Reconstruction of the microtic ear remains one of the most challenging procedures encountered by the reconstructive surgeon. The use of autogenous rib cartilage continues to be the gold standard for microtia repair. Numerous refinements and modifications in the original technique described by Tanzer have paved the way for exceptional results in experienced hands. However, ideal results are not always achieved, and there continue to be drawbacks with the standard approach to reconstruction with autogenous rib cartilage. In an attempt to circumvent these shortcomings, surgeons have developed alt

Cartilage has shown promise as a graft material to close perforations in the tympanic membrane (TM), particularly in cases of advanced middle ear pathology. Although it is similar to fascia, its more rigid quality tends to resist resorption and retraction. However, it is this rigid quality that has led many to anticipate a significant conductive hearing loss when using cartilage to reconstruct the TM. Because little has been reported in the literature comparing hearing results using cartilage with results using other grafting materials, this retrospective study was conducted to compare the hea

BACKGROUND: Cartilaginous disorders comprise a wide range of diseases that affect normal joint movement, ear and nose shape; and they have great social and economic impact. Mesenchymal stem cells (MSCs) provide a promising regeneration alternative for treatment of degenerative cartilaginous disorders. This study aimed to compare therapeutic potential of different types of laser activated MSCs to promote auricular cartilage regeneration. Twelve adult rabbit allocated equally in four groups, all animals received a surgical mid auricular cartilage defect in one ear; Group I (Positive control) inj

Corrective otoplasty is a commonly performed procedure to change the shape of the auricular cartilage. Many techniques use permanent sutures to maintain the cartilage folding, whereas other techniques rely on cartilage incisions (partial thickness or full thickness). At this institution, a cartilage cutting and anterior scoring technique has been used for more than 30 years with pleasing results. The surgical techniques published in the past have been reviewed and compared with the procedure used at this institution to point out the advantages, disadvantages, and differences of these various t

Following the unilateral implantation of metal ear tags in female Crl:CD(SD)BR-rats, chronic inflammatory lesions were observed in both auricles in 7% and 24% of the animals after 30 and 60 weeks, respectively. Involvement of the collateral auricles was identified only after diffuse inflammation of the ear tag-marked pinnae had developed. Histological examination revealed a multifocal granulomatous chondritis, characterized by progressive destruction of the cartilaginous plate and excessive regeneration of cartilaginous tissue. IgG and complement deposits were present in the matrix of the marg

BACKGROUND: Cauliflower ear deformity, a common sequela of auricular trauma, presents an esthetic and reconstructive challenge. Existing surgical techniques have limitations, including complexity, donor site morbidity, and variable long-term outcomes.
MATERIALS AND METHODS: In this case series, we present a novel and minimally invasive surgical approach for the correction of cauliflower ear deformity that adapts the Valente otoplasty technique; it combines cartilage debulking with helical rim release and Mustardé mattress stitches to restore ear contour and reduce the risk of recurrence. The

The combined Heermann and Tos (CHAT) technique is the combination of Heermann's 'cartilage palisade tympanoplasty' and Tos's 'modified combined approach tympanoplasty = modified intact canal wall mastoidectomy'. The first author (Cem Uzun) performed the CHAT technique as a one-stage operation in 15 ears of 15 patients with cholesteatoma. Two patients (one with a follow up of less than six months and one who did not show up at the final re-evaluation) were excluded from the study. Median age in the remaining 13 patients was 37 years (range: 14-57 years). Cholesteatoma type was attic, sinus (Tos

BACKGROUND: In patients with microtia, auricular reconstruction is ideally performed promptly to prevent impaired socialization during formative childhood years. The earliest viable age for reconstruction is widely accepted from 7-10 years of age, as full auricular size is achieved around age 8, with some variability dependent on sex. This retrospective study aims to provide an auricular growth curve that accounts for age and sex, enhancing the individualized approach to ear reconstruction.
METHODS: A total of 319 images of unaffected patients who underwent computed tomography angiography of

Ossiculplasty is the preferred intervention to restore the mechanism of sound transmission in patients with ossicular deformities. Here, we present a case of a young female who underwent cartilage tympanoplasty with total ossicular replacement prosthesis (TORP) to the right ear. Her recovery was progressing well with signs of postoperative improvements for almost two years until she was started on Isotretinoin 40 mg by her dermatologist. A few months later, she presented with worsening hearing loss and bloody discharge to the same ear. Consequently, examination showed that part of the prosthet

