Expedited transfer to emergency department vs. cardiac catheter laboratory in a cardiac arrest centre for non-ST-elevation out-of-hospital cardiac arrest: ARREST trial as-treated analysis.

AIMS: The ARREST trial demonstrated that in adult patients, transfer to a cardiac catheter laboratory in a cardiac arrest centre (CAC) following resuscitated out-of-hospital cardiac arrest (OHCA) without ST-elevation did not reduce deaths at 30 days compared with delivery to the geographically closest emergency department (standard care). More than half of the CACs had a co-located emergency department to which patients were delivered as part of the standard care arm, which may have influenced outcomes. AIMS: We performed a pre-specified as-treated analysis to determine if a CAC and the locat

Activity-based protein profiling guided new target identification of quinazoline derivatives for expediting bactericide discovery: Activity-based protein profiling derived new target discovery of antibacterial quinazolines.

INTRODUCTION: The looming antibiotic-resistance problem has imposed an enormous crisis on global public health and agricultural development. Even worse, the evolution and widespread distribution of antibiotic-resistance elements in bacterial pathogens have made the resurgence of diseases that were once easily treatable deadly again. The development of antibiotics with novel mechanisms of action is urgently required. OBJECTIVES: Inspired by charming activity-based protein profiling (ABPP) technology and increasing attention to quinazolines in the development of antibacterial agents, this study

A 66-Year-Old Woman with Intermittent Chest Pain and Dyspnea Who Underwent Continued ST-Segment Monitoring to Identify Occult ST-Segment Elevation that Expedited Coronary Angiography and Revascularization.

BACKGROUND In patients admitted to the hospital owing to suspected acute coronary syndrome and where the first electrocardiogram (ECG) is non-diagnostic of ST-elevated myocardial infarction (STEMI), international guidelines recommend that sequential ECGs are performed to identify new or intermittent ST-deviations. Continued monitoring for ST-segment alterations is rarely performed, in contrast to the continued monitoring for arrhythmias. Continued monitoring for ST-segment alteration may detect intermittent ST-segment elevations not captured by routine 12-lead ECGs. We present a case in which

A Pilot Evaluation of Expedited Partner Treatment and Partner Human Immunodeficiency Virus Self-Testing Among Adolescent Girls and Young Women Diagnosed With Chlamydia trachomatis and Neisseria gonorrhoeae in Kisumu, Kenya.

BACKGROUND: Expedited partner treatment (EPT) is effective for preventing sexually transmitted infection recurrence, but concerns about intimate partner violence and missed opportunities for human immunology virus (HIV) testing have limited its use in African settings. METHODS: We conducted a pilot prospective evaluation of EPT among adolescent girls and young women (AGYW) accessing HIV preexposure prophylaxis in an implementation project in Kisumu, Kenya. Those with etiologic diagnosis of Chlamydia trachomatis and Neisseria gonorrhoeae were treated and given the option of delivering sexually

Mountaineering medical events and trauma on Denali, 1992-2011.

Denali (Mt. McKinley) is the tallest mountain in North America and a popular climbing destination for high altitude mountaineering expeditions. National Park Service (NPS) personnel care for and manage medical incidences and traumatic injuries for mountaineers each year. We retrospectively examined NPS medical reports from the climbing seasons of 1992-2011. Medical complaints, diagnoses, treatment, provider training, and overall numbers of injuries and illness were analyzed. Fatalities were included only if they were cared for by NPS medical personnel prior to death. Of the 24,079 climbers on

Using mobile health to expedite access to specialty care for youth presenting to the emergency department with concussion at highest risk of developing persisting symptoms: a protocol paper for a non-randomised hybrid implementation-effectiveness trial.

