Central oculomotor disturbances and nystagmus: a window into the brainstem and cerebellum.

BACKGROUND: Oculomotor disturbances and nystagmus are seen in many diseases of the nervous system, the vestibular apparatus, and the eyes, as well as in toxic and metabolic disorders. They often indicate a specific underlying cause. The key to diagnosis is systematic clinical examination of the patient's eye movements. This review deals mainly with central oculomotor disturbances, i.e., those involving smooth pursuit, saccades, gaze-holding, and central types of nystagmus. METHODS: We searched the current literature for relevant publications on the diagnosis and treatment of oculomotor distur

A reinterpretation of the purpose of the translational vestibulo-ocular reflex in human subjects.

In a prior study we reported that the human translational vestibulo-ocular reflex (tVOR) in response to vertical (bob) 2 Hz oscillations generated eye rotations of only 60% of those required to keep the eyes pointed at a stationary visual target, whether located at near (approximately 17 cm) or far (2 m). Best responses occurred in ambient illumination and we concluded that relative image motion between the target and background was an important determinant of tVOR behaviour. To investigate further how visual conditions influenced tVOR, we measured responses as subjects binocularly viewed the

[Fixation disparity with the Pola pointing test: not representative for eye position under natural viewing conditions].

BACKGROUND: According to certain findings obtained with the Zeiss Polatest, H.J. Haase defined a "Fixation Disparity Type One". In this diagnosis, the "Zeigertest" is particularly important. The Zeigertest consists of a central ring presented to both eyes for fixation, a vertical clock hand presented to the right eye and two markings at the six and twelve o'clock positions presented to the left eye. All parts are surrounded by a binocularly visible frame. Subjects with a "Fixation Disparity Type One" see a misalignment between the clock hand and the peripheral markings. We investigated (1) whe

[Benign paroxysmal tonic upward gaze syndrome].

INTRODUCTION: The benign paroxysmal tonic upward gaze syndrome (BPTUG) is a rare condition. We present two new cases analyzing the clinical, genetic, evolution and therapeutic aspects. CLINICAL CASES: Case 1. A 2 year-old girl with no family history of similar disorders started, at the age of 6 months, to have episodes of upward deviation of gaze with hyperextension of the neck and vertical nystagmus of fixation, increased by nervousness and episodes of fever. Some episodes caused the patient to fall in spite of there being no alteration of consciousness. Case 2. From the age of five months a

Oculomotor apraxia and dilated cardiomyopathy with ataxia syndrome: A case report.

Dilated cardiomyopathy with ataxia syndrome (DCMA) is a rare mitochondrial condition associated with early onset cardiomyopathy and non-progressive ataxia. The cardiac manifestations may be progressive and often severe, resulting in significant morbidity and mortality. While optic nerve atrophy has been described in patients with DCMA, to our knowledge, there have been no reports of additional ocular phenotypes. We present two related Dariusleut Hutterite patients with documented DCMA syndrome and disorders of ocular motility: poor smooth pursuit and difficulty initiating saccadic eye movement

Intermediate results of sutureless intrascleral posterior chamber intraocular lens fixation.

PURPOSE: To report the intermediate multicenter results of a technique of sutureless intrascleral fixation of a standard 3-piece posterior chamber intraocular lens (PC IOL) in the ciliary sulcus. SETTING: Four European ophthalmology centers. METHODS: A technique for sutureless intrascleral fixation of the haptics of a standard 3-piece PC IOL was retrospectively evaluated. The technique uses standardized maneuvers to fixate the PC IOL without need for special haptic architecture or preparation or haptic suturing. All patients having IOL implantation by the technique were evaluated for preoper

Advances in ophthalmic structural and functional measures in multiple sclerosis: do the potential ocular biomarkers meet the unmet needs?

PURPOSE OF REVIEW: Multiple sclerosis is a heterogeneous disorder. Biomarkers to monitor disease activities are highly desirable especially because of the recent shift toward personalized medicine that coincides with the expansion of disease-modifying therapy. The visual system is highly involved in multiple sclerosis, and the rapid advancement of ophthalmic techniques has boosted the development of potential ocular biomarkers for multiple sclerosis management. RECENT FINDINGS: Recent studies have found that the rapid thinning of the peripapillary retinal nerve fiber layer and ganglion cell-i

Assessment of visual function in chronic progressive external ophthalmoplegia.

AIMS: To assess the visual function of patients with chronic progressive external ophthalmoplegia (CPEO) using the Visual Function Index (VF-14). To identify discriminatory questions that reflect visual disability in mitochondrial ocular myopathies. To investigate the relationship between visual impairment and the ocular parameters routinely measured in clinical practice. METHODS: We studied 40 CPEO patients. Each patient underwent ophthalmological assessment, including best-corrected visual acuity, ptosis measures, and fundus examination for pigmentary retinopathy, and orthoptic assessment i

'Staircase' square-wave jerks in early Parkinson's disease.

