Sequential therapy is a treatment strategy involving the use of specific therapeutic agents in a deliberate sequence to optimize the therapeutic outcome. The rationale for this strategy in psoriasis is that it is a chronic disease requiring long-term maintenance therapy as well as quick relief of symptoms and that some therapies available for psoriasis are better suited for rapid clearance while others are more appropriate for long-term maintenance. Sequential therapy involves 3 main steps: (1) the clearing, or "quick-fix" phase; (2) the transitional phase; and (3) the maintenance phase. In th

Low-dose pulse methotrexate has emerged as one of the most frequently used slow-acting, symptom-modifying antirheumatic drugs in patients with rheumatoid arthritis (RA) because of its favourable risk-benefit profile. Methotrexate is a weak bicarboxylic acid structurally related to folic acid. The most widely used methods for the analysis of methotrexate are immunoassays, particularly fluorescence polarisation immunoassay. After oral administration, the drug is rapidly but incompletely absorbed. Since food does not significantly affect the bioavailability of oral methotrexate in adult patients,

The efficacy of Mebendazole and Niclosamide was studied in two groups of 24 and 38 cases, respectively of patients suffering from taeniasis. Mebendazole with dose schedule of 200 and 300 mg twice daily for 3 consecutive days showed a cure rate of 71.42% and 92.30%, whereas Niclosamide at the dose rate of 200mg per patient was 94.76% effective. Flubendazole showed a cure rate of 66.66% only. Mebendazole and Niclosamide possess high taeniacidal activity, ability to reduce the clinical symptoms of taeniasis without any side effects. Niclosamide with high activity and excellent tolerance, is a dru

A case of erythema in a 'flagellate' pattern due to administration of bleomycin in a patient with Hodgkin's disease is described. Emphasis is placed on the low dose of bleomycin administered--15 mg parenterally--and the short time interval between drug administration and beginning of symptoms: 24 h. The histopathological findings were consistent with a mechanism of inflammatory oncotaxis: the attraction of tumor cells by the skin perhaps due to the bleomycin accumulated and the trauma caused by scratching subsequent to pruritus.

The prompt administration of antibiotics as therapy for fever in neutropenic patients has become an accepted principle. Combinations of an aminoglycoside plus a broad-spectrum beta-lactam agent have been used most extensively, producing response rates of 65%-70% at M. D. Anderson Cancer Center, Houston, Texas. We found that in neutropenic patients aminoglycosides alone were not effective, even against infections caused by susceptible gram-negative bacilli. Among patients with neutrophil counts of less than 100/mm3, only approximately 25% responded. The first studies of combinations of a penici

Doxorubicin is one of the most commonly used antineoplastic agents today. Dosing schedules are inexact and there remains a need to develop predictive models for its administration. Data from prior work in humans is difficult to interpret because of poor patient selection, poor drug assays, lack of knowledge of metabolite toxicity, concurrent treatment with other hepatically metabolized drugs, and individual pharmacogenetics which are poorly described. We have developed a rabbit model of in vivo drug pharmacokinetics in the setting of enzyme inhibition and sublethal hepatocellular necrosis. Our

Background/Objectives: Diabetic foot ulcers are characterized by persistent inflammation, impaired angiogenesis, and delayed tissue regeneration. Epidermal growth factor (EGF) is an important regulator of wound repair; however, its therapeutic application may be limited by rapid degradation and poor retention within the wound environment. This study investigated the therapeutic efficacy of local administration of EGF and ceftriaxone using autologous red blood cell-derived carriers (RBCDCs) in an experimental model of diabetic wound healing in rats. Methods: Experiments were conducted on 30 alb

BACKGROUND: Treatment-resistant schizophrenia (TRS) is a condition characterized by intense symptom severity and poor response to different antipsychotic agents. The first therapeutic option in TRS is clozapine, but often high/medium doses are not tolerated. Adding an oral antipsychotic to low doses of clozapine is a promising strategy in the management of TRS. On the contrary, there are few data on combined clozapine/long-acting injectable (LAI) medications, and none on clozapine/LAI-aripiprazole.
CASE: A 21-year-old male schizophrenic patient, resistant to several oral and LAI medications,

The antimalarials, mainly chloroquine and hydroxychloroquine, derive from the quinoleine core of quinine. Their initial therapeutic indication was the treatment of malaria attacks but, because of anti-inflammatory and immuno-modulatory activities, they have been since used to treat many other pathologies, in particular dermatological ones. For some of these pathologies, lupus or porphyria cutanea tarda for example, the use of these molecules is based on obvious scientific evidence. For other pathologies (cutaneous sarcoidosis, polymyositis, polymorphous light eruption...), the data on the medi

A 39-year-old man with no prior history of atrial fibrillation was hospitalized with atrial fibrillation and a rapid ventricular rate. For the 7 months before presentation, he had been chewing nicotine polacrilex gum on his own. The week he first developed palpitations, he was chewing more than 1 piece of nicotine Polacrilex gum per hour during work. His diagnostic work-up during hospitalization found no cause for atrial fibrillation. He was cardioverted to sinus rhythm. At 6-month follow-up, he had not renewed chewing nicotine polacrilex gum, was in sinus rhythm, and had no history of palpita

The increased frequency of congenital malformations including caudal regression syndrome, in infants of women with insulin-dependent diabetes mellitus is well documented. Most of the related animal research has involved the in vitro embryo culturing methodology. This study involved the alternative in vivo approach in order to determine the effects of treatment of pregnant hamsters with the D- and L-isomers of glucose at five times just before and during the period of embryonic organogenesis on maternal blood glucose levels and the rates and types of fetal abnormalities. One group of animals wa

Phenobarbital is known to reduce serum bilirubin concentration in the newborn infant, but optimal dosage is unknown. Ten pregnant women and their infants were given a standard regimen including prenatal maternal administration and postnatal administration to the infant during the first week of life. The plasma levels of phenobarbital in the infants were found to increase during the period of administration, and to remain high for many days beyond the period of hyperbilirubinemia. Optimal dose schedules for phenobarbital should be based both upon pharmacologic effects (including those other tha

The costs associated with obtaining illicit drugs can fluctuate depending upon the relative drug availability. As a consequence of the changing costs, the effort that one must exert to obtain drugs is dynamic. Considerable evidence illustrates a critical role for dopamine in the ventral medial striatum in mediating drug reinforcement. However, little is known regarding how dopamine release is affected by changes in the costs associated with earning drugs. We used fast-scan cyclic voltammetry to determine how changes in the operant requirement affected dopamine release to self-administered coca

Drugs are administered at a dosing schedule set by their therapeutic index, and termination of action is achieved by clearance and metabolism of the drug. In some cases, such as anticoagulant drugs or immunotherapeutics, it is important to be able to quickly reverse the drug's action. Here, we report a general strategy to achieve on-demand reversibility by designing a supramolecular drug (a noncovalent assembly of two cooperatively interacting drug fragments held together by transient hybridization of peptide nucleic acid (PNA)) that can be reversed with a PNA antidote that outcompetes the hyb
