Impact of knee marker misplacement on gait kinematics of children with cerebral palsy using the Conventional Gait Model-A sensitivity study.

Clinical gait analysis is widely used in clinical routine to assess the function of patients with motor disorders. The proper assessment of the patient's function relies greatly on the repeatability between the measurements. Marker misplacement has been reported as the largest source of variability between measurements and its impact on kinematics is not fully understood. Thus, the purpose of this study was: 1) to evaluate the impact of the misplacement of the lateral femoral epicondyle marker on lower limb kinematics, and 2) evaluate if such impact can be predicted. The kinematic data of 10 c

Effect of foot and ankle immobilization on able-bodied gait as a model to increase understanding about bilateral transtibial amputee gait.

BACKGROUND: The anatomical foot-ankle complex facilitates advancement of the stance limb through foot rockers and late-stance power generation during walking, but this mechanism is altered for persons with bilateral transtibial amputation when using passive prostheses. OBJECTIVES: To study the effects of bilateral foot and ankle immobilization on able-bodied gait to serve as a model for understanding gait of persons with bilateral transtibial amputation and associated compensatory mechanisms. STUDY DESIGN: Comparative analysis. METHODS: Nine able-bodied persons walked at self-selected slow,

The influence of spasticity in the lower limb muscles on gait pattern in children with sacral to mid-lumbar myelomeningocele: a gait analysis study.

Gait analysis and recording of standing position were performed in 38 ambulatory children with myelomeningocele. Thirty-four were independent ambulators and four required a walking aid. All subjects were assigned one of four muscle function groups based on muscle strength. They were also divided into subgroups based on the distinction between flaccid and spastic paresis in the lower limb joints. A comparison was made between the gait pattern of the children with spasticity and that of the children with flaccid paresis in each muscle function group. Spasticity in only the ankle joint muscles in

Comparing gait metrics from in-lab gait analyses to free-living assessment from wearable sensors in end-stage osteoarthritis patients.

BACKGROUND: Gait analysis provides objective metrics to evaluate mobility in populations such as individuals with knee osteoarthritis. However, in-lab assessments may not reflect real-world gait. Wearable inertial sensors offer a promising alternative, but few studies have directly compared concurrent gait measures from motion capture and wearable sensor-based gait analysis with free-living data collections. METHODS: This study collected gait data preoperatively from 45 older adults with end-stage knee osteoarthritis using a concurrent collection of in-lab markerless motion capture and wearab

Comparison of Continuous and Gait-synchronized Transcutaneous Electrical Nerve Stimulation During Gait Training in Chronic Stroke: A Pilot Randomized Trial.

BACKGROUND: Post-stroke gait dysfunction may involve impaired integration of motor commands, somatosensory feedback, and phase-dependent peripheral sensory input during locomotion. Transcutaneous electrical nerve stimulation (TENS) combined with gait training may provide additional sensory input, but the relevance of different temporal stimulation profiles remains unclear. OBJECTIVE: To evaluate the feasibility, safety, and preliminary clinical signals of continuous and gait-synchronized TENS during gait training in individuals with chronic stroke. METHODS: Twenty-four participants with chro

High-intensity interval training with robot-assisted gait therapy vs. treadmill gait therapy in chronic stroke: a randomized controlled trial.

OBJECTIVE: Stroke is a leading cause of long-term disability, significantly impacting patients' mobility and quality of life. Robot-assisted gait therapy (RAGT) and high-intensity interval training (HIIT) have individually shown potential in improving gait function in chronic stroke patients. This study investigated the potential effectiveness of combining high-intensity interval training (HIIT) with robot-assisted gait therapy (RAGT) to enhance gait, balance, and lower limb function in patients with chronic stroke. METHODS: A randomized controlled trial was conducted with 48 patients with ch

Anodal transcranial patterned stimulation of the motor cortex during gait can induce activity-dependent corticospinal plasticity to alter human gait.

