[Internal medical emergencies in the pregnant patient : Peripartum sepsis, metabolic derailment, endocrinological emergencies and pulmonary edema].

Peripartum emergencies that require intensive medical care represent a major challenge for the interdisciplinary treatment team. Due to physiological changes in pregnant women symptoms can be masked and the initiation of treatment is delayed. Peripartum sepsis has a relatively high incidence. The anti-infective treatment depends on the spectrum of pathogens to be expected. Endocrinological emergencies are rare but can be fulminant and fatal. The development of ketoacidosis is favored by decreased bicarbonate buffer and placental hormones. In the case of thyrotoxicosis, propylthiouracil and thi…

Emergencies related to recreational drug abuse in Spain compared to emergencies attended in 3 European areas.

OBJECTIVES: To analyze epidemiologic, clinical, and care characteristics in cases in which patients came to 2 Spanish emergency departments (EDs) with symptoms caused by recreational drug abuse. To compare the characteristics with those reported for other areas of Europe. MATERIAL AND METHODS: Secondary analysis of the registry of the European Drug Emergencies Network (Euro-DEN Plus), which collects cases in 14 European countries and 20 EDs. The registry included all patients attending EDs with symptoms of recreational drug abuse (excepting cases involving alcohol alone) over a period of 39 c…

[Essential Diagnostic Tests in Neurological Emergencies].

Neurological emergencies are time-sensitive conditions in which rapid diagnosis and early intervention critically influence survival and functional outcomes. This review summarizes essential diagnostic strategies for neurosurgeons involved in acute care, guided by the principles of Emergency Neurological Life Support (ENLS). Initial management prioritizes stabilization of the airway, breathing, and circulation, followed by focused neurological assessment, with particular attention to pupillary findings and quantitative monitoring such as the Neurological Pupil index. Laboratory evaluation aims…

Urologic oncologic emergencies: a radiologist's guide.

Urologic oncologic emergencies represent a relatively narrow yet diverse group of critical conditions that require prompt recognition and intervention to prevent potentially life-threatening complications. These oncologic emergencies may arise as direct consequences of a malignancy, including local invasion, or as sequelae of surgical or therapeutic interventions. Common urologic emergencies include malignant obstructive uropathy or ureteral obstruction, which may lead to urosepsis or acute kidney injury; large volume hematuria and hemorrhagic cystitis, which both can result in substantial blo…

Gastrointestinal oncologic emergencies: a radiologists' guide.

The prevalence of cancer continues to increase both in the United States and worldwide. Oncologic emergencies are potentially life-threatening conditions in cancer patients, and these may be directly or indirectly related to the underlying malignancy. Radiologists must be well-versed not only in cancer staging, but also in potential cancer related complications and emergencies to guide prompt appropriate management. Oncologic emergencies may occur due to direct local effects by the tumor or metastasis, associated systemic manifestations, or due to treatment related effects. These emergencies m…

Acute Kidney Injury in Endocrine Emergencies.

BACKGROUND: Acute kidney injury (AKI) is a serious condition in clinical medicine that significantly increases morbidity and mortality, particularly in critically ill patients. Rapid and accurate identification of the underlying causes of AKI are crucial for determining appropriate therapeutic management and potentially saving the patient's life. Although endocrine emergencies are a less common cause of AKI in critically ill patients, recognizing when they occur is vital to comprehensive care. SUMMARY: AKI can impair the endocrine system and result in critical conditions for patients, particu…

Protection of Child Health in Emergencies.

Natural disasters present a significant and growing threat to the well-being of children. Every year 175 million children globally are expected to be affected by natural disasters, including floods, cyclones, droughts, heatwaves, severe storms, and earthquakes. In emergencies, children of all age groups, especially those under 5, are the most affected part of the community, with child mortality rates 2-70 times higher than average. Clean water, sanitation and hygiene measures, vaccination to prevent infectious diseases, providing psychological support to vulnerable children in an age-appropria…

Paediatric resident identification of cardiac emergencies.

OBJECTIVES: Critical CHD is associated with morbidity and mortality, worsened by delayed diagnosis. Paediatric residents are front-line clinicians, yet identification of congenital CHD remains challenging. Current exposure to cardiology is limited in paediatric resident education. We evaluated the impact of rapid cycle deliberate practice simulation on paediatric residents' skills, knowledge, and perceived competence to recognise and manage infants with congenital CHD. METHODS: We conducted a 6-month pilot study. Interns rotating in paediatric cardiology completed a case scenario assessment d

Hematologic Emergencies: Recognition and Initial Management.

Hematologic emergencies are bleeding or clotting disorders that are hereditary or acquired and must be treated emergently to avoid significant morbidity or mortality. Patients experiencing a hematologic emergency may present with spontaneous bleeding, jaundice, petechiae, or purpura. Initial diagnostic testing should include a complete blood count. Patients who have bleeding associated with a hereditary disorder should receive clotting factor replacement before diagnostic testing. Acute chest syndrome is an uncommon but serious complication of sickle cell disease. Hemolysis caused by autoimmun…

BACKGROUND: Neurosurgical emergencies in the obstetric setting pose considerable challenges. Decision-making involves deliberations on the gestational age, critical nature of the illness, timing of surgery, maternal positioning during neurosurgery, anesthesiologic strategies, monitoring of the pregnancy during surgery, and the mode of delivery. The present study discusses the management and ethical dilemmas encountered during the management of six obstetric patients with neurosurgical emergencies. METHODS: A retrospective review of all neurosurgical operations performed between January 2016 a…
