Patients with unstable slipped capital femoral epiphysis have antecedent symptoms.

BACKGROUND: The characteristics of patients who sustain unstable slipped capital femoral epiphyses (SCFEs) are not well described compared to their counterparts who sustain stable SCFE. Although patients with unstable slips are usually identified owing to acute symptoms, it is unclear whether these patients have premonitory symptoms that could heighten the awareness of treating physicians to the possibility of an unstable slip and lead to timely diagnosis and treatment. QUESTIONS/PURPOSE: We determined whether most patients experienced pain and limp before developing an unstable SCFE. METHOD

Patients with severe slipped capital femoral epiphysis treated by the modified Dunn procedure have low rates of avascular necrosis, good outcomes, and little osteoarthritis at long-term follow-up.

AIMS: The modified Dunn procedure has the potential to restore the anatomy in hips with severe slipped capital femoral epiphyses (SCFE). However, there is a risk of developing avascular necrosis of the femoral head (AVN). In this paper, we report on clinical outcome, radiological outcome, AVN rate and complications, and the cumulative survivorship at long-term follow-up in patients undergoing the modified Dunn procedure for severe SCFE. PATIENTS AND METHODS: We performed a retrospective analysis involving 46 hips in 46 patients treated with a modified Dunn procedure for severe SCFE (slip angl

Frequency of radiographic signs of slipped capital femoral epiphysiolysis sequelae in hip arthroplasty candidates for coxarthrosis.

INTRODUCTION: The rate of osteoarthritis of the hip secondary to slipped capital femoral epiphysis (SCFE) is a subject of debate, and its frequency is underestimated in particular because of subclinical forms that may not be diagnosed during childhood. HYPOTHESIS: The frequency of radiographic anomalies subsequent to SCFE in adults requiring hip arthroplasty is higher than that reported in recent studies (6% to 15%). MATERIALS AND METHODS: A prospective single-center epidemiological radiography study was performed by one observer. Hip X-rays of patients who were being treated by arthroplasty

Salter-Harris type II fractures of the capital femoral epiphysis.

Salter-Harris type II fractures of the capital femoral epiphysis have not been previously documented. The authors have treated three patients who sustained four such fractures. One child had a recurrent fracture two years after the first had healed satisfactorily. Two fractures were treated by spica cast immobilization, one fracture by closed reduction and internal fixation, and the other fracture healed without treatment. No patient developed avascular necrosis or other complications. Two of the children had an association with idiopathic slipped capital femoral epiphysis. An etiologic relati

Outcomes of slipped capital femoral epiphysis treated with in situ pinning.

BACKGROUND: Previous long-term studies have shown good outcomes for most patients after in situ pinning of slipped capital femoral epiphyses (SCFE). However, concern is growing about the effects of leaving the epiphysis in a nonanatomic position. We undertook a retrospective study to carefully document patient-reported outcomes and need for additional surgery after in situ pinning of SCFE. Further, we sought to determine the risk factors for persistent pain and dysfunction after in situ pinning. METHODS: Between 1965 and 2005, 146 patients (176 hips) with SCFE underwent in situ pinning at a t

[The modified Dunn procedure to treat severe slipped capital femoral epiphysis].

The modified Dunn procedure enables restoration of the proximal femoral anatomy and normal hip function in patients with slipped capital femoral epiphysis (SCFE). Surgery is indicated in severe SCFE and in hips with a moderate slip angle and impaired function. To prevent further dislocation of the femoral head, the authors recommend non-weightbearing until surgery, since an accurate evaluation of slip stability is not possible in the clinical setting. Only a well-trained orthopedic surgeon with a high level of expertise in hip preservation surgery should perform this procedure. Precise knowled

Pinning in slipped capital femoral epiphysis: long-term follow-up study.

This long-term follow-up study was aimed at evaluating patients with hip epiphysiolysis treated by pinning and previously controlled by short-term follow-up, in order to evaluate both clinical and radiographic evolution. The case series included 36 patients (17 female and 19 male) for overall 48 hips (21 right, 27 left). The epiphysiolysis was bilateral in 12 patients. Median follow-up was 17.9 years (range 8.8-29.2). Clinical results were evaluated according to Harris, radiographic results according to Boyer. The patients were divided into three groups according to slipping degree calculated

Incidence of slipped capital femoral epiphysis is associated with low childhood opportunity index.

Slipped capital femoral epiphysis (SCFE) has historically been associated with Black race, male gender, and Hispanic ethnicity, obesity, and adverse social determinants of health. The child opportunity index (COI) 2.0 is an aggregate measure by ZIP code of 29 features of communities that can impact the life outcomes of children. We aimed to investigate the relationship between the incidence of SCFE and residential ZIP code COI. We retrospectively queried the surgical database of a tertiary-referral children's hospital in a medium-sized metropolitan area. We identified subjects who underwent in

Single-Event Bilateral Modified Dunn Osteotomy for Bilateral Severe SCFE: Restoring Function in a High-Level Athlete.