The corticospinal system and local spinal circuits control human bipedal locomotion. The primary motor cortex is phase-dependently activated during gait; this cortical input is critical for foot flexor activity during the swing phase. We investigated whether gait-combined rhythmic brain stimulation can induce neuroplasticity in the foot area of the motor cortex and alter gait parameters. Twenty-one healthy subjects participated in the single-blinded, cross-over study. Each subject received anodal transcranial patterned direct current stimulation over the foot area of the right motor cortex dur

Hip mechanics underlie lower extremity power training-induced increase in old adults' fast gait velocity: The Potsdam Gait Study (POGS).

BACKGROUND: Aging is associated with slowed gait and old compared with young adults generally walk with greater positive hip work (H1) and reduced positive ankle work (A2). The role of exercise interventions on old adults' gait mechanics that underlie training-induced improvements in gait velocity is unclear. We examined the effects of lower extremity power training and detraining on old adults' gait kinetics. METHODS: As part of the Potsdam Gait Study (POGS), healthy old adults completed a no-intervention control period (69.1±4.4yrs, n=14) or a power training program followed by detraining (

Spatiotemporal gait compensations following medial collateral ligament and medial meniscus injury in the rat: correlating gait patterns to joint damage.

INTRODUCTION: After transection of the medial collateral ligament and medial meniscus (MCLT + MMT) in the rat, focal cartilage lesions develop over 4-6 weeks; however, sham surgery (MCLT alone) does not result in cartilage damage over a similar period. Thus, comparison of MCLT + MMT with the MCLT sham group offers an opportunity to investigate behavioral modifications related to focal cartilage and meniscus damage in the rat. METHODS: MCLT or MCLT + MMT surgery was performed in the right knees of male Lewis rats, with spatiotemporal gait patterns and hind limb sensitivity assessed at 1, 2, 4,

Radar Network for Gait Monitoring: Technology and Validation.

In recent years, radar-based devices have emerged as an alternative approach for gait monitoring. However, the radar configuration and the algorithms used to extract the gait parameters often differ between contributions, lacking a systematic evaluation of the most appropriate setup. Additionally, radar-based studies often exclude motorically impaired subjects, leaving it unclear whether the existing algorithms are applicable to such populations. In this paper, a radar network is developed and validated by monitoring the gait of five healthy individuals and three patients with Parkinson's dise

Towards the Prevention of Freezing of Gait in Parkinson's Disease.

Despite optimal management, freezing of gait (FOG) remains a disabling complication of Parkinson's disease (PD), contributing substantially to falls, loss of independence and reduced quality of life. This highlights the pressing need for a shift towards preventive approaches, aiming to postpone or even prevent its development. In this narrative review, we examine potential avenues for the secondary prevention of FOG. We discuss how FOG might be prevented specifically, by targeting modifiable risk factors that seem associated with the development of FOG. Examples of potentially effective strate

Walking as a Window to the Brain: Redefining Gait in Neurology.

Walking is not merely locomotion but a window into the nervous system, integrating cortical, subcortical, cerebellar, spinal, and peripheral networks into a unified motor behavior. Across neurological diseases-including Parkinson's disease, atypical parkinsonism, cerebellar ataxias, stroke, multiple sclerosis, neuropathies, neuromuscular disorders, and functional gait syndromes-gait disturbances are among the most disabling clinical features, contributing to falls, loss of independence, institutionalization, and premature mortality. Traditional bedside observation remains indispensable, but it

An Evaluation of Orthotics on In-Toeing or Out-Toeing Gait.

Background and Objectives: In-toeing and out-toeing gait are rotational deformities commonly observed in children with neuromuscular conditions. These gait abnormalities often result from internal tibial torsion, increased femoral anteversion, and metatarsus adductus. This study was conducted to create a comprehensive evaluation of the effectiveness of lower extremity orthotics as a non-operative treatment option, given their regular use in clinical settings. The aim of this literature review was to understand the efficacy of various orthotic devices in correcting rotational deformities in the
