In elite sports, indirect testing of testosterone abuse is mainly based on the testosterone over epitestosterone (T/E) ratio. Since this marker is characterized by a small ratio of intra- to inter-individual variation, it is surprising that current anti-doping strategy uses a screening test based on a population-based limit. From a database of more than 15,000 steroid profiles obtained from routine controls, the collection of steroids profiles of 11 elite athletes followed during 2 years, and a longitudinal study involving 17 amateur athletes, 8 of which were orally administrated testosterone

Background: Prior concussion studies have shown that objective neurophysiological measures are sensitive to detecting concussive and subconcussive impairments in youth ice-hockey. These studies monitored brain vital signs at rink-side using a within-subjects design to demonstrate significant changes from pre-season baseline scans. However, practical clinical implementation must overcome inherent challenges related to any dependence on a baseline. This requires establishing the start of normative reference data sets. Methods: The current study collected specific reference data for N = 58 elite,

Are the ethics of athletes complex? Consider that an elite bicyclist might inject himself with hormones to increase his exercise capacity, but if his opponent falls, then that same hormone-abusing athlete will likely stop until his opponent has regained control. This article reviews key concepts from the philosophy of sport. Using six cases from the sports archives, it presents a practical framework for interpreting and assessing the moral status of an athlete's behavior. This work enables readers to appreciate more clearly both the contextual subtleties and the robust relationship between ethi

In brief Two healthy young athletes, a 22-year-old college football player and a 31-year-old elite runner, were evaluated for paroxysmal tachycardia. The football player's resting ECG showed the short PR interval and delta wave characteristic of Wolff-Parkinson-White syndrome. Electrophysiologic studies were required to diagnose accessory-pathway conduction in the runner. The high incidence of tachyarrhythmias seen among people who have this preexcitation syndrome makes detection essential. When such arrhythmias cannot be controlled with drug therapy, catheter ablation helps.

BACKGROUND: Olympic class sailing poses physiological challenges similar to other endurance sports such as cycling or running, with sport specific challenges of limited access to nutrition and hydration during competition. As changes in hydration status can impair sports performance, examining fluid consumption patterns and fluid/electrolyte requirements of Olympic class sailors is necessary to develop specific recommendations for these elite athletes. The purpose of this study was to examine if Olympic class sailors could maintain hydration status with self-regulated fluid consumption in cold

High-intensity resistance training causes intermittent extreme afterload, but its potential to induce maladaptive myocardial fibrosis remains unclear. This study investigated the molecular and tissue-level cardiac consequences of long-term resistance training. A cross-sectional study of 13 elite male weightlifters (≥ 5 years experience) and 13 age-matched healthy controls. Participants underwent transthoracic echocardiography (longitudinal strain) and cardiac magnetic resonance imaging (native T1, extracellular volume [ECV], and late gadolinium enhancement [LGE]). Serum biomarkers (galectin-3,

BACKGROUND: This investigation sought to elucidate the differential effects of varied strength training approaches on sprint velocity and initial acceleration in high-level sprinters.
METHODS: The study engaged 48 elite track athletes (equally divided between males and females, aged 18-25) who were randomly allocated to four distinct training regimens: conventional resistance training (CRT), plyometric training (PLT), integrated complex training (ICT), and a control cohort (CC) adhering to their standard training protocol. The experimental phase spanned 12 weeks, with three weekly sessions su

INTRODUCTION: Downhill Mountain Biking is an extreme sport requiring high mental strength to perform on the best level in a competition with only one run to win the race. The substantial challenge here is to control automatic processes like competitive anxiety and stress. Hypnosis can address these automatic processes. We developed and evaluated a hypnosis audio-intervention to activate the optimal racing mindset.
METHODS: In our study, 19 elite Downhill Mountainbike athletes registered at two consecutive races of the IXS Downhill Cup. After the first race, athletes listened to the hypnosis a

