The frequency and features of joint involvement were evaluated in 29 patients with polymyositis/dermatomyositis (PM/DM); eight of them (27.5%) presented arthritis. Mean age was 30 years, and mean disease duration was 5.3 years. Oligoarthritis was observed in 5 cases, while 3 presented polyarthritis; in most cases arthritis was concurrent with the musculocutaneous picture. On comparing the 8 arthritis patients with the remaining 21, no significant clinical or serological differences were found. However, males predominated in the arthritis group, 75% vs 19% (p < 0.05). Arthritis responded favour…

Autoantibodies are found in most patients with polymyositis (PM) or dermatomyositis (DM) and 35-40% of these patients have myositis-specific antibodies. Twenty-five to thirty percent have anti-aminoacyl-tRNA synthetases, of which anti-Jo-1, directed at histidyl-tRNA synthetase, is by far the most common. Patients with anti-synthetases have a high frequency of myositis, interstitial lung disease, Raynaud's phenomenon, and other features constituting an "anti-synthetase syndrome." Anti-synthetases tend to react with conformational epitopes and to inhibit enzymatic activity, suggesting reaction w…

Cardiac involvement in dermatomyositis has been well described; myocarditis and cardiac arrhythmias are the most frequent manifestations. An 8-year-old girl is presented, who developed pericardial tamponade in the course of the disease. It is the first time this association has been reported.

A muscle biopsy established the diagnosis of dermatomyositis in a 22-year-old woman with normal muscle enzyme levels, 18 months after she presented with constitutional symptoms, a rash and polyarthritis. Her subsequent course was complicated by cutaneous ulcers with histological changes suggesting vasculitis, interstitial pulmonary involvement and recurrent pneumomediastinum, the latter being recently reported in 2 other cases of dermatomyositis. Cutaneous vasculitis has been present in all cases and it is suggested that the pneumomediastinum may be due to pulmonary vasculitis.

The authors present the case history of an 11-year-old girl who suffered from the rarely occurring grave juvenile dermatomyositis associated with recurring pancreatitis. Factors which may cause pancreatitis: juvenile dermatomyositis as underlying disease, steroid-, cytostatic- and thiazide diuretic treatment are analysed.

A 10-year-old girl from southern Alberta, Canada, who had close contact with cats, developed typical features of dermatomyositis. The diagnosis was confirmed by muscle biopsy. A toxoplasmosis titer was 1:16,384 by indirect fluorescent antibody technique, and the IgM response to toxoplasma was positive. Only minimal improvement followed prednisone and azathioprine administration, but she rapidly improved after 4 weeks of treatment for toxoplasmosis with pyrimethamine and sulfadiazine. A year after the onset of dermatomyositis, she showed no weakness or cutaneous lesions, and a repeat muscle bio…

Eight Australian cases of D-penicillamine induced polymyositis/dermatomyositis (PM/DM) are reported. In terms of clinical, pathological and electromyographic features, D-penicillamine PM/DM is similar to idiopathic PM/DM but is generally less severe. Recovery is usually rapid when D-penicillamine is withdrawn. Sera were available for study in 6 of the 8 reported cases. Two of the 6 had elevated titers of acetylcholine receptor autoantibodies. Neither of these patients had clinical signs of myasthenia gravis. In 3 of 6 patients typed for C2, no bands were detected suggesting homozygous C2 defic…

Sixty-six patients with possible juvenile dermatomyositis (JDMS) were observed at the Children's Hospital of Los Angeles from 1960 to 1982. In patients initially given high doses of corticosteroids followed by low-dose therapy, three different clinical courses had previously been observed: monocyclic, polycyclic, and chronic continuous. We reviewed the records of 32 patients who met study criteria. The course of JDMS was monocyclic in eight children, chronic polycyclic in 10, and chronic continuous in 14. Of these children, 25 are well and not receiving medication; one has mild JDMS, without c…

An eleven year old child with dermatomyositis and calcinosis cutis universalis is presented. She showed heliotrope bloating of eyelids, Gottrons sign, proximal muscle wasting with contractures and extensive areas of calcification over the shoulder, pelvic girdles and proximal extremities. The dignosis was confirmed by biochemical and histopatholo "cal studies as 91 well as electromyography. High doses of steroids and supportive measures made the patient ambulatory.
