Bazex syndrome (acrokeratosis paraneoplastica) is characterized by a psoriasiform eruption that favors acral sites and has been associated with an underlying malignancy in all reported cases. Of the 93 patients in this series, 89 were male with a mean age of 60 +/- 8.5 years. Squamous cell carcinomas of the head and neck and squamous cell tumors of unknown primary with cervical lymph node metastases were the most commonly associated neoplasms, suggesting that the factor(s) responsible for the development of the syndrome are relatively specific for tumors of the upper aerodigestive tract. Plasma cell pododermatitis was diagnosed in 2 cats with enlargement of the metacarpal and metatarsal footpads, ulceration of one of the affected footpads, and a history of chronic hemorrhage from the ulcerations. One cat was anemic (PCV, 14.6%). The ulcers were debrided and sutured to control hemorrhage, and the cats were treated with immunosuppressive doses of corticosteroids. Both cats had considerable reduction in footpad size after 3 to 4 weeks of treatment. Although there is evidence to suggest that plasma cell pododermatitis might be immune-mediated, or perhaps an allergic disease, the c

Drs. Black and Fenske have presented a topic that fits in very well with the discussions provided by Dr. Graff concerning overall patient evaluation and the understanding that skin diseases may be external expressions of internal disease. As podiatrists, we are frequently exposed to patients who present with skin conditions secondary to rheumatic diseases or other forms of connective tissue disease. This review, with illustrations, will help us to improve our diagnostic abilities.

Viruses are considered intracellular obligates with a nucleic acid RNA or DNA. They have the ability to encode proteins involved in viral replication and production of the protective coat within the host cells but require host cell ribosomes and mitochondria for translation. The members of the families Herpesviridae, Poxviridae, Papovaviridae, and Picornaviridae are the most commonly known agents for cutaneous viral diseases, but other virus families, such as Adenoviridae, Togaviridae, Parvoviridae, Paramyxoviridae, Flaviviridae, and Hepadnaviridae, can also infect the skin. We report on a 58-year-old woman with long-lasting (36 years) chromomycosis on the foot and secondary self-inoculation from foot to hand 4 years ago. Mycological classification was performed after culture on Sabouraud glucose agar. We used haematoxylin and eosin and Giemsa staining and an antibody to heat shock protein (HSP) 27 (Stress Gen, Clone G3.1) on paraffin-embedded and cryostat specimens of chromomycosis. The mycological culture revealed the fungus Fonsecaea pedosoi. Histopathology revealed dermal fibrosis with persistent fungi (Medlar bodies), numerous mast cells and pseudoepithelioma

For the past 4 years a 23 y-old female has noticed erythema on the dorsum of the right foot, where skin was grafted due to a traffic accident 20 years ago. She visited the Department of Dermatology of Gifu Prefectural Hospital on Oct. 19, 1998; her general health was good. Physical examination disclosed a swelling with erythema, papules and pustules on the dorsum of the right foot. The results of routine laboratory investigations were within normal limits except for the white blood cell count (11,300/mm(3)), blood sediment rate (25 mm/hrs), C reactive protein (1.21) and rheumatoid factor (x 16

A case of pseudo-Kaposi sarcoma of the feet (acroangiodermatitis) is reported. The patient showed clinical and light-microscopic signs closely resembling Kaposi's sarcoma, but by electron microscopy of skin biopsies definite signs of degeneration of vascular walls and infiltrating cells were demonstrated. Electron microscopy should be applied when conventional histology is inconclusive or fits poorly to the anamnestic information and/or clinical appearance of the lesions.

Soft tissue conditions of the foot vary considerably. They include problems of the skin and subcutaneous tissues, tendons and the plantar aponeurosis, ligaments and joints, and supporting structures, including vessels and nerves. Some conditions occur from everyday use, others are caused by continuous and repetitious motions from a particular activity or sport. Overuse syndromes occur from the fatigue of soft tissues through continued use beyond the body's ability to heal microscopic daily wear and tear. Acute trauma occurs when the powerful forces generated by the running or jumping athlete f

Elephantiasis nostras verrucosa (ENV) comprises an uncommon skin disease characterised by dermal fibrosis with hyperkeratotic, verrucous and papillomatous lesions that usually occur after chronic lymphoedema. We describe the case of a 56-year-old-man with neurofibromatosis type 1, no known surgical history, no chronic medication and no travel history, presenting with worsening non-pitting oedema and impressive foul-smelling mossy plaques and cobblestone-like nodules in both legs and feet, especially on the right, compatible with the diagnosis of ENV.

Dactylitis is diffuse swelling of the digits that is usually related to an underlying inflammatory or infiltrative disorder. Psoriatic arthritis (PsA) is the most common severe disease thought to cause dactylitis. Our understanding of the pathogenesis of PsA-related dactylitis comes from experimental animal models of PsA-like disease, as well as advances in imaging and other clinical studies. Clinical trials in PsA have increasingly included dactylitis as an important secondary outcome measure. These studies indicate that cytokines drive multi-locus microanatomical pan-digital pathology.