CASE: A 13-year-old competitive basketball player presented with bilateral acute-on-chronic, unstable (1 side) slipped capital femoral epiphysis. He underwent a single-event bilateral modified Dunn capital realignment osteotomy and fixation. At 4 years postsurgery, he has excellent clinical function, has returned to playing competitive basketball, and has good radiological outcomes. CONCLUSION: A single-event bilateral modified Dunn osteotomy is an option for severe bilateral SCFE. Centers with the necessary setup and a dedicated hip team can attempt this procedure. Modified Dunn osteotomy ca

Surgeon Preference for Prophylactic Contralateral Fixation in Slipped Capital Femoral Epiphysis (SCFE) Patients: A Nationwide POSNA Survey Study.

BACKGROUND: It is estimated that 11% to 60% of unilateral slipped capital femoral epiphysis (SCFE) patients will develop contralateral pathology, usually within 18 months after the first event. Despite this, prophylactic fixation remains controversial, and there is significant variability in surgeon preferences. Thus, this study aimed to determine which factors predict surgeon preferences for prophylactic contralateral pinning in SCFE patients. METHODS: We designed a survey for pediatric orthopedic surgeons to collect data on (1) surgeon and hospital characteristics, (2) individual preference

Hip morphology in slipped capital femoral epiphysis.

Several mechanical factors have been related to slipped capital femoral epiphysis (SCFE). Main aim of this study is to investigate the acetabular coverage and acetabular version in unilateral SCFE hips in order to detect a potential pincer-type deformity as predisposing factor; second, we compared those measurements either to the contralateral, uninvolved hips either to a matched healthy control population. A total of 85 patients treated for unilateral SCFE were retrospectively reviewed. The lateral center-edge angle (LCEA) and the Tönnis angle were used to assess acetabular coverage, whereas

[Osteonecrosis of the femoral head complicating prophylactic screw fixation for upper femoral epiphyolysis: case report and literature review].

The therapeutic approach for the treatment of children with radiologically "normal" hip presenting with epiphysiolysis of the contralateral upper femur is controversial. We here report the case of a 12-year-old boy with osteonecrosis of radiologically "normal" and asymptomatic left femoral head prophylactically fixed due to acute femoral epiphysiolysis of the contralateral hip. Eight months after first surgery, the patient developed symptoms of avascular osteonecrosis of the femoral head. Specific criteria to be used in evaluating the most effective treatment between preventive screw or simple

[Bilateral caput femoris epiphysiolysis after epileptic seizures].

This case rapport is about a 12-year-old boy with loss of gait function over seven months after epileptic seizures. X-ray confirmed bilateral slipped capital femoral epiphysis (SCFE) probably with traumatic origin caused by the seizures. SCFE is characterised by slip of the epiphysis in relation to the metaphysis of the femoral bone. Traumatic SCFE is painful, and if untreated it can cause severe deformities and caput necrosis. Surgical treatment is necessary, one option is in situ fixation, which was the treatment of the boy in this report. At six-week post-operative control some stand functi

[Fixation techniques for slipped capital femoral epiphysis : Principles, surgical techniques, and complications].

The purpose of this review is to present the pros and cons as well as the surgical techniques of conventional implants used for fixation of slipped capital femoral epiphysis (SCFE). Worth mentioning are K‑wires, Hansson pins, transfixing screws, and gliding screws. We searched PubMed for "ECF" and "SCFE" in combination with "in situ fixation," "pin," "wire," "screw," and "nail." We considered Johansson nail, Knowles pin, and Nyström nail to be obsolete and of historical interest only. We noticed a trend from absolute stability towards some form of dynamic fixation over time, likely related to

Prepubertal onset of slipped capital femoral epiphysis associated with hypothyroidism: a case report and literature review.

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a common hip disorder characterized by displacement of the capital femoral epiphysis from the metaphysic through the femoral epiphyseal plate. SCFE usually occurs during puberty, with obesity a common risk factor. We experienced a rare case of SCFE associated with hypothyroidism in a prepubescent patient who was not obese. CASE PRESENTATION: The patient was an 8-year-old boy suffering from bilateral SCFE with hypothyroidism. The patient's growth had started to slow at 4 years of age, and at 8 years he was of short stature. During his eva

Towards evidence-based emergency medicine: Best BETs from the Manchester Royal Infirmary. BET 2: Is ultrasound or plain film radiography a more sensitive diagnostic modality for diagnosing slipped capital femoral epiphysis?

A short-cut review was carried out to establish whether ultrasound or plain radiography is a more sensitive diagnostic modality for slipped capital femoral epiphysis. Three studies were directly relevant to the question. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of these papers are shown in table 2. The clinical bottom line is that ultrasonography is more sensitive then radiography for diagnosing slipped capital femoral epiphysis based on the results of small studies.
