Pathobiology of the human A1 pulley in trigger finger.

Eighty-nine A1 pulleys from 65 patients with trigger digits and 20 control A1 pulleys from fresh-frozen cadavers were studied comparatively with histology, immunohistochemistry, and transmission electron microscopy. In both normal and pathologic specimens, the A1 pulley was composed of two layers: an outer, vascularized, convex layer and an inner, concave, friction, flexor tendon gliding layer. In the latter, the cells and adjacent matrix had several characteristics of fibrocartilage, including chondrocytes. In trigger digits, the number of chondrocytes and adjacent extracellular matrix was si…

[Temporomandibular fibro-chondromalacia. Physiological basis. Classification on the basis of clinical characteristics and course. Spongialisation (author's transl)].

The temporomandibular joints are neural structures which develop from the cephalic ectomesenchyme of the neural crests. They are analogous to the effector organs. Their lesional abnormalities have two characteristics:--pain sensation,--"foreign body". A classification is suggested on the basis of clinical characteristics and course of articular signs due to temporo-menisco-condylar stress resulting from occlusion problems, and concomitant lesions of the articular fibrocartilage: closed and open fibro-chondromalacia. Emphasis is placed upon the constant dissociation between articular clinical f…

The histological spectrum of subperiosteal fibrocartilaginous pseudotumor of long bone (focal fibrocartilaginous dysplasia).

Clinicopathological features in six cases of focal fibrocartilaginous dysplasia (FFCD) which involved either the tibia (n = 4) or the femur … ells around cartilaginous area were positive for S-100 protein. One case contained both hyaline and fibrocartilage, and architecturally mimicked normal tendinous insertion. One case, which involved proximal tibia, was purely composed of fibrous tissue without fibrocartilage. All cases formed undulating and irregular borders against underlying cortical bone. Hist …

Distal radioulnar joint arthroscopy and the volar ulnar portal.

Pain on the ulnar side of the wrist remains poorly understood. As attention has shifted toward the myriad causes of ulnar-sided wrist pain, … agnosis and treatment of patients with injuries to the ulnar sling mechanism. It aids in triangular fibrocartilage repairs especially those involving the dorsal aspect between the ulnar styloid and the radial insertion, because the proximity of the 4, 5, and 6R portals makes triangulation of the instruments difficult. Although arthroscopy of the dorsal aspect of …

Wrist arthroscopy: principles and clinical applications.

With the development of better and smaller equipment, arthroscopy of the wrist offers the same benefits achievable with arthroscopy of the knee, shoulder, or elbow - not only diagnostic information but also a therapeutic option. Standardized techniques of performing wrist arthroscopy have been developed to evaluate the treat various wrist disorders, such as lesions of the triangular fibrocartilage complex, intra-articular distal radius fractures, and scaphoid fractures. Arthroscopy is now performed in the treatment of dorsal-wrist ganglion cysts and interosseous ligament disruptions, as well a…

Evaluation of chronic wrist pain by arthrography, arthroscopy, and arthrotomy.

Arthrography and arthroscopy of the wrist were compared as diagnostic tools in the evaluation of chronic wrist pain. Arthroscopy proved to be a more sensitive procedure than arthrography in 20 consecutive patients with confirmed wrist injuries. There was 86% correlation (the exception--one false negative arthroscopic procedure) between arthroscopy and arthrography in eight patients with triangular fibrocartilage tears. Compared with arthroscopy, there were five false negative and five false positive arthrograms when evaluating interosseous scapholunate and lunotriquetral ligament tears. Arthro…

Radiocarpal arthroscopy and arthrography in the diagnosis of ulnar wrist pain.

