The frequency and associated factors of typhoid carriage in patients undergoing cholecystectomy for gallbladder disease in Pakistan: A cross-sectional study.

BACKGROUND: Enteric fever is caused by Salmonella enterica serovars Typhi (S. Typhi) and Paratyphi A, B, and C. It continues to be a significant cause of morbidity and mortality worldwide. In highly endemic areas, children are disproportionately affected, and antimicrobial resistance reduces therapeutic options. It is estimated that 2-5% of enteric fever patients develop chronic asymptomatic infection. These carriers may act as reservoirs of infection; therefore, the prospective identification and treatment of carriers are critical for long-term disease control. We aimed to find the frequency

Traveling gallstones: review of MR imaging and surgical pathology features of gallstone disease and its complications in the gallbladder and beyond.

Gallstone-related disease comprises a spectrum of conditions resulting from biliary stone formation, leading to obstruction and inflammatory complications. These can significantly impact patient quality of life and carry high morbidity if not accurately detected. Appropriate imaging is essential for evaluating the extent of gallstone disease and assuring appropriate clinical management. Magnetic Resonance Imaging (MRI) techniques (including Magnetic Resonance Cholangiopancreatography (MRCP) are increasingly used for diagnosis of gallstone disease and its complications and provide high contrast

Clinical significance of preoperative antibiotic use in mild to moderate acute inflammatory gallbladder disease: A randomized controlled trial.

BACKGROUND: Many patients receive empirical antibiotics for the prevention of postoperative infectious complications following cholecystectomy due to acute cholecystitis (AC). The purpose of this study was to investigate the clinical significance of preoperative antibiotics in mild to moderate AC patients undergoing emergency laparoscopic cholecystectomy. METHODS: This was a double-blind, placebo-controlled, randomized study. Patients with AC eligible for emergency laparoscopic cholecystectomy were randomly assigned to an antibiotic or a placebo group. Clinical outcomes including infectious c

Acute abdominal disease associated with gallbladder torsion recovered after cholecystectomy: a rare case report and literature review.

BACKGROUND: Gallbladder torsion is very rare and easily misdiagnosed as biliary disease. It is defined as the rotation of the gallbladder along the axis of the cystic pedicle on the mesentery. As gallbladder rotation involves the gallbladder artery, the blood supply is blocked, resulting in gallbladder ischemia and eventual necrosis. If misdiagnosis occurs and treatment is delayed, gallbladder torsion can develop into a lethal disease. The typical imaging features of gallbladder torsion in this case are a good learning resource for our young physicians, as well as providing a rare, unusual and

Gallbladder bleeding-related severe gastrointestinal bleeding and shock in a case with end-stage renal disease: A case report.

Gallbladder (GB) bleeding is very rare and it is caused by cystic artery aneurysm and rupture, or GB wall rupture. For GB rupture, the typical findings are positive Murphy's sign and jaundice. GB bleeding mostly presented as hemobilia. This is the first case presented with severe GI bleeding because of GB rupture-related GB bleeding. After comparing computed tomography, one gallstone spillage was noticed. In addition to gallstones, uremic coagulopathy also worsens the bleeding condition. This is also the first case that patients with GB spillage-related rupture and bleeding were successfully t

Fourteen-years of disease-free survival in a patient with advanced gallbladder carcinoma after radical resection: a case report.

We describe a case of advanced gallbladder carcinoma (GBC) with extensive local invasion that has had 14-years of disease-free survival after complete resection. A 53-year-old woman was referred to our hospital with right hypochondriac pain and constipation. A preoperative diagnosis of advanced GBC with invasion into the liver bed, the abdominal wall, the duodenum, the common bile duct and the hepatic flexure of the colon were made. It was performed an extensive en bloc resection of the GBC, which included a cholecystectomy, resection of the associated liver bed, pancreatoduodenectomy, right h

Endoscopic transpapillary catheterization of the gallbladder followed by external shock wave lithotripsy and solvent infusion for the treatment of gallstone disease.