QUESTION: Ear piercing is one of the most common forms of body modification seen in children and adolescents presenting to my office. Parents of my younger pediatric patients inquire about potential post-piercing complications and risk factors associated with earlobe infections. What guidance should I give them? Also, are there any specific post-piercing complications to consider for older pediatric patients seeking second piercings in the upper cartilage area?
ANSWER: Piercing the earlobe or auricular cartilage continues to be a popular procedure among children and adolescents. Despite its w

OBJECTIVES/HYPOTHESIS: Cartilage-perichondrium grafting of the tympanic membrane has been used in an effort to reduce recurrence or progression of middle ear disease. The rigidity of cartilage has obvious benefit in preventing tympanic membrane retraction, but concern has been raised regarding its sound conduction properties Few studies in the literature address hearing results after cartilage tympanoplasty. The purpose of this study was to investigate the hearing results after primary cartilage tympanoplasty and compare them with results after primary tympanoplasty with temporalis fascia.

OBJECTIVE: To study the auricular cartilage palisade technique for repairing tympanic membrane perforation.
METHOD: Sixty-six cases of large tympanic membrane perforation were treated with auricular cartilage. The results were compared with that of temporal fascia repairing in 60 ears.
RESULTS: The closure rates of tympanic membrane perforation were 92.4% in cartilage group and 80% in temporal fascia group, respectively. There was no significant difference between two groups in hearing improvement. The cartilage is quite suitable for repairing large perforation of the tympanic membrane and f

The cartilage anchor is a cartilagineous strip inserted subcutaneously in front of the nasolabial fold which allows precise forming and movement of the nasolabial fold in facial palsy. It is taken from ear cartilage. Static and dynamic symmetry can be improved considerably by fixing a free microvascular muscle transplant or a sling plasty to the cartilage anchor. In cases of significant laxity of the paralysed side of the face, the combination of muscle transplant and sling plasty may be helpful.

This case report describes a patient with a left ear deformity resulting from a flame burn sustained 20 years ago. The patient underwent an ear reconstruction procedure utilizing an expanded neck flap. The autologous rib cartilage was used as the framework, while an expanded neck flap served as the covering for the framework. The surgery was completed in 3 stages. Initially, tissue expanders were implanted and gradually inflated with water. After sufficient expansion, the expanders were removed, and the scar tissue was excised. Subsequently, the expanded flap was used to cover the defects, and

OBJECTIVE: The etiology of osteoarthritis (OA) is unknown, however, there appears to be a significant contribution from genetics. We have identified recombinant inbred strains of mice derived from LG/J (large) and SM/J (small) strains that vary significantly in their ability to repair articular cartilage and susceptibility to post-traumatic OA due to their genetic composition. Here, we report cartilage repair phenotypes in the same strains of mice in which OA susceptibility was analyzed previously, and determine the genetic correlations between phenotypes.
DESIGN: We used 12 recombinant inbre

In facial esthetics, an anatomically normal external ear is integral in maintaining facial proportions, balance, and symmetry. The shape and size of the ear can have a considerable effect on confidence and self-esteem. When the external ear is significantly underdeveloped at birth, as in microtia, many aspects of the child's life are impacted. Microtia can negatively alter body image and disrupt personal and social development. Reconstructing the ear will enable the child to feel 'normal', thereby preventing the psychological and social challenges associated with the condition. Autologous cost

This report presents 2 cases of atypical small concha-type microtia, which were reconstructed with full utilization of the remnant cartilage. To repair the deformity, we harvested the 8th and 9th costal cartilage to form the helix and anthelix structure. The remnant auricular cartilage mass was retained and utilized as the base block of the framework, onto which sculptured helical rim and anthelix were added. Thus an ear of symmetrical shape, projection and visible convolutions was reconstructed. We suggest making use of the remnant cartilage to minimize the donor site morbidity, especially fo

There has been renewed interest in the use of cartilage for middle-ear reconstructions. The aim of the present review is to examine the indications, techniques and surgical outcomes of cartilage tympanoplasties reported in the literature. There have been concerns regarding weakening of cartilage struts, from histological studies involving explants; as a result, cartilage struts for ossiculoplasty have not gained popularity. On the other hand, cartilage tympanoplasty is now an established procedure for tympanic membrane and attic reconstruction. The commonest techniques involve cartilage palisa