INTRODUCTION: Paediatric concussion is a common injury. Approximately 30% of youth with concussion will experience persisting postconcussion symptoms (PPCS) extending at least 1 month following injury. Recently, studies have shown the benefit of early, active, targeted therapeutic strategies. However, these are primarily prescribed from the specialty setting. Early access to concussion specialty care has been shown to improve recovery times for those at risk for persisting symptoms, but there are disparities in which youth are able to access such care. Mobile health (mHealth) technology has th

Unravelling the diversity of Scalibregma Rathke, 1843 (Annelida: Polychaeta: Scalibregmatidae) from southeast Brazilian coast.

Scalibregmatidae Malmgren, 1867 is a relatively small family of annelids, with only two taxa reported to Brazil. In this work, we expand the scientific knowledge of this group by presenting two new Scalibregma Rathke, 1843 species. The new species S. lanai sp. nov. and S. cinthyae sp. nov. were collected during an expedition to the Campos and Santos oceanographic basins. Scalibregma cinthyae sp. nov. is unique among its congeners by the presence of two rows of paired lobes on the mouths upper and lower lips, whereas S. lanai sp. nov. is unique among its congeners by the presence of two rows of

The signs and symptoms of Ernest Shackleton.

Ernest Shackleton, an accomplished Antarctic explorer, developed a life-threatening illness during the Discovery Antarctic expedition of 1901-4. His documented signs and symptoms included inflamed gums attributed to scurvy, severe dyspnea, and exercise intolerance, presenting in a setting of nutritional deficiency. Physical examinations at a later date, also following a prolonged diet of limited fresh food, revealed a pulmonary systolic murmur. Thiamine deficiency with cardiomyopathy, either alone or subsequently exacerbated by advanced scurvy, may have been a prominent cause of Shackleton's c

The SAVE FGR study: Short term variation Analysis versus Visual Evaluation of cardiotocography in early-onset Fetal Growth Restriction to trigger expedited birth - study protocol for a stepped wedge cluster randomized trial.

BACKGROUND: Early-onset fetal growth restriction (eoFGR) is associated with high risks of adverse outcomes and a key dilemma in clinical obstetrical practice in determining the optimal timing of birth. Cardiotocography (CTG) is an important tool for antepartum fetal surveillance and typically assessed visually. Computerised CTG including short term variation (STV) analysis offers an objective analysis by quantifying the beat-to-beat variation in the fetal heart rate. Currently, there is a lack of randomized studies of sufficient power that examine whether birth intervention based on such quant

Clinical features and health-threatening conditions of the trek to Capanna Margherita.

BACKGROUND: Since trips to high altitude have become popular, reports on clinical and environmental characteristics during expeditions to popular destinations are needed. METHODS: A group of 15 healthy adults was monitored during a trek to Capanna Margherita (4556 m). A hypoxic stress test was applied before the expedition started. Environmental characteristics were acquired with a portable device. Vital signs were compared at low and high altitude, and altitude sickness was diagnosed by the Lake Louise scoring system. Ocular symptoms and intraocular pressure were recorded. RESULTS: Temperat

Reduced vagal modulations of heart rate during overwintering in Antarctica.

Long-duration Antarctic expeditions are characterized by isolation, confinement, and extreme environments. Here we describe the time course of cardiac autonomic modulation assessed by heart rate variability (HRV) during 14-month expeditions at the German Neumayer III station in Antarctica. Heart rate recordings were acquired in supine position in the morning at rest once before the expedition (baseline) and monthly during the expedition from February to October. The total set comprised twenty-five healthy crewmembers (n = 15 men, 38 ± 6 yrs, n = 10 women, 32 ± 6 yrs, mean ± SD). High frequency

A faunal inventory of methane seeps on the Pacific margin of Costa Rica.

The methane seeps on the Pacific margin of Costa Rica support extensive animal diversity and offer insights into deep-sea biogeography. During five expeditions between 2009 and 2019, we conducted intensive faunal sampling via 63 submersible dives to 11 localities at depths of 300-3600 m. Based on these expeditions and published literature, we compiled voucher specimens, images, and 274 newly published DNA sequences to present a taxonomic inventory of macrofaunal and megafaunal diversity with a focus on invertebrates. In total 488 morphospecies were identified, representing the highest number o
