A 49-year-old female patient previously treated for scleritis and uveitis-induced cataract in the right eye presented with a subretinal white lesion in the same eye. With a preliminary diagnosis of choroidal tumor, enucleation of the eyeball was performed in accordance with the patient's request. Histologic and immunohistologic examinations were consistent with immunoglobulin G4-related disease. The case demonstrates that it is important to consider IgG4-related disease in the differential diagnosis of an intraocular tumor.

A total of 99 patients with bilateral retinoblastoma who had been treated between 1965 and 1982 were reexamined in April 1988 to study the late effects of treatment. OBJECTIVE: To report on a patient who committed self-enucleation with his own fingers.
METHODS: Case report of a 28-year-old man who self-enucleated his right eye. After ophthalmological treatment, he was referred to the psychiatric department for further evaluation and treatment.
RESULTS: Reconstruction of the orbit was performed, and neither ophthalmological nor neurological complications were noted. The psychiatric history revealed continuous drug abuse (cannabis and amphetamines). The diagnosis of a drug-induced psychosis was established.

While the act of self-enucleation is rare, other self-inflicted eye injuries may be more common. Six patients are reported who have documented histories of ocular self-mutilation. Several common factors are observed in these patients and in other cases reported in the literature: (1) the patient was judged to be psychotic; (2) the patient had a tyrannical conscience; (3) a source of guilt was present; (4) this guilt was displaced to the eye; and (5) an attempt at self-inflicted eye injury was followed by relief from anxiety when completed or by frustration when injury was prevented.
