Mediastinitis as a rare complication of an odontogenic infection. Report of a case.

Mediastinitis occasionally develops as a complication of cervical or odontogenic infections resulting in neck sepsis, which spreads to the mediastinum via the cervical facial planes. Nowadays, this rare complication of pyogenic orofacial infections has an high-unchanged mortality rate. Delayed diagnosis and inadequate mediastinal drainage are the primary causes of this high mortality rate. Aggressive empirical antibiotic combinations should be directed towards this polymicrobial process. Antibiotic therapy alone is inadequate and the mainstay of treatment is aggressive surgical drainage throug

Bloodstream infections in the elderly.

PURPOSE: Bacteremia in the elderly is associated with a different clinical course and a higher mortality rate when compared with that in younger age groups. In order to examine these issues in the aged, we reviewed the clinical course and factors involved in the outcome of 100 episodes of bloodstream infections in patients over 65 years of age. PATIENTS AND METHODS: The hospital records of all patients over 65 years of age at The Mount Sinai Hospital with a positive blood culture result during the period October 1984 to October 1986 were reviewed. Place of residence before hospital admission,

Long-Term Follow-Up in Children With Focal Cortical Dysplasia IIId for Early Brain Injuries, Including Neuropathological Findings.

BACKGROUND: Early cerebral injury has a close relationship with epilepsy and focal cortical dysplasia Ⅲd. We investigated children with focal cortical dysplasia Ⅲd who underwent surgery for epilepsy. METHODS: We selected 49 patients from among 260 pediatric patients who had undergone epilepsy surgery, analyzing their clinical materials and pathology data. The selected patients had been followed for more than two years. RESULTS: The 49 patients were divided into seven groups based on different early brain injuries. There was a significant difference (P < 0.05) between Engel class I ratio of c

OCULAR BARTONELLOSIS PRESENTING WITH FOCAL CHOROIDITIS, FOCAL RETINITIS, BRANCH RETINAL ARTERY OCCLUSION, AND PARACENTRAL ACUTE MIDDLE MACULOPATHY: LONGITUDINAL MULTIMODAL IMAGING ANALYSIS.

BACKGROUND/PURPOSE: To report an atypical case of ocular bartonellosis evaluated longitudinally with multimodal imaging and review the literature on the less common posterior segment manifestations of bartonella infection. METHODS: Case report and review of the literature. RESULTS: A 26-year-old man presented with focal choroiditis in the right eye and focal retinitis, a branch retinal artery occlusion, and paracentral acute middle maculopathy in the left eye after a systemic prodrome including headache and lymphadenopathy. Serologic testing revealed positive bartonella immunoglobulin G with

Rituximab as a Preemptive Treatment to Prevent Recurrence of Primary Focal Segmental Glomerulosclerosis: A Novel Approach.

OBJECTIVES: Primary idiopathic focal segmental glomerulo-sclerosis is a serious disease, frequently progressing to end-stage kidney failure. Management of recurrence after kidney transplant is challenging despite multiple proposed therapeutic approaches. Available treatment for focal segmental glomeru-losclerosis recurrence is plasma exchange, intravenous cyclosporine, and rituximab. In this study, we investigated kidney transplant recipients with focal segmental glomerulosclerosis who were at high risk for recurrence. Patients were given preemptive rituximab at day 0 posttransplant. MATERIAL

[Collapsing glomerulopathy].

Collapsing glomerulopathy is a cause of nephrotic syndrome with massive proteinuria secondary to podocyte proliferation and glomerular collapse. It is characterized by an almost inevitable progression to end stage renal failure, poor response to treatment and high post-transplant recurrence. Its frequency has increased in recent years due to its common association with Human Immunodeficiency Virus (HIV) infection and the growing recognition of new etiologic agents such as drugs and parvovirus B19. Therefore, it is a disease of growing interest for clinicians. The aim of this review is to updat

Actinomycosis of the masseter muscle: report of a case and review of the literature.

A rare case of cervicofacial actinomycosis arising primarily in the masseter muscle is described. The patient was a healthy 74-year-old woman who was not immunocompromised and had no other primary pathological finding in the oral cavity. The importance of the differential diagnosis for this unusual infection is demonstrated with tumoral pathological findings. Possible predisposing factors as well as diagnostic and therapeutic methods are discussed.

Craniocervical necrotizing fasciitis in Ile-Ife, Nigeria.

Sixteen cases of necrotizing fasciitis were seen at the Obafemi Awolowo University Teaching Hospital, Ile-Ife, Nigeria from 1990 to 2000. Primary craniocervical involvement was recorded in seven patients (five men and two women). The clinical records of five patients were sufficiently detailed to allow us to report their age, aetiology, predisposing illness, clinical features, complications, management regimen and outcome. The patients were aged 30-75 years and in four of them odontogenic infections were the cause of the condition. Hypertension, diabetes mellitus and obesity were the underlyin

Descending necrotizing mediastinitis. Advantage of mediastinal drainage with thoracotomy.

Descending necrotizing mediastinitis can occur as a complication of oropharyngeal and cervical infections that spread to the mediastinum via the cervical spaces. Delayed diagnosis and inadequate mediastinal drainage through a cervical or minor thoracic approach are the primary causes of a high published mortality rate (near 40%). Between 1985 and 1992, six men (mean age, 49 years) with descending necrotizing mediastinitis were surgically treated at our institution. The primary oropharyngeal infection was peritonsillar abscess (three cases) and odontogenic abscess (three cases). In all cases, o

[The correlation between dental pathology and ophthalmic pathology].

The study aims to assess the correlation between dental pathologies and ophthalmic diseases. Based on a statistical evaluation, the study examines the presence of dental diseases in patients suffering from optic neuritis or uveitis in comparison to a control group of subjects. The possible interactions between the two ophthalmic pathologies and dental diseases are then analyzed in order to highlight possible clinical signs which may be useful for their identification.

[Suppurative cervico-mediastinitis of odontogenic origin].

Five cases are presented, of suppurated cervico-mediastinitis developed after acute suppurated apical parodontitis of a lower molar tooth. In two cases the severity and the extension of the lesions resulted in a fatal outcome. In one of the cases the patient also had pericarditis, pleurisy and purulent meningitis, and in the second case the necrotizing mediastinitis extended to the pleural and pericardic cavities, and was associated to bilateral bronchopneumonia. In three cases the patients were cured following cervical mediastinotomy, efficient mediastinal aspiration of the anterior and of th

Ultrasonographic and computed tomographic appearance of focal tuberculosis of the liver. A case report.

A 50-year-old woman presented with a focal liver mass which mimicked the clinical and imaging (ultrasonographic and computed tomographic) characteristics of a pyogenic or amoebic abscess. Ultrasonographic-guided liver biopsy demonstrated features of a focal tuberculous abscess of the liver.

Selective anesthesias of peripheral branches of the trigeminal nerve due to odontogenic infection.

Infection of dental origin may induce anesthesias of peripheral branches of the trigeminal nerve and should be considered in the differential diagnosis of such neuropathies. This article presents two cases in which focal odontogenic infection caused sensory disruption at peripheral points along the maxillary and mandibular divisions of the trigeminal nerve. In both cases resolution of symptoms followed extraction of teeth responsible for the infection.