BACKGROUND/AIMS: Visually guided saccades and gaze-fixation ability were recorded in patients with early Parkinson's disease (PD). METHODS: Magnetic search coil system was used to measure horizontal and vertical eye positions. RESULTS: 'Staircase' visually guided saccades (multiple hypometric saccades separated by an intersaccadic interval) and 'staircase' square-wave jerks (SWJ) were present in all patients. The SWJ amplitude (total amplitude of the series of hypometric saccades comprising the SWJ) was abnormally large (mean 2°). SWJ frequency was also abnormally high (50-70 intrusions/min)

Reading-related oculomotor testing and training protocols for acquired brain injury in humans.

Many individuals with acquired brain injury (ABI) report reading problems of oculomotor origin. These may include frequent loss of place, skipping of lines and difficulty shifting to the next line of print. We describe two protocols for the testing and training of reading-related eye movements in adult individuals with ABI (traumatic brain injury [TBI] and stroke with hemianopia), who experience oculomotor-based symptoms when reading. These protocols use objective eye movement recording techniques and computer-based stimulus presentation and analysis. One protocol tests and the other trains ba

Diagnostic occlusion and clinical management of latent hyperphoria.

Occasionally in patients who have symptoms suggestive of a vertical heterophoria no deviation is found, even on careful examination. Six days of occlusion have been recommended for uncovering such "latent" vertical deviations. We investigated prolonged monocular occlusion (Part I) and found that vertical deviations of varying amounts manifested on symptomatic and asymptomatic subjects. Thus, results of prolonged occlusion can be difficult to interpret. Nonadaptive vertical vergence systems have been implicated in development of symptoms. Therefore, it may be that diagnostic monocular occlusion

Nonvisual eye position control in a patient with ocular lateropulsion.

For elucidating the nonvisual eye position control system, we studied the accuracy of saccades toward the remembered position (memory-guided saccades) in a patient with ocular lateropulsion having tonic bias of the eye position. Although the saccadic dysmetria caused by ocular lateropulsion was recorded during memory-guided saccades in the same manner as visually guided saccades, the eyes reached precisely the intended position by corrective saccades. The mean latency of corrective saccades was significantly longer than that of corrective saccades observed for visually guided saccades. This lo

Rotation and decentration of an undersized plate-haptic trifocal toric intraocular lens in an eye with moderate myopia.

We present the case of a 56-year-old woman with moderate myopia and bilateral cataract who had cataract extraction and intraocular lens (IOL) implantation. Due to the patient's desire for spectacle independence, a trifocal IOL with toric correction for astigmatism was implanted. During the follow-up, it became obvious that the implanted IOL had rotated and tilted due to insufficient fixation in the large capsular bag of the myopic eye. An IOL explantation was therefore performed, and the original IOL was exchanged for a bifocal toric IOL with a larger overall diameter. Stable fixation of the I

A novel method of head fixation for the study of rodent facial function.

The rodent vibrissial system offers an excellent model for the study of both sensory and motor function. It has been widely employed to gather data pertaining to sensory and motor function involving the 5th and 7th cranial nerves and the central nervous system. Existing methods of head fixation for precise measurements of ocular and vibrissial function involve exposing the cranium and applying dental cement from which two or more threaded rods emerge. This common approach is suboptimal, requiring a relatively complicated implantation procedure, and results in a large, chronic interface between

Morphometric analysis of T lymphocyte compartmentation in experimental autoimmune uveoretinitis.

Experimental autoimmune uveoretinitis (EAU) in the Lewis rat is characterized by extensive infiltration of inflammatory cells into all compartments of the eye, only some of which become irreversibly damaged. The apparent differences in the pathogenic impact of inflammatory cells within different ocular compartments may suggest that different mechanisms underlie cellular infiltration and selective tissue destruction. In order to investigate the importance of T lymphocyte infiltration, we carried out a precise topographical and temporal analysis of T cell infiltration into five compartments of t

Fixation and its role in the causation, laterality and location of pterygium: a study in amblyopes and non-amblyopes.

AIM: To evaluate the role of fixation in causing pterygium and determining its laterality and location. METHODS: This is a prospective, observational, case-control study. Cases were defined as patients with primary pterygium who had unilateral amblyopia with eccentric fixation. Controls were age-matched patients with primary pterygium, but without amblyopia and eccentric fixation. All patients underwent complete ocular, orthoptic, and systemic examination and a detailed risk-factor assessment (latitude of residence, exposure to sunlight, sand, and a high-reflectance environment). The role of

Microorbitalism: a technique for orbital rim expansion.

A simple technique for orbital aperture expansion to facilitate placement of ocular prostheses is described. Both superolateral and inferolateral orbital margins are released by means of a single burr hole craniectomy of the frontosphenoid bone behind the orbital process of the frontal bone. Vertical and horizontal marginal lengthenings are performed by a rotatory displacement of one bone segment alongside the other. The expanded osseous aperture is secured with wire and plate-and-screw fixation following a supraorbital rim craniectomy to allow an adequate fit. The result provides for easier a

Age-related differences in resting and event-evoked ocular response dynamics.

Ocular measures are increasingly used as non-invasive proxies of cognitive processes such as attention and listening effort. However, their interpretation in aging populations is complicated by concurrent changes in ocular physiology and oculomotor control, raising a critical question: to what extent do age-related differences in these measures reflect cognitive rather than other physiological factors? Here, we dissociate these contributions by characterizing ocular dynamics (resting and event-evoked) during passive fixation in younger (N = 98, 18-35 years) and older adults (N = 71, 60 + years
