Frontal sinus approach in complicated chronic rhinosinusitis with nasal polyps: Our experience.

BACKGROUND: Hyperostosis is a common consequence of chronic rhinosinusitis with nasal polyps (CRSwNP) and other chronic rhinosinusitis, affecting mainly structures of the ethmoid labyrinth, frontal and maxillary sinuses. These neo-bones are found in advanced stages of rhinosinusitis causes exacerbation because of ostia outflow blockage. Frequent relapses of the disease due to hyperostosis restenosis, represent a problem in the treatment of chronic frontal sinusitis. We report our cases experience about the treatment of frontal chronic sinusitis because of frontoethmoidal area hyperostosis, tre

Frontal sinus obliteration by a composite multifractured osteoperiosteal flap: a pilot study.

The objective result is to obliterate the frontal sinus by a neo-osteogenic tissue, which develops behind the composite multifractured osteoperiosteal flap (CMOF) tailored from the anterior wall's bone cortex of the frontal sinus in the New Zealand rabbit model. In this study, as surgical objects 4 New Zealand rabbits were used. First, in each animal, the CMOF was formed from the anterior wall's bone cortex of the frontal sinus. After obtaining the CMOF, the remainder of the cortical bone of the anterior wall was removed by a drill. This procedure provided an anteriorly opened frontal sinus ca

The Pott puffy tumor revisited: neurosurgical implications of this unforgotten entity. Case report and review of the literature.

The Pott puffy tumor is a subperiosteal abscess of the frontal bone that appears as a localized swelling of the overlying region of the forehead associated with frontal osteomyelitis. The authors report the case of an 11-year-old boy who presented with a 6-week history of frontal headaches and a recent sudden-onset, progressively enlarging swelling of his midline forehead associated with immediate relief of headaches. A computed tomography (CT) study revealed 1) a subperiosteal abscess with intracranial extension through the perforated posterior table of the frontal sinus and 2) a large epidur

Light and transmission electron microscopic studies following frontal sinus obliteration with ionomer cement in cats.

Osteoplastic frontal sinus surgery in combination with sinus obliteration can be performed for various indications, including chronic sinusitis, frontal sinus trauma and removal of osteomas. In an experimental study using cats, the mucous lining of the frontal sinus was removed, the nasofrontal duct sealed with semifluid ionomer cement and the cavity filled with Ionogran, a solid and porous bone substitute based on ionomer cement. Histological investigations up to 1 year after surgery showed increasing sinus obliteration by regenerating bone, starting from the sinus wall, and formation of conn

[Long-term experience with the use of a device for trepanopuncture of original design].

The paper summarizes the experience with the treatment of more than 2,500 patients by trepanopuncture of frontal sinuses using an original device designed by the author. The instrument allows all intraoperative complications to be avoided, ensures maximally injury-sparing one-step intervention, reduces its duration, and enhances safety due to the use of a short-tube cannula and a cap of large diameter (greater than the tube diameter) limiting the depth of trepanation. The device is fixed on the bony wall of the frontal sinus with side cutting edges of the cannula and bone chip collector to pre

Rapid re-synostosis following suturectomy in pediatric mice is age and location dependent.

Craniosynostosis is the premature fusion of the cranial sutures early in development. If left untreated, craniosynostosis can lead to complications resulting from cranial deformities or increased intracranial pressure. The standard treatment involves calvarial reconstruction, which in many cases undergoes rapid re-synostosis. This requires additional surgical intervention that is associated with a high incidence of life threatening complications. To better understand this rapid healing, a pediatric mouse model of re-synostosis was developed and characterized. Defects (1.5mm by 2.5mm) over the

Finite element analysis of ramming in Ovis canadensis.

The energy produced during the ramming of bighorn sheep (Ovis canadensis) would be expected to result in undesirable stresses in their frontal skull, which in turn would cause brain injury; yet, this animal seems to suffer no ill effects. In general, horn is made of an α-keratin sheath covering a bone. Despite volumes of data on the ramming behavior of Ovis canadensis, the extent to which structural components of horn and horn-associated structure or tissue absorb the impact energy generated by the ramming event is still unknown. This study investigates the hypothesis that there is a mechanica

Repair of the immature craniofacial skeleton with a calcium phosphate cement: quantitative assessment of craniofacial growth.

The potential for growth restriction has limited the use of alloplastic materials for reconstruction of the growing craniofacial skeleton. A calcium phosphate cement that has been introduced recently for craniofacial reconstruction crystallizes in situ into a substance that resembles more closely the mineral phase of bone, thereby offering the potential for enhanced bioresorption and osteoconductivity. The purpose of these experiments was to assess quantitatively craniofacial growth after reconstruction of frontal craniectomy defects in skeletally immature animals with this calcium phosphate b

Nasal Bone Fractures: Analysis of 1193 Cases with an Emphasis on Coincident Adjacent Fractures.

Importance: The nasal bone is one of the most commonly fractured bones of the midface. However, the frequency of coincident fractures of adjacent bones such as the frontal process of the maxillary bone, nasal septum, and medial or inferior orbital walls has not been fully evaluated. Objective: The purpose of this study was to investigate the incidence of fractures of adjacent structures in the setting of a nasal bone fracture. Second, we propose a new classification system of nasal bone fractures with involvement of adjacent bony structures. Design, Setting, and Participants: One thousand, one

Cranial bone microarchitecture in a mouse model for syndromic craniosynostosis.