While we now appreciate that autonomic dysfunction can impact wheelchair rugby performance, this is currently not being assessed during classification, largely due to lack of a standardized and evidence-based strategy to assess autonomic function. Our aim, therefore, was to establish the optimal autonomic testing protocol that best predicts cardiovascular capacity during competition by comprehensively examining autonomic function in elite wheelchair rugby athletes with cervical SCI and thereby enhance the standardized classification. Twenty-six individuals with cervical SCI (C4-C8; AIS A, B, C

OBJECTIVE: To assess whether previous freezing cold injuries (FCI) would affect digit skin temperatures and rewarming rates during a follow-up cold stress test protocol.
DESIGN: Nonrandomized control trial.
METHODS: Twenty elite alpinists participated; alpinists with previous FCI requiring digit amputations (injured, INJ: n = 10 total, n = 8 male) were compared with ability-matched, uninjured alpinists (control, CON: n = 10, all male). Digit skin temperature was measured using infrared thermography as an index of peripheral digit perfusion after a cold stress test, which consisted of 30 minu

Background: Participation in soccer imposes high physical, mechanical, and cognitive demands. Recent evidence suggests that cognitive load, often overlooked in injury prevention, interacts with biomechanical factors and injury risk, resembling a dual-task paradigm where players must adapt motor responses while processing unpredictable game situations. This cross-sectional observational study examined how adding a dual-task during single-leg countermovement-jumps (SLCMJ) affects neuromotor control and performance in elite soccer players. Methods: Players performed SLCMJ on the injured leg while

BACKGROUND: Caffeine is a well-known ergogenic aid that can enhance physical and cognitive performance. However, it often induces side effects, such as anxiety and overstimulation, which can be problematic in high-pressure sports like wrestling. L-theanine, a non-stimulant amino acid found in tea, may help mitigate these effects by promoting a calm yet focused mental state. This study aimed to investigate the acute effects of caffeine, L-theanine, and their combination on physical performance, cognitive function, and anxiety in elite male wrestlers.
METHODS: In a double-blind, placebo-control

BACKGROUND: Although antiretroviral therapy (ART) is effective in suppressing viral replication, HIV-1 persists in reservoirs and rebounds after ART has been stopped. However, a very few people (eg, elite and post-treatment controllers) are able to maintain viral loads below detection limits without ART, constituting a realistic model for long-term HIV remission. Here, we describe the HIV control mechanisms of an individual who showed exceptional post-treatment control for longer than 15 years.
METHODS: We report the case of a Hispanic woman aged 59 years with sexually acquired acute HIV infe

An intriguing quality of our brain is that when actions are imagined, corresponding brain regions are recruited as when the actions are actually performed. It has been hypothesized that the similarity between real and simulated actions depends on the nature of motor representations. Here we tested this hypothesis by examining S.D., who never used her legs but is an elite wheel chair athlete. Controls recruited motor brain regions during imagery of stair walking and frontal regions during imagery of wheel chair slalom. S.D. showed the opposite pattern. Thus, brain simulation of actions may be g

Hamstring injuries continue to be very common for both elite and amateur athletes. Given their high recurrence rate, the ability to treat these injuries effectively is critical to helping athletes return to their previous level of activity without putting them at risk for future injury. Most hamstring strains can be treated with initial pain control and a course of rehabilitation focused on a gradual return to activity. However, an exact, evidence-based rehabilitation protocol has yet to be studied. Although surgery is rare and reserved for complete hamstring ruptures, results show high patien

This paper examines the rationality of elite bunker building as a response to anticipated societal collapse. Indeed, the phenomenon of "prepping" for "the Event" can be framed as self-insurance and relies on a transactional view of humanity, if one is to ensure the control of a compound and fight off potential assailants. We draw on economic decision modeling to analyze how the necessity of internal control by the leader, resentment, or the perception of potential loot by outsiders interact with fortification strategies. We introduce a "Machiavelli index" to represent hostility and show that e
