Future perspectives and research initiatives in fast-track surgery.

BACKGROUND AND AIMS: Major surgery is still followed by a risk of morbidity, a need for hospitalisation and convalescence. Fast-track surgery has been introduced as a coordinated effort to combine unimodal evidence-based principles of care into a multi-modal effort to enhance recovery. The aim of this article was to update recent data on fast-track abdominal surgery and outline future strategies for research. RESULTS: The data from fast-track colonic resection support the validity of the concept because pain, ileus, cardiopulmonary function and muscle function were all improved, compared with

Gastrointestinal bleeding in emergency setting: two cases of intestinal angiodysplasia and unusual conservative surgical treatment.

The authors report two cases of gastrointestinal bleeding in an emergency setting caused by angiodysplasia of colon and small bowel. They stress the rarity of the lesion and consider the difficulties involved in obtaining a preoperative diagnosis. The optimal management is uncertain and depends on the severity and rate of bleeding. A conservative medical approach is indicated for many patients, while surgery constitutes definitive treatment in case of massive hemorrhage or recurrent bleeding. In the cases reported the diagnosis was performed only via intra-operative enteroscopy. An unusual con

Elective delayed midgut reduction-No anesthesia for gastroschisis: Selection and conversion criteria.

BACKGROUND/PURPOSE: In 1998 Bianchi and Dickson published their proposal for elective delayed midgut reduction without anaesthesia (EDMR-No GA). The study has been prospectively extended to develop "selection and conversion criteria" to ensure safe application of the technique. METHODS: In a prospective study from 1993 to date, EDMR-No GA was considered as first-line postnatal management in 35 children. The protocol and technique were those described by Bianchi and Dickson in 1998. RESULTS: EDMR-No GA was completed successfully in 25 children, of whom, 23 are normal with an aesthetic, scarle

[Ambulatory surgical interventions in patients with proctological diseases].

The results of 629 operations for the most frequent proctological diseases (hemorrhoid, anal scissure, rectal fistula, acute paraproctitis) were analyzed. 134 operations were performed in outpatient clinic, 225--in one-day hospital and 270 (control group)--in coloproctology department. The place of operation did not influence postoperative period and temporary disability time. The mean stay of patient in outpatient department and hospital surgical unit was 3.8 and 324 hours, respectively. Since the second day after operation in specialized department it is possible to perform therapeutic manip

Evaluation of new cephalosporins for prophylaxis of surgical infection.

The appropriate use of recently marketed cephalosporins for antimicrobial prophylaxis during surgery is discussed. New cephalosporins (cefamandole, cefoxitin, cefotaxime, moxalactam, and cefoperazone) are often substituted in situations where older, "first-generation" cephalosporins or other antimicrobials traditionally have been used. Adoption of these newer agents for routine use in prophylaxis of surgical infection has led to concerns of substantially increased costs as well as development of bacterial resistance. To date, some of the newer cephalosporins have been investigated for prophyla

Minimally Invasive Neonatal Surgery: Hirschsprung Disease.

Transanal pull-through (TAPT) is the procedure of choice for treating Hirschsprung disease and should be performed with laparoscopic assistance using the anorectal line (ARL) to ensure optimum postoperative bowel function (POBF). The dentate line (DL) has traditionally been used as the landmark for commencing dissection during TAPT, but we prefer the ARL because the DL is too subjective and can be associated with risk for injury to delicate sensory innervation required for normal defecation in the anal transition zone. An intact anal transition zone and total excision of the posterior rectal c

Martius flap: an adjunct for repair of complex, low rectovaginal fistula.

BACKGROUND: Complex, rectovaginal fistula (RVF) are uncommon but difficult therapeutic problems. Local repair and flap advancement techniques have a high incidence of recurrence with poor functional outcomes. Transperineal repair with anal sphincter reconstruction, when indicated, and placement of a Martius flap (bulbocavernosus pedicled transplant) result in improved rates of repair and better functional outcomes. METHODS: A consecutive series of patients were retrospectively reviewed from a prospective database between 2002 and 2006. Data were gathered from 2 colon- and rectal-specialty pra

Intraoperative Adverse Event Reporting in Gastrointestinal Cancer Surgery: A Narrative Review on the Application of ClassIntra.

