INTRODUCTION: Peripheral neuropathy is a common feature of many vasculitic syndromes. In some patients, the neuropathy may be the sole manifestation of vasculitis.
EXEGESIS: A 74-year-old lady complained of pain and weakness of the lower limbs. In her history, we noted right optic neuritis, monoclonal gammopathy and dyslipidemia treated by fenofibrate. Clinical examination showed proximal muscle strength deficit of lower limbs, with quadriceps femoral muscles atrophy. Muscle stretch reflexes were absent. There was no deficit in light touch and pain sensation, but proprioception was impaired.

Complications following lateral retroperitoneal transpsoas lumbar fusion (LLIF) surgery include femoral nerve apraxia, bowel/bladder injury, ureteral injury, and potentially, as illustrated in this case report, Guillain-Barré syndrome. Guillain-Barré syndrome (GBS) is an autoimmune inflammatory condition that typically presents after infection, or, less frequently, post-operatively. We report a case of GBS following elective lumbar fusion through the lateral retroperitoneal transpsoas approach (LLIF). A 56-year-old patient presented with left lower extremity (LLE) weakness on post-operative da

The study concerned electrophysiological parameters during stimulation of peroneal, tibial, sural and femoral nerves of patients with ischemia of lower limbs. The disease was clinically verified and confirmed on the basis of an ankle-brachial index (ABI). Peripheral nerves injuries caused by other reasons have been excluded from the research. For statistical evaluation of the material, groups of 86 chronic ischemic limbs (ABI < 0.91) and 57 healthy ones (ABI > 0.99) were created. All observation allowed to recognize an axonal-demyelization neuropathy, with strong axonal element. Changes concer

BACKGROUND: Entrapment or injury of the lateral femoral cutaneous nerve (LFCN) is being recognized with increasing frequency, often requiring a surgical approach to relieve symptoms. The presence of anatomic variations can lead to errors in diagnosis and intraoperative decision-making.
METHODS: This study presents the experience of a single surgeon (T.W.T.) in managing 184 patients referred with clinical issues related to the LFCN. A comprehensive review of these cases was conducted to develop a prospective surgical management algorithm. Data on the LFCN's anatomic course, pain relief outcome

The peripheral nerve stimulation (PNS) is an established therapy in the management of neuropathic pain. Here, we describe a case of successful epifascial stimulation in the proximity of the genitofemoral nerve, which was injured during an endoscopic hernioplasty. During the following months the patient developed an intense neuropathy of this nerve, which we now treated by permanent PNS with an 8-polar electrode. During the 1 year follow-up the patient described a pain reduction of nearly 70%. There were no adverse events until now. In conclusion, PNS is a promising procedure in the treatment o

This is a case report of a 60 year-old female who presented with pain on the anterolateral aspect of her right thigh. The patient had a history of placement of a Drug Administration System (DAS, Opiate pump) in August, 1998 for chronic lumbar radiculopathy and a multiply operated on spine which she had suffered with since October, 1991. She presented with the subacute onset of focal pain on the anterior aspect of her right superior iliac crest region and dysesthesias on the anterolateral aspect of her right thigh. There was no history of recent trauma, worsening low back pain, or any other sen

In diabetics, vascular examination may consider great and medium arteries and microcirculation. The aim of this study is correlation between stenosis in carotid and femoral bifurcation, ultrasound biopsy, and laser-Doppler measurements of distal microcirculation in diabetics. During a six month period, we have examined 90 diabetics in the diabetological service of the Civita Castellana Hospital (Viterbo, Italy), asymptomatic for any vascular distal, cerebral or coronary disease. Subjects were 49 +/- 20 years old with 15 +/- 5 years of diabetes; glycosilate haemoglobin was 7% (mean). 70% of dia

BACKGROUND: To describe the Sacral Hitch, vesical pexys to sacral promontory, an alternative technique to the Psoas Hitch when this is impossible to perform. We report intraoperative complications, and long-term functional results.
METHODS: From 1989 to date we performed 66 ureteroneocystostomies (UNCS) (transbladder technique and Politano-Leadbetter antireflux plasty): 51 with ancillary "Psoas Hitch" (11 Casati-Boari); 15 with "Sacral Hitch" because of the intraoperative finding of deficiency or lack of psoas tendon. Note of technique. Ureter and bladder are dissected as usual. Sacral Hitch:

BACKGROUND AND AIM: Diabetic neuropathy (DN) is a recognised risk factor for fragility fractures. However, the mechanisms linking DN, bone health, and falling risk remain unclear. We aimed to assess bone health and risk of falls, with their contributing factors, in young elderly patients with type 2 diabetes (T2D) and mild-to-moderate DN.
METHODS: We enrolled 144 subjects with T2D, excluding those with severe DN (neuropathy disability score -NDS- ≥ 9) or fracture history. Clinical and biochemical data were collected, including surrogate markers of insulin resistance, such as the triglycerides

BACKGROUND: Anterior ischemic optic neuropathy (AION) is a potentially vision threatening disorder that affects middle-aged and elderly individuals. The pathogenesis of the disease remains incompletely understood. However, the most universally accepted hypothesis suggests that a transient reduction in optic nerve head (ONH) perfusion, secondary to hypovolemia, nocturnal hypotension, or small-vessel disease, results in ischemic infarction of the anterior portion of the optic nerve.
CASE PRESENTATION: We present the case of a 61-year-old male patient with a history of arterial hypertension, typ

AIMS/HYPOTHESIS: Diabetic peripheral neuropathy (DPN) is a chronic microvascular complication that is strongly associated with poor glycaemic control and also with a worse prognosis. We aimed to evaluate the predictors of the development and progression of DPN in a cohort of high-risk patients with type 2 diabetes.
METHODS: In a prospective study, 477 patients with type 2 diabetes were clinically assessed for the presence of DPN at baseline and after a median follow-up of 6.2 years (range 2-10 years). Clinical laboratory data were obtained at study entry and throughout the follow-up. Aortic s

OBJECTIVE: Diabetic foot ulcer (DFU) is a common and serious complication of diabetes, often leading to infection, amputation and poor quality of life. Bone marrow mesenchymal stem cells (BMSCs) have shown promise in treating chronic wounds, but their therapeutic efficacy is limited due to poor survival and low regenerative activity. Low-intensity pulsed ultrasound (LIUS), a non-invasive physical modality, has been shown to enhance the biological behavior of BMSCs. LIUS in combination with BMSCs shows promising potential to promote healing of refractory foot ulcer wounds.
METHODS: Several met

AIM: Muscle bleed in hemophilics can result in compressive neuropathy and permanent disability if not treated properly in time. Twenty cases of peripheral nerve involvement in hemophilics are presented with an aim to highlight its features and the role of pulsed ultrasound in the management.
METHODOLOGY: Between January, 1999 and 2000 June, 134 patients of bleeding disorders were examined. Twenty of them presented with features of nerve palsy. In addition to usual conservative treatment, pulse ultrasound electrotherapy of 10-15 sittings was used in all the cases.
RESULTS: Femoral nerve was i

The X-linked form of Charcot-Marie-Tooth disease (CMTX1) is the second most common form of CMT. In this study we used CRISPR/Cas9 to develop new "knock-in" models of CMTX1 that are more representative of the spectrum of mutations seen with CMTX1 than the Cx32 knockout (KO) mouse model used previously. We compared mice of four genotypes - wild-type, Cx32KO, p.T55I, and p.R75W. Sciatic motor conduction velocity slowing was the most robust electrophysiologic indicator of neuropathy, showing reductions in the Cx32KO by 3 months and in the p.T55I and p.R75W mice by 6 months. At both 6 and 12 months

AIMS: Femoral nerve palsy is a potential complication of brace treatment for children with developmental dysplasia of the hip (DDH). Little is known about its causes, and previous studies have been limited by their small sample size, retrospective design, and/or being single-centre series. The aim of this study was to examine the risk factors for femoral nerve palsy in the largest prospective cohort of children to date with DDH treated using an orthosis.
METHODS: A global multicentre prospective database of children with DDH was analyzed. Those treated primarily using an orthosis were include