Crouzon syndrome is a congenital craniofacial disorder caused by mutations in the Fibroblast Growth Factor Receptor 2 (FGFR2). It is characterized by the premature fusion of cranial sutures, leading to a brachycephalic head shape, and midfacial hypoplasia. The aim of this study was to investigate the effect of the FGFR2 mutation on the microarchitecture of cranial bones at different stages of postnatal skull development, using the FGFR2C342Y mouse model. Apart from craniosynostosis, this model shows cranial bone abnormalities. High-resolution synchrotron microtomography images of the frontal a

Multicentric Calvarial Hemangioma with Dural Invasion: A Case Report.

Calvarial hemangiomas are rare benign vascular tumors originating from the skull bones, most commonly observed in the frontal and parietal bones of the cranium. Although most cases are asymptomatic, they can present with headache, cosmetic deformity, or, rarely, neurological deficit. The lesions are usually solitary; multicentric involvement and dural invasion are exceedingly rare phenomena. Here, we report a 22-year-old male patient who presented with frontal headache and a palpable scalp swelling. Imaging revealed two separate expansile lytic lesions in the right and left frontal bones with

[A rare case of cranial metastasis from hepatocellular carcinoma].

This is the first reported case of magnetic resonance imaging (MRI) findings of cranial metastasis from hepatocellular carcinoma. A 53-year-old male was admitted to our hospital on August 23, 1994, complaining of severe headache and a subcutaneous mass on the forehead. He was diagnosed as hepatocellular carcinoma in February, 1994, at another hospital. Because of multiple intrahepatic metastasis, he was inoperable and received transarterial embolization (TAE) on February 15, 1994. He had noticed the subcutaneous mass two months prior to admission, and its recent rapid growth, and morning heada

Fibrous dysplasia of the frontal sinus: an uncommon cause of frontal lobe abscess.

Fibrous dysplasia of the cranial air sinuses is rarely reported in the literature. This is the first report of frontal lobe abscess (FLA) associated with fibrous dysplasia of the frontal sinus (FDFS). A 29-year-old female presented with seizures and acute confusion. Cranial computed tomography (CT) revealed fibrous dysplasia of the left frontal sinus and associated FLA. She was referred to the neurosurgical service and the abscess and dysplastic tissue were removed. Histological examination confirmed fibrous dysplasia. We review the radiological appearance of FDFS with FLA. Clinicians should b

Chronic frontal sinusitis and osteolysis in desert bighorn sheep.

Chronic frontal sinusitis and osteolysis in a herd of desert bighorn sheep (Ovis canadensis nelsoni) in Zion National Park, Utah, was attributed to bacterial infection secondary to aberrant nasal bot migration. Analysis of skulls and live adult sheep (equal to 1 year old) showed that 9 to 22 animals were affected. One ram and 1 ewe were treated by trephination and irrigation. Treatment was successful for the ewe, but not for the ram. Radiography, thermography, and blood profiles were used in an attempt to find a way to diagnose sinusitis in its early stages; at this point, only thermography sh

Is it more reasonable to categorize frontal cells on the basis of their location rather than on their type?

Frontal cells appear in two locations-in the frontal recess and in the frontal sinus. The aim of this study was to analyze the anatomic and clinical differences between the frontal cells at each location. The author reviewed 487 left and right sides of coronal computed tomography (CT) scans of the sinuses obtained from 300 consecutively presenting patients (600 sides) who were being evaluated for chronic sinusitis. For the purposes of this study, the frontal cells were classified according to location; group A cells were located in the area of the frontal recess (Bent and Kuhn cell types I and

Long-term results of fronto-orbital advancement and remodeling using distraction osteogenesis in craniosynostosis patients.

In the initial report describing the fronto-orbital distraction technique, bone segments were advanced preserving their attachments with the dura. This approach does not allow for the remodeling of the frontal segment. However, in the technique described herein, the fronto-orbital segment is separated from dura, remodeled, and advanced as a bone graft using distractors. Twenty-seven craniosynostosis patients that underwent fronto-orbital advancement and remodeling using distraction osteogenesis were retrospectively reviewed. The mean age at the time of surgery was 19.03 ± 9.19 months; the mean

Microsurgery "Under the Eaves" Using ORBEYE: A Case of Dural Arteriovenous Fistula of the Anterior Cranial Fossa.

BACKGROUND: One of the merits of exoscopes, including ORBEYE, is that they are superior to a microscope in terms of ergonomic features. We report a case of dural arteriovenous fistula (dAVF) that was cured by direct surgery using the ergonomic advantages of ORBEYE. CASE DESCRIPTION: A 78-year-old man was found to have dAVF of the anterior cranial fossa incidentally. We performed direct surgery via bifrontal craniotomy. Because the frontal sinus was large, we reserved the frontal bone-like eaves in order not to open the frontal sinus. The vertex of his head was sufficiently down to shift the f

Frontal sinus obliteration with the pericranial flap.

BACKGROUND: Frontal sinus obliteration is often accomplished by autologous grafts such as fat, muscle, or bone. These avascular grafts carry an increased risk of resorption and infection as well as donor site morbidity. Vascular regional flaps may be used to obliterate small sinuses with less morbidity. OBJECTIVES: To review our experience with the use of the pericranial flap for obliteration of the frontal sinus. METHODS: The records of 10 patients who underwent obliteration of the frontal sinus with the pericranial flap were reviewed. Demographics, indications for frontal sinus obliteratio
