reversal therapy improved significantly more. A strong correlation was found between reductio

Atopic dermatitis. Clinical spectrum and approach to treatment.

The fact that atopic dermatitis is such a common medical problem underlies the need for better understanding of the basic disease processes and the development of new therapeutic approaches. Infection is now recognized as an important exacerbating factor, and the role of inflammatory and immunologic mediators is receiving scrutiny. The development of potent topical corticosteroids has provided a new component in the therapeutic armamentarium that previously included only cool soaks, menthol or phenol, tars, antihistamines, and combinations of these. Appropriate and aggressive therapy given ear

T and B lymphocytes in contact and atopic dermatitis.

Twenty patients with contact dermatitis, eighteen with atopic dermatitis, two with mixed dermatitis, and 20 controls were examined for the number of T and B lymphocytes, and serum concentrations of IgD and IgE. Significantly higher counts of B lymphocytes bearing IgE and high serum IgE values were found in the atopic group. No other significant differences were found. In particular, we found a normal frequency of IgD bearing lymphocytes in contact dermatitis, and normal T lymphocyte counts in both groups of patients.

Atopic dermatitis and beyond: Staphylococcus aureus skin adaptation.

Chronic skin conditions are natural experiments in within-host bacterial evolution. Taking atopic dermatitis as the main example, we discuss how studies starting from collections of clinical skin isolates identify Staphylococcus aureus variants that arise independently in different patients, and how this convergence can prioritize candidates for functional genetics.

Atopic dermatitis: pathogenesis, immunology, microbiome, and salivary biomarkers.

Atopic dermatitis is a chronic inflammatory skin disease influenced by genetic predisposition, immune dysregulation, and environmental factors, as well as skin and gut microbiota. This review summarises the key mechanisms involved in this disease development, including disturbances in innate and adaptive immune responses, ethnic differences in immune pathways, and the contribution of microbial imbalance to skin barrier dysfunction. Special attention is given to salivary biomarkers-such as cortisol, melatonin, alpha-amylase, chromogranin A, C-reactive protein, and immunoglobulin A-which offer a

Atopic dermatitis reconsidered: Clinical mimics and diagnostic pearls.

Atopic dermatitis (AD) is a chronic, pruritic inflammatory skin condition with a complex pathogenesis and variable clinical presentation, often overlapping with other dermatologic diseases. This article considers the differential diagnoses for AD, emphasizing the importance of accurately distinguishing it from both common and rare conditions. Among inflammatory dermatoses, seborrheic dermatitis, psoriasis, allergic and irritant contact dermatitis, lichen simplex chronicus, and pityriasis rosea share overlapping features but can be differentiated based on history, distribution, and morphology.

Iron deficiency in dogs suffering from atopic dermatitis.

BACKGROUND: Iron-deficiency is associated with increased morbidity and mortality in non-communicable diseases. However, iron parameters are rarely assessed in dogs. Here, we aimed to assess and correlate iron parameters in dogs suffering from Canine Atopic Dermatitis (CAD) compared to non-atopic, healthy dogs. RESULTS: For this retrospective study, blood values and sera of 34 dogs with confirmed CAD were compared with 94 healthy non-atopic dogs. In our cohort, dogs with CAD had significantly lower mean corpuscular volume (MCV, ) mean corpuscular hemoglobin (MCH) but higher white blood cell co

Eczema Herpeticum Complicating Atopic Dermatitis: A Rare Presentation.

Atopic dermatitis (AD) has become a global health concern due to an increase in its frequency over the past few decades. This illness not only reduces the quality of life but also imposes a considerable financial burden due to the increased risk of skin infections. This case report explores the presentation of a four-month-old male infant with a personal history of atopic dermatitis that developed yellow scaly lesions on the scalp, which were assumed to be cradle cap. However, there was a clinical worsening of the cutaneous lesions, with the appearance of vesicles, so he was referred to the Pe