Intraoperative adverse events are inevitable during complex gastrointestinal cancer surgeries, ranging from minor technical issues to life-threatening complications. Their impact on patient outcomes underscores the need for standardised reporting as no universally accepted system currently exists. The ClassIntra classification system is a five-tier severity scale based on the required interventions, mirroring the Clavien-Dindo Classification for postoperative complications. The current narrative review was planned to explore current intraoperative adverse event reporting practices in gastroint

[Postoperative peritonitis: epidemiology, diagnosis, surgical treatment and prognosis].

Postoperative peritonitis is a rare but dangerous complication of gastrointestinal tract surgeries. Analysis of epidemiology, diagnostics, treatment, and prognosis of postoperative peritonitis was based on 35 manuscripts. Postoperative peritonitis is an uncommon (up to 6-7%) but dangerous complication with mortality rate >30%. Old age, comorbidities, impaired sealing of proximal digestive tract and delayed redo laparotomy are the main factors of high mortality. Late diagnosis of peritonitis is due to no specific signs, and its symptoms are explained by peculiarities of postoperative period and

Nutritional Considerations in Neonates Requiring Gastrointestinal Surgery.

Neonates who require gastrointestinal surgery are a complex group of patients that require special consideration with regard to nutritional supplementation and growth. During the acute postoperative phase, a major stress response causes catabolism with degradation of the body's nutrient stores leading to poor growth. Following surgery, parenteral nutrition is often required to support the surgical neonate; although, enteral nutrition, if feasible, is critical because it helps improve intestinal adaptation. However, the best type, mode, and duration of feeding is not established in the current

Geropharmacology and gastrointestinal surgery.

Operating on elderly patients has always been a risky task for surgeons. They are not only frail and susceptible to operative complications, but they also require meticulous preparation before their surgery to secure the optimal result. Unfortunately, most of these patients have comorbidities which increase the challenge. Even though the medication they use is helpful to control their diseases, it can change the plan of the surgery and its outcome dramatically. Postoperative medications and treatment also have a unique importance; underestimating them may lead to catastrophic results. Restarti

Use of statistical process control in quality improvement projects in abdominal surgery: a PRISMA systematic review.

BACKGROUND: The use of quality improvement methodology has increased in recent years due to a perceived benefit in effectively reducing morbidity, mortality and length of stay. Statistical process control (SPC) is an important tool to evaluate these actions, but its use has been limited in abdominal surgery. Previous systematic reviews have examined the use of SPC in healthcare, but relatively few surgery-related articles were found at that time. OBJECTIVE: To perform a systematic review (SR) to evaluate the application of SPC on abdominal surgery specialties between 2004 and 2019. METHODS:

Meckel's diverticulectomy: a multi-centre 19-year retrospective study.

BACKGROUND: Meckels diverticulum (MD) causes a number of acute surgical pathologies and can contain ectopic tissue with the surgical aim to resect all ectopic mucosa. This has traditionally implied a small bowel resection (BR); though contemporary literature has demonstrated Meckel's diverticulectomy to be safe. The aim of this study was to determine optimal resection strategy, and assess MD histopathological features and their relationship to outcomes. METHODS: A 19-year retrospective review of patient medical records across seven hospitals was conducted with demographic, clinical and pathol

A prediction model and nomogram for technical difficulty of peroral endoscopic myotomy.

BACKGROUND AND AIMS: Peroral endoscopic myotomy (POEM) is a promising endoscopic technique for achalasia. We aimed to establish a regression model and develop a simple nomogram to predict the technical difficulty of POEM in a single center with large volume cases. METHODS: 3385 achalasia patients treated with POEM were included, and the technical difficulty was systemically evaluated. All of them were randomized into the training cohort (n = 1693) or internal validation cohort (n = 1692). Then, the prediction model and nomogram were proposed based on multivariate logistic regression analysis

Surgical strategies for intestinal Crohn's disease.

OBJECTIVES: As a type of inflammatory bowel disease, Crohn's disease (CD) is characterized by jumping lesions and transmural inflammation. The treatment goal is to control the development of inflammation with drugs such as 5-Amino Salicylic Acid, azathioprine and hormones, but most patients still need surgical treatment eventually. The surgical treatment of CD requires exquisite surgical design and solid surgical procedures to minimize the progression of intestinal lesions and preserve the normal bowel segment as much as possible. This study aims to summarize and discuss the surgical treatment
