22 patients with fractures of the femoral shaft were divided into 2 groups. One group (12 patients) were given the standard postoperative physiotherapy and the remainder (10 patients) received intensive physiotherapy. Clinical examination of the knee joint was performed 2, 4 and 9 weeks postoperatively. The patients' mean age was 31 years; 15 of them were men. The fractures in 18 patients were treated with intramedullary nailing, 2 with a long-shanked Jewett nail and 2 with AO-nailing. Mobilisation and quadriceps atrophy of the injured limb of the patients given intensive physiotherapy appeare

Thirty-nine patients admitted to hospital with fractures of the neck of the femur had mean serum 25-hydroxycholecalciferol (25-(OH)D3) levels markedly lower than that of young healthy controls. There was also a significant seasonal variation in these 25-(OH)D3 levels, values in winter being about half those in summer. It is suggested that vitamin D deficiency plays an an important role in the pathology of old age.

Due to demographic changes and increased survival rates of total hip arthroplasties, the incidence of periprosthetic proximal femoral fractures is increasing. The current treatment concept requires accurate preoperative planning. Besides patient-related risk factors, fracture type, prosthesis stability, and bone quality influence whether osteosynthesis or a revision arthroplasty is required.

The atypical femoral fracture (AFF) has been attracting significant attention because of its increasing incidence; additionally, its treatment is challenging from biological and mechanical aspects. Although surgery is often required to manage complete AFFs, clear guidelines for the surgical treatment of AFFs are currently sparse. We reviewed and described the surgical treatment of AFFs and the surveillance of the contralateral femur. For complete AFFs, cephalomedullary intramedullary nailing spanning the entire length of the femur can be used. Various surgical techniques to overcome the femora

The treatment of pathological femoral neck fracture in children due to osteopetrosis is not clear as only a few cases have been reported to date. We encountered a 7-year-old osteopetrotic girl with asynchronous bilateral femoral neck fractures. Radiographical assessment showed markedly dense bones with loss of corticomedullary differentiation, sandwich vertebrae and increased density of the ribs. Her mother had a similar radiological picture. Genetic testing showed CLCN-7 mutation which confirmed the diagnosis of autosomal dominant osteopetrosis. As the parents opted against operative treatmen

INTRODUCTION: The OTA/AO type 31 A3 intertrochanteric fracture has a transverse or reverse oblique fracture at the lesser trochanteric level, which accentuates the varus compressive stress in the region of the fracture and the implant. Intramedullary fixation using different types of nails is commonly preferred. The purpose of this study is to evaluate intertrochanteric femoral fractures with intramedullary nail treatment in regard to surgical procedure, complications, and clinical outcomes.
METHODS: From one level 1 trauma center, 216 consecutive adult intertrochanteric femoral fractures (OT…

PURPOSE: Osteogenesis imperfecta (OI) is a condition characterised by bone fragility and multiple fractures, which cause considerable morbidity in the affected patients. Most cases are associated with mutations in one of the type I collagen genes. Recently, bisphosponates have been used widely to reduce pain and the incidence of fragility fractures in OI in children, even though there have been concerns raised regarding the long-term complications of it due to their effect on the bone. The fragility fractures involving the neck of the femur in children with intramedullary rods in the femoral s

BACKGROUND: Nowadays pediatric femoral fractures are more commonly managed with operative treatment rather than conservative treatment because of more rapid recovery and avoidance of prolonged immobilization. Children between the ages of 5-13 years are treated either by traction plus hip spica and flexible/elastic stable retrograde intramedullary nail, or external fixators in the case of open fractures. The aim of this study is to evaluate the outcome of pediatric femoral shaft fractures treated by stainless steel flexible intramedullary nail in children between 5 and 13 years of age.

Displaced femoral head fractures often require open reduction and internal fixation. This article describes 3 cases of displaced large-fragment femoral head fractures (OTA 31-C1.3) that were treated by arthroscopic reduction and internal fixation, which was accomplished using an anterolateral viewing portal, an anterior portal, and an accessory distal anterior working portal. By 3 months postoperatively, all 3 patients had returned to full function. Therefore, we conclude that an arthroscopic approach results in stable fixation and early joint motion, thereby effectively treating displaced fem

BACKGROUND: Open proximal femoral fractures are rare injuries that often result from wartime high-energy causes. Limited data exist regarding the treatment and complications of these injuries.
METHODS: We retrospectively reviewed the records of combat casualties treated at two institutions between March 2003 and March 2008. The casualty patient databases, medical records, radiographs, and laboratory data were reviewed to determine time to union, complication rates, and patient outcomes.
RESULTS: Forty-one patients (thirty-nine men and two women) with a mean age of 25.7 years were identified …

INTRODUCTION: Trauma remains a substantial cause of paediatric morbidity and mortality. Femur fractures are common in children, and frequently are the result of high-energy mechanisms of injury. A complete description of missed injuries in this population has not previously been described.
MATERIALS AND METHODS: A retrospective chart review of patients presenting to the Alberta Children's Hospital with femoral shaft fractures was performed. Associated injuries, and injuries with delayed diagnosis were recorded.
RESULTS: Twenty-percent of the patients suffered at least one other injury with t

The purpose of this presentation is to describe the Gotfried (nonanatomical) reduction technique of unstable subcapital femoral fractures. In addition, to report, retrospectively, on the radiological outcomes of displaced subcapital femoral fractures reduced with the Gotfried reduction method. The technique involves creation of a positive buttress between the proximal and distal fracture parts to prevent future reduction deterioration once fracture line bone resorption takes place as well as reduction in valgus. Presented are unstable Garden 3/4 cases, 4 cases stabilized with the Physiological…

Periprosthetic supracondylar femur fractures following total knee arthroplasty are infrequent, but challenging to clinicians. In this article, we present a 74-year-old female case of bilateral supracondylar femoral periprosthetic fractures which were treated with locking plates. The fractures were healed with osseous union, the range of motion was 0-110° for each knee, and there was a 6° residual varus deformity in the left knee. The functional results were excellent during two-year follow-up. Open reduction and locking plate fixation are effective treatment methods for periprosthetic supracon

The surgical management of ipsilateral fractures of the femoral neck and shaft presents a difficult and challenging problem for the orthopaedic surgeon. The purpose of the present study was to report the mid-term results and complications in a series of patients who sustained ipsilateral femoral neck and shaft fractures and treated in our trauma department with a single reconstruction nail for both fractures. Eleven patients were included in the study with an average age of 46.4 years. The mean follow-up was 47 months (range, 15-75 months). There were no cases of a missed diagnosis at initial

Several cases have been reported of unusual spontaneous fractures with minimal trauma in the subtrochanteric and diaphyseal areas of the femur linked to long-term bisphosphonate use. After encountering three such patients, we conducted a review of published cases. The average age of these individuals were 68 years and approximately 25% had received concomitant glucocorticoids. Histomorphometric bone biopsy studies performed in some individuals have suggested that severe suppression of bone turnover may be the underlying cause; however, cause and effect has not been firmly established. Moreover

OBJECTIVE: This retrospective study was conducted to evaluate the outcome for cattle with diaphyseal fractures of the femur, but not including capital physeal injuries.
METHODS: Sources of information were medical records of cattle having a definitive diagnosis of diaphyseal femoral fractures and telephone survey of owners.
RESULTS: Medical records for 26 cattle with femoral fracture were found; of the 20 aged less than 2 months, 15 were treated surgically, 4 conservatively (stall rest) and 1 was euthanased without treatment. The surgical treatment varied according to the configuration of th…
