Factors associated with epiglottic petiole prolapse repositioning success.

OBJECTIVE: Epiglottic petiole prolapse is an overlooked entity that could lead to supraglottic airway obstruction for patients with complex airway history. Classical symptoms include exercise intolerance, obstructive sleep apnea, and difficulty with decannulation. The goal of this project was to evaluate the factors associated with epiglottic petiole repositioning success. METHODS: Retrospective case series of patients with a complex history of airway reconstruction evaluated by the aerodigestive team at a tertiary pediatric hospital from May 2003 to August 2017. All patients underwent reposi

[Transoral CO2 laser epiglottectomy for early epiglottic carcinomas].

OBJECTIVE: To compare the functional results of transoral CO2 laser epiglottectomy with transcervical supraglottic laryngectomy and to find the landmarks for CO2 laser surgery. METHODS: Forty-nine preoperatively untreated patients diagnosed as clinical T1-2N0M0 epiglottic carcinomas enrolled from June 1, 2006, to November 1, 2009 in the Second Affiliated Hospital of Harbin Medical University were studied retrospectively. Seventeen cases were treated by CO2 laser whereas 32 by open surgeries. The upper edge of thyroid cartilage and mucosal recess infra epiglottic tubercle could be used as intr

A huge epiglottic cyst causing airway obstruction in an adult.

An epiglottic cyst causing airway obstruction is rare in an adult. Early definitive diagnosis and management obviate an unnecessary tracheostomy. We report a case of a 64-year-old woman who arrived at our hospital with progressive stridor and foreign body sensation when swallowing for 6 weeks. A hot potato voice and biphasic stridor were remarkable upon physical examination. Indirect mirror and fibroscopic examination revealed a huge epiglottic cyst. The neck lateral X-ray and computed tomography scan demonstrated a huge cystic mass over the epiglottis. A 2.5 x 3.0 cm cystic mass was removed w

Endoscopic division of the ary-epiglottic folds in severe laryngomalacia.

OBJECTIVE: Laryngomalacia is the commonest cause of congenital stridor. The underlying anatomical abnormality associated with this condition is a prolapse of the supraglottic tissues into the laryngeal inlet during inspiration, and may involve the epiglottis, ary-epiglottic folds and the corniculate mounds of the arytenoids. However, it has been noted that the most consistent structural abnormality seen in these cases is the shortening of the ary-epiglottic folds and marked side to side curling of the epiglottis. We describe the follow-up and outcome of 33 cases treated by the simple division

[Acute epiglottitis in children: current criteria for the diagnosis and treatment].

Acute epiglottitis is still a potentially lethal pathology, particularly in early childhood. The present study involves seven cases of acute epiglottitis in children under 4 years of age. The authors describe the diagnostic and therapeutic protocols used in these pediatric patients placing particular emphasis on the use of endoscopy and the need for prompt hospitalization in an intensive care unit to best integrate the diagnostic approach with therapeutic treatment.

[The surgical treatment for the epiglottic cancer extended to the base of tongue].

26 cases of the epiglottic cancer extended to the base of the tongue were studied retrospectively. Among them, 14 were male and 12 female. The age varied from 43 to 62. Besides two cases who underwent total laryngectomy, the other twenty-four were treated with supraglottic horizontal partial laryngectomy or horizontovertical subtotal laryngectomy and resection of part of base of the tongue. 11 cases received bilateral radical neck dissection (RND) and 10 cases unilateral RND simultenously. The results showed twenty-four cases had resumed speaking ability, twenty had been decannulated and resto

Acute epiglottitis in children: a conservative approach to diagnosis and management.

We report a series of 60 children with epiglottitis, 42 who were admitted via our emergency room and 18 who were transferred to Children's Hospital of Michigan (CHM) after initial airway management elsewhere. Patients managed entirely at CHM had lateral neck x-rays performed (diagnostic in each case), and underwent nasotracheal intubation in the operating room. There was no mortality or permanent morbidity in this group. Transferred patients were managed in a variety of ways at their referring institutions, sometimes without an artificial airway. Complications in this group included transient …

Nasotracheal intubation--a modality in the management of acute epiglottitis in adults.

Acute epiglottitis in adults is usually treated by close observation or tracheostomy. In the last decade, this therapeutic attitude has been challenged by the impressive results obtained by nasotracheal intubation in the treatment of pediatric cases. Many authors have suggested the use of nasotracheal intubation in adults as well as in children: it would be a natural complement to close observation when airway shunting becomes advisable thus rendering tracheostomy unnecessary. Three conditions have to be met before safe nasotracheal intubation can be considered: interested anesthesiological se…

Incidence and prognosis of acute epiglottitis in children in a Swedish region.

Acute epiglottitis, which is almost exclusively caused by Haemophilus influenzae type b, was studied retrospectively in children 0 to 15 years old in a defined region of southwest Sweden from 1971 through 1980; 211 patients with clinically well-documented acute epiglottitis were found. The age-specific incidence was 14/100,000/year, which was higher than the incidence of H. influenzae meningitis. The median age was 4 years. Because 96% of the patients were older than 18 months, disease in those patients could theoretically have been prevented by the purified H. influenzae polysaccharide vaccin…

Respiratory status of children with epiglottitis with and without an artificial airway.

During a ten-year period, 22 children from our 170 cases of acute epiglottitis had reliable records of arterial blood gas data. The arterial/alveolar (a/A) oxygen tension ratios were calculated, with a value less than 0.75 representing abnormal gas exchange. The mean a/A ratio for the whole group, 17 of whom already had an artificial airway, was 0.59 (range, 0.29 to 0.83). A subgroup of five children with blood samples taken during conservative treatment or before airway insertion had a mean a/A ratio of 0.62 (range, 0.49 to 0.77) without hypercapnia (mean Paco2, 32 mm Hg; range, 29 to 39 mm H…

Association Between Air Pollutants and Infectious Epiglottitis Incidence: A Observational Study.

BACKGROUND AND AIMS: Infectious epiglottitis is inflammation of epiglottis and surrounding structures and may be life-threatening without treatment. Air pollution is a critical risk factor for human health. This study aims to explore the correlation between infectious epiglottitis and air pollutants. METHODS: We collected the daily infectious epiglottitis cases in our hospital and the daily meteorological data, including average temperature, relative humidity, and the daily concentrations of air pollutants of Shanghai, China, from January, 2015 to December, 2019. Air pollutants include partic…

Acute epiglottitis in adults: an under-recognized and life-threatening condition.

Epiglottitis is a potentially life-threatening condition resulting from infection of the epiglottis and surrounding structures, causing classical signs and symptoms related to inflammation and progressive airway obstruction. We briefly present two recent adult cases of epiglottitis, followed by a review of the literature concerning the subject. In diagnosing epiglottitis, direct visualization by laryngoscopy is the gold standard. Lateral neck X-rays also have great utility, but with less sensitivity than direct visualization. Though historically more of a burden for the pediatric population, e…

Acute epiglottitis in northern Finland today--still a potential emergency situation.

To examine patient characteristics related to airway intervention in acute epiglottitis and how the intervention affected the further course of the disease, 46 pediatric and 49 adult cases were retrospectively evaluated. All three airway management approaches, namely observation, nasotracheal intubation and tracheotomy were needed in both the children and adults. Five out of every six children needed an artificial airway, while observation was sufficient in three out of every five adults. H influenzae type b bacteremia in children, a short duration of the symptoms in adults and most important,…
