Novel management of granuloma formation secondary to dermal filler: A multi-modality approach.

BACKGROUND: Dermal fillers for soft tissue augmentation have become increasingly popular among patients of all ages and ethnicities. With more widespread use, there has been an increased incidence of adverse reactions, one of which is the granulomatous foreign body reaction (GFBR). MATERIALS & METHODS: We present a three patient case series in which GFBR secondary to dermal filler was successfully treated with a multi-leveled approach. The first modality involves intralesional injection of a mixture containing 1 cc of 5-fluorouracil (5-FU), 0.5 cc of dexamethasone sodium phosphate, and 0.1 cc

Review of models for titanium as a foreign body.

A growing number of theories has evolved attempting to explain the process of dental implant failure. Titanium implants utilized outside of the mouth have exhibited breakdown through a foreign body reaction. Phenomena occurring in the body, such as passive dissolution, osteolysis and metallosis, have not been discussed relative to dental implants. The dental community must consider the full spectrum of implant interactions within the body to understand the differences and similarities within the mouth.

Cervical thorium dioxide granuloma ('thorotrastoma').

An elderly woman had an expanding cervical mass that entrapped and compressed the adjacent cranial nerves, blood vessels, and muscles. The mass was dense on radiographs, extended from the skull base to low neck in the prevertebral and parapharyngeal tissues, and showed mixed intensity on MR. A previous direct carotid arteriogram with thorium dioxide as the contrast agent suggested the histologically proved diagnosis of a cervical thorium dioxide granuloma ("thorotrastoma").

[Plexiglas (PMMA) in the anterior chamber. An experimental study on biological compatibility].

For six months the anterior chambers of rabbits were examined at regular intervals with the slit-lamp photograph. The reactions of iris, aqueous, and cornea were observed after implanting one PMMA-disc (0.25 mm thickness and 1.5 mm diameter) through a lancet incision. At the place where the disc touched the iris one could see during the half year period hyperaemia iridis, ciliary injection, atrophia iridis, and foreign-substance-granuloma of the iris with partial adhesion of the iris wrinkles. The cause for these reactions is a toxin in the PMMA. Mechanical irritation and plastic alteration co

Cytopathology of intraocular lens implantation.

The cytopathology of intraocular lens (IOL) following implantation is reviewed. A newly placed implant attracts macrophages and these settle on its surfaces to form optically clear membranes composed of so-called fibroblast-like cells and a film of proteinaceous material. The membranes apparently become tougher and more firmly adherent with time. The fibroblast-like cells have phagocytic abilities and may form fibrous structures on the surface of implants in complicated situations. Few multinucleated giant cells may be seen in successful cases, but great numbers of giant cells are indicators o

[Tuberculosis of the peripheral lymph nodes].

In Germans as well as in "foreign workers" acute tuberculous lymphnode disease is most often seen in the drainage areas of old sometimes even calcified tuberculous processes. The infective agent is almost always mycobacterium tuberculosis. Resistant strains are rarely found even in recurrent disease. Differential diagnosis includes tulareaemia, cat scratch syndrome, lues, and foreign body reaction. In those cases in which the histopathologic findings are similar to productive tuberculosis, sarcoidosis and epitheloid cell reaction in the drainage area of malignant tumors have to be taken into a

Tissue Compatibility of SN-38-Loaded Anticancer Nanofiber Matrices.

Delivery of chemotherapy in the surgical bed has shown preclinical activity to control cancer progression upon subtotal resection of pediatric solid tumors, but whether this new treatment is safe for tumor-adjacent healthy tissues remains unknown. Here, Wistar rats are used to study the anatomic and functional impact of electrospun nanofiber matrices eluting SN-38-a potent chemotherapeutic agent-on several body sites where pediatric tumors such as neuroblastoma, Ewing sarcoma, and rhabdomyosarcoma arise. Blank and SN-38-loaded matrices embracing the femoral neurovascular bundle or in direct co

[Pulmonary microcalcification with miliary foreign-body granulomas. Residuals of cured Pneumocystis carinii pneumonia in AIDS].

Pneumocystis carinii pneumonia (PCP) is one of the most frequent infectious lung diseases in immunocompromised patients, especially in AIDS cases. The present case report describes pulmonary miliary microcalcifications with and without foreign-body reaction in a patient with successfully treated PCP who was known to have AIDS. This form of residues of completely healed PCP tends to be rare. Other infectious granulomas, foreign-body granulomas and sarcoidosis must be differentially diagnosed. After cautious decalcification, pneumocysts can be demonstrated with methamine silver (Grocott). It is …

Chronic, Symptomatic Orbital Inflammation Resulting From Retained Bone Wax.

A 58-year-old man presented with a 9-month history of a chronically draining surgical wound and low-grade periorbital inflammation following a right lateral orbitotomy. Imaging of the right orbit revealed a peculiar lesion in the right lateral orbit that was hypointense on both T1- and T2-weighted MRI with peripheral enhancement. Exploratory orbitotomy and biopsy established the diagnosis of a chronic foreign body inflammatory reaction to bone wax; symptoms resolved following evacuation of the retained foreign material.A 58-year-old man developed a chronic, symptomatic, inflammatory reaction t

Detailed analysis of allergic cutaneous reactions to spinal cord stimulator devices.

The use of spinal cord stimulation (SCS) devices to treat chronic, refractory neuropathic pain continues to expand in application. While device-related complications have been well described, inflammatory reactions to the components of these devices remain underreported. In contrast, hypersensitivity reactions associated with other implanted therapies, such as endovascular and cardiac rhythm devices, have been detailed. The purpose of this case series is to describe the clinical presentation and course of inflammatory reactions as well as the histology of these reactions. All patients required

[Comparative studies in chronic ocular chalcosis].

Seven eyes containing a copper foreign body for a period from 8 months to 2.5 years were studied histopathologically. Foreign bodies, containing 99% copper, were in all eyes, encapsulated and located in the anterior vitreous. Characteristic features were: formation of foreign body granuloma, especially in later stages, marked fibroblastic proliferation in the vitreous with traction retinal detachment, choroidal effusion with fibrosis and foci of chronic nongranulomatous inflammation in cyclitic membranes, iris, ciliary body and sclera. Copper could be identified by rubeanic acid and rhodanine

Experimental gypsy moth (Lymantria dispar) ophthalmia nodosa.

Ophthalmia nodosa is the nodular granulomatous inflammatory response of ocular tissue to caterpillar hairs. We experimentally simulated this condition by surgically implanting gypsy moth hairs in the rabbit cornea. We were not, however, able to produce ophthalmia nodosa solely by placing cilia in the rabbit cul-de-sac. The experimentally induced inflammation closely resembled human ophthalmia nodosa in both its clinical and histopathologic features.

[A rare form of congenital esophageal stenosis: abortive esophageal atresia (author's transl)].

This communication describes two new cases of esophageal malformation situated at the usual level of atresias. In these two stenosis, the superior segment overlapped the inferior segment a certain length, and the inferior segment adhered closely to the trachea. When recognized, this malformation responds well to instrumental dilatation.

Exploring therapy transport from implantable medical devices using experimentally informed computational methods.

Implantable medical devices that can facilitate therapy transport to localized sites are being developed for a number of diverse applications, including the treatment of diseases such as diabetes and cancer, and tissue regeneration after myocardial infraction. These implants can take the form of an encapsulation device which encases therapy in the form of drugs, proteins, cells, and bioactive agents, in semi-permeable membranes. Such implants have shown some success but the nature of these devices pose a barrier to the diffusion of vital factors, which is further exacerbated upon implantation
