Female Nonobstetric Genitourinary Emergencies.

The emergency medicine provider sees a broad range of pathology involving the female genitourinary system on a daily basis. Must-not-miss diagnoses include pelvic inflammatory disease and ovarian torsion, because these diagnoses can have severe complications and affect future fertility. Although most patients with abnormal uterine bleeding are hemodynamically stable, it can present as a life-threatening emergency and providers should be adept managing severe hemorrhage. Bartholin gland cysts are common complaints that often require procedural intervention. This article discusses these diagnose

Retrieval of a rectal foreign body using enemas in a 13-year-old boy.

In adults, rectal foreign bodies are a well-described phenomenon. In the pediatric emergency department, however, insertion of foreign bodies into the rectum is a rarely seen condition. Herein, we discuss a case of a rectal foreign body in a 13-year-old boy and the modality of removal.

[Patients with alteration of consciousness in the emergency department].

A subject is conscious when he is awake and with an adequate awareness of him and the environment. The term alteration of consciousness requires specification as to whether it defines alteration of arousal--when the patient might be confused, in a stupor or in some degree of coma (light, deep)--or alteration of awareness--that is, confused (spatio-temporally disoriented, with difficulty in maintaining his attention), with or without delirious ideation. The coma, in the strict sense, originates from structural (neurological) or functional (metabolic) dysfunction of the ascending reticular activ

Recommended modifications and applications of the Hospital Emergency Incident Command System for hospital emergency management.

The Hospital Emergency Incident Command System (HEICS), now in its third edition, has emerged as a popular incident command system model for hospital emergency response in the United States and other countries. Since the inception of the HEICS in 1991, several events have transformed the requirements of hospital emergency management, including the 1995 Tokyo Subway sarin attack, the 2001 US anthrax letter attacks, and the 2003 Severe Acute Respiratory Syndrome (SARS) outbreaks in eastern Asia and Toronto, Canada. Several modifications of the HEICS are suggested to match the needs of hospital e

Hospital admissions from the emergency department of adult patients affected by myopathies.

BACKGROUND AND PURPOSE: Myopathies are associated with classic signs and symptoms, but also with possible life-threatening complications that may require assistance in an emergency setting. This phenomenon is understudied in the literature. We aimed to assess the presentation, management, and outcomes of clinical manifestations potentially related to a muscle disorder requiring referral to the adult emergency department (ED) and hospitalization. METHODS: Anonymized patient data retrieved using the International Classification of Diseases, Ninth Revision codes related to muscle disorders over

Case study: ED acts quickly after anthrax.

After last year's anthrax attacks, EDs at Capital Health System in Trenton, NJ, had to address decontamination of large numbers of patients. Patients who didn't require decontamination were sent to the hospital's employee health facility so they could bypass the ED. Clinicians, nursing staff, educators, and paramedics were trained in the decontamination process. Direct communication with first responders was established with a portable scanner.

Emergency department presentations and associated hospital admissions for low back pain in Australia.

OBJECTIVE: To determine factors associated with hospital admissions from ED presentations for low back pain (LBP). METHODS: This was a retrospective analysis of 147 ED centres in New South Wales (NSW), Australia, between 2005 and 2014. Data were accessed from the NSW Emergency Department Data Collection to determine patients who attended ED and whether or not they were admitted to hospital. RESULTS: There were 1 388 078 ED visits for LBP across 147 different ED centres between 2005 and 2014. Between 2005 and 2013, hospital admission rates from ED visits for LBP increased annually by 5.1% (95

Improving early recognition and treatment of sepsis in paramedic services and emergency departments: a pan-Canadian mixed methods application of the Theoretical Domains Framework.

OBJECTIVES: Efforts to implement evidence-based guidelines for sepsis care (Surviving Sepsis, CAEP guideline) have met with limited success and mortality from sepsis remains high. We sought to understand barriers and facilitators from the frontline perspective of emergency nurses, physicians, and paramedics. METHODS: In an explanatory sequential mixed methods approach, a 55-question survey was developed using the Theoretical Domains Framework (TDF) and scoping review (reported separately) and distributed via Canadian emergency, nurse, physician, and paramedic organizations in English and Fren

Indiana Adults Who Participated In Treatment Court Programs Had Better Health Outcomes Than Those Who Did Not.

More people are arrested in the US for substance-related crimes than for any other offense. Treatment courts are a judicial intervention offering a holistic, public health approach to substance use, and they provide an alternative to incarceration. There is, however, great variability with respect to court practices and interventions across the US. This article explores whether there is an association between participation in treatment court programs and three health-related outcomes, as measured by mortality, emergency department use, and emergency medical services (EMS) use. We studied peopl

Emergency department care experiences among members of equity-deserving groups: quantitative results from a cross-sectional mixed methods study.

BACKGROUND: Emergency departments (EDs) serve an integral role in healthcare, particularly for vulnerable populations. However, marginalized groups often report negative ED experiences, including stigmatizing attitudes and behaviours. We engaged with historically marginalized patients to better understand their ED care experiences. METHOD: Participants were invited to complete an anonymous mixed-methods survey about a previous ED experience. We analysed quantitative data including controls and equity-deserving groups (EDGs) - those who self-identified as: (a) Indigenous; (b) having a disabili

Accuracy of ultrasound in diagnosing ankle injuries in emergency care.

BACKGROUND: Ankle injuries are one of the most common presentations in the ED. Although fractures can be ruled out using the Ottawa Ankle Rules, the specificity is low, which means many patients may still receive unnecessary radiographs. Even once fractures are ruled out, assessment of ankle stability is recommended to rule out ruptures, but the anterior drawer test has only moderate sensitivity and low specificity and should be performed only after swelling has receded. Ultrasound could be a reliable, cheap and radiation free alternative to diagnose fractures and ligamentous injuries. The pur

Have the Diagnoses of Patients Transported by Ambulances Changed in the Early Stage of the COVID-19 Pandemic?

INTRODUCTION: Since December 2019, emergency services and Emergency Medical Service (EMS) systems have been at the forefront of the fight against the coronavirus disease 2019 (COVID-19) pandemic world-wide. OBJECTIVE: The objective of this study was to examine the reasons and the necessity of transportation to the emergency department (ED) by ambulance and the outcomes of these cases with the admissions during the COVID-19 pandemic period and during the same period in 2019. METHODS: A retrospective descriptive study was conducted in which patients transported to the ED by ambulance in April

Community-acquired bacterial meningitis in adults: emergency department management protocol.

Acute bacterial meningitis (ABM) is a rare but disabling infectious condition that requires a performant multidisciplinary management approach. Between 70 and 90 adult patients are diagnosed with community-acquired ABM in Belgium annually, and reported case fatality rates range from 17 to 40%. The currently available guidelines provide evidence-based guidance on how to manage this disease. However, these guidelines do not translate the evidence to the daily practice at the emergency department in a Belgian healthcare context. We created a taskforce in University Hospitals Leuven consisting of

Computed tomographic angiography in stroke and high-risk transient ischemic attack: Do not leave the emergency department without it!

Stroke is a major cause of morbidity and mortality worldwide, and effective treatment requires rapid diagnosis and recognition of relevant vascular lesions. In this review we will discuss the usefulness and versatility of computed tomography angiography in the setting of stroke, be it ischemic or hemorrhagic, minor or disabling. Furthermore, we also highlight how we use computed tomography angiography in decision making in transient ischemic attacks, acute disabling ischemic stroke, and hemorrhagic stroke.