The purpose of this prospective study was to compare arthroscopy with arthrography in the diagnosis of ulnar wrist pain. Thirty-seven consecutive patients with ulnar wrist pain but normal routine and stress radiographs had dynamic and static radiocarpal arthrograms (R.G.H.) and arthroscopy (J.H.R.) performed. Sixteen arthrograms demonstrated a leak of contrast into the distal radioulnar joint. Arthroscopy demonstrated a perforation of the triangular fibrocartilage complex in all 16. Seven arthrograms demonstrated a leak of contrast into the midcarpal joint. Arthroscopy demonstrated lunotriquet…

[Pathology of the triangular fibrocartilaginous complex in distal radius fractures].

Distal radius fracture is actually quite rarely isolated and triangular fibrocartilaginous complex (TFCC) is often involved with the same mechanism of injury. A contemporary management of the treatment of the initial fracture is ideal and so benefits from a routine arthroscopic management. Nevertheless, in the absence of diagnosis and therefore early treatment, the symptoms may be located at the ulnar crossroads and justify specific treatment of TFCC. It is generally conducted after an accurate arthroscopic lesion classification guiding a procedure, depending on the lesion vascularization as w

Mechanically Preconditioned Biomimetic Gradient Scaffold for Tendon-to-Bone Regeneration in a Rabbit Rotator Cuff Tear Model.

BACKGROUND: Healing of the tendon-bone interface (TBI) after rotator cuff injury is limited, often leading to scar-mediated repair. Decellularized tendon fibrocartilage-bone composite (dTFBC) and bone marrow mesenchymal stem cell sheets (BMSCS) improve repair in preclinical models, but full scaffold encapsulation with mechanical preconditioning remains unexplored. PURPOSE: To evaluate TBI regeneration using mechanically preconditioned, BMSCS-encapsulated dTFBC scaffolds in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 48 male New Zealand White rabbits were r…

Immunohistochemical characterization of reparative tissue present in human osteoarthritic tissue.

Studies involving disease progression in osteoarthritis (OA) have typically focused on the deterioration of native articular cartilage (AC) rather than the de novo cartilage which is frequently present. In general, there are two categories of de novo tissue observed in OA: (1) a pannus-like fibrocartilage that overlays native AC and (2) osteophytes. In this study, 30 AC samples representing a range of disease stages consistent with early to intermediate OA were examined for the occurrence of pannus-like tissue. All AC samples were examined immunohistochemically and compared with cartilage from…

Hedgehog signaling underlying tendon and enthesis development and pathology.

Hedgehog (Hh) signaling has been widely acknowledged to play essential roles in many developmental processes, including endochondral ossification and growth plate maintenance. Furthermore, a rising number of studies have shown that Hh signaling is necessary for tendon enthesis development. Specifically, the well-tuned regulation of Hh signaling during development drives the formation of a mineral gradient across the tendon enthesis fibrocartilage. However, aberrant Hh signaling can also lead to pathologic heterotopic ossification in tendon or osteophyte formation at the enthesis. Therefore, th…

Part II: Temporomandibular Joint (TMJ)-Regeneration, Degeneration, and Adaptation.

PURPOSE OF REVIEW: Elucidate temporomandibular joint (TMJ) development and pathophysiology relative to regeneration, degeneration, and adaption. RECENT FINDINGS: The pharyngeal arch produces a highly conserved stomatognathic system that supports airway and masticatory function. An induced subperiosteal layer of fibrocartilage cushions TMJ functional and parafunctional loads. If the fibrocartilage disc is present, a fractured mandibular condyle (MC) regenerates near the eminence of the fossa via a blastema emanating from the medial periosteal surface of the ramus. TMJ degenerative joint diseas…

[Anterior and lateral disk and capsule tissue of the temporomandibular joint and its relevance for temporomandibular joint diagnosis].

For the temporomandibular joint, such disc fixations in the anterior and lateral regions have not been described, even in MRT findings of anterior disc dislocations without reduction. By observing collagenous fibres of the anterior and lateral discuss and capsule tissue, we sought to establish whether tensile load capacities typical for fibrocartilage also apply to the discuss articularis of the mandibular joint in sagittal direction. MATERIALS AND METHODS: Using dissecti…
