Ampullary adenomas occur sporadically and in the setting of familial polyposis syndromes. OBJECTIVES: This study evaluates the periodontal health status and the esthetic results of teeth subjected to orthodontic traction, after their exposure by an apically positioned flap.

The authors report 6 cases of gallstone ileus due to cholecystoduodenal fistula. In 4 patients a simple enterolithotomy was performed, while in one case the enterolithotomy was associated to cholecystectomy and duodenal suture; in the last patient it was associated to cholecystectomy and gastric resection. Only one death was registered among the cases treated with enterolithotomy. The authors believe that the severity of the pathology, generally combined with other important diseases, needs an eclectic treatment depending on the patient status.

Based on an analysis of the material of clinical observations of 1523 patients with acute cholecystitis the authors make a conclusion that findings of repeated echosonography with photometry and endoscopic intraduodenal electrothermometry of the duodenal mucosa allow reliable diagnosis of acute cholecystitis, estimation of the character of morphological alterations of the gallbladder wall, determination of the morphofunctional state of the liver. These methods allow to reliably control the course of the inflammatory process in acute cholecystitis and to timely change the curative tactics.

Fifteen patients (eight males, seven females; age range: 23-76 years) presenting with acute suppurative cholangitis underwent endoscopic retrograde cholangiography and sphincterotomy within 1-10 days of hospitalization. Cholangitis was due to common duct stones in all patients; all but one of them had their gall-bladders in situ. All of them had fever, jaundice, abdominal pain, leucocytosis and deranged liver function while 26.6% were in shock, 13.3% in coma and 40% in azotaemia. The effectiveness of esophagogastroduodenoscopy in the elderly was evaluated in a prospective study of 656 consecutive patients undergoing endoscopic examination of the upper gastrointestinal tract. Forty-six percent of the patients were younger than 65 years, and 13% were in the age group above 79 years. Initial complaints and final endoscopic diagnosis were related to sex and age. Two hundred twenty-three patients underwent upper gastrointestinal endoscopy and biopsy. Correlation of endoscopic and histological findings revealed that a normal gastric and duodenal mucosa could be correctly diagnosed endoscopically in about 80 and 88% of cases, respectively. Duodenitis was diagnosed correctly in 83% of cases; however, the correct endoscopic recognition of gastritis was achieved in only about 50% of the cases. The aim of this work was to describe the endoscopic features and clinical outcome of the duodenal complications in anaphylactoid purpura. Over a 3-year period, 20 patients were hospitalized in our unit because of purpura rheumatica. Duodenal complications occurred in 5 cases warranting endoscopic assessment. Between January 1975 and December 1979, 71 patients over the age of 70 underwent attempted duodenoscopic sphincterotomy for stones in the common bile duct. Fifteen patients still had gall bladders in situ. Sphincterotomy was possible in 69 of the patients and in 65 of these duct clearance was achieved, giving an overall success rate of 92%. Failure to achieve sphincterotomy in two cases was due to substantial peripapillary diverticula. Duct clearance failed in four patients, mostly due to the size of the retained stones.