Herein we describe the techniques and early results of attempted endoscopic transpapillary catheterization of the gallbladder (ETCG) in combination with external shock wave lithotripsy (ESWL) and solvent infusion in the treatment of 23 consecutive, symptomatic patients with gallstones. Despite anticipated difficulty in advancing wires and catheters through the cystic duct, impeded by tortuosity of its lumen, ETCG was successfully accomplished in 20 of the 23 patients (86.9%). Through an indwelling catheter, obstructed gallbladders were drained in 4 patients and combined treatment with ESWL and

Incretin-based drugs and the risk of gallbladder or biliary tract diseases among patients with type 2 diabetes across categories of body mass index: a nationwide cohort study.

BACKGROUND: Despite emerging evidence of gallbladder or biliary tract diseases (GBD) risk regarding incretin-based drugs, population-specific safety profile considering obesity is lacking. We aimed to assess whether stratification by body mass index (BMI) modifies the measures of association between incretin-based drugs and the risk of GBD. METHODS: We conducted an active-comparator, new-user cohort study using a nationwide claims data (2013-2022) of Korea. We included type 2 diabetes (T2D) patients stratified by Asian BMI categories: Normal, 18.5 to <23 kg/m2; Overweight, 23 to <25 kg/m2; Ob

Is cholecystectomy a real risk factor for Metabolic dysfunction-associated steatotic liver disease (MASLD)? A longitudinal cohort study from a population with a high burden of gallbladder diseases.

BACKGROUND: Cholecystectomy due to gallstones is one of the most frequent surgeries worldwide. Observational studies suggest that cholecystectomy may be a risk factor for metabolic dysfunction associated with steatotic liver disease (MASLD). However, gallstone disease and MASLD share common risk factors, which could make cholecystectomy a confounder. AIM: To assess the relationship among cholecystectomy, gallstones, and MASLD in a longitudinal cohort study from a population at high risk of gallbladder diseases. METHODS: A longitudinal retrospective cohort study compared consecutive patients …

Laparoscopic cholecystectomy in a patient with sickle cell disease and acquired cranial gallbladder malposition: technical adaptations to achieve safe dissection.

We report the case of a 38-year-old male with sickle cell disease (SCD) who underwent elective laparoscopic cholecystectomy for recurrent biliary colic. Intraoperative exploration revealed a rare acquired cranial gallbladder malposition, with the gallbladder lying high in the subdiaphragmatic space and displaced posteriorly, in the setting of marked hepatomegaly and chronic inflammation. This abnormal positioning resulted in significant exposure limitations and difficulty achieving the classical critical view of safety. Conventional fundal traction was ineffective. Adequate exposure was ultima

Chronological change of gallbladder fossa nodularity in the liver as observed in patients with alcoholic liver disease: cross-sectional and longitudinal observation.

PURPOSE: To confirm the concept that gallbladder fossa nodularity (GBFN) as observe in patients with alcoholic liver disease (ALD) develops in a biphasic fashion as the cirrhotic process progresses, by clarifying the sequential or chronological change of GBFN both in cross-sectional and longitudinal analyses. MATERIALS AND METHODS: We retrospectively recruited 52 ALD patients who had both quadruple phase CT and gadoxetate-enhanced MRI (EOB-MRI) within 6 months between 2013 and 2020, and GBFN were morphologically classified into grades 0-3, as previously described. As a cross-sectional study,

Robotic-assisted versus conventional/single-incision laparoscopic cholecystectomy for benign gallbladder disease: A systematic review and meta-analysis.

BACKGROUND: Robotic surgery has shown great advantages in many complicated endoscopic operations compared with laparoscopic surgery. However robotic-assisted cholecystectomy (RAC) is still a controversial issue compared with conventional/single-incision laparoscopic cholecystectomy (CLC/SILC). The aim of this study was to compare the safety and efficacy of RAC with CLC/SILC for benign gallbladder disease. METHODS: Embase, Medline, Pubmed, and Cochrane library databases were searched to obtain comparative studies evaluating the safety and efficacy between RAC and CLC/SILC. Only randomized tria

The oncologic burden of residual disease in incidental gallbladder cancer: An elastic net regression model to profile high-risk features.

INTRODUCTION: Incidental Gallbladder Cancer (IGBC) following cholecystectomy constitutes a significant portion of gallbladder cancer diagnoses. Re-exploration is advocated to optimize disease clearance and enhance survival rates. The consistent association of residual disease (RD) with inferior oncologic outcomes prompts a critical examination of re-resection's role as a modifying factor in the natural history of IGBC. METHODS: All patients diagnosed with gallbladder cancer between 2012 and 2022 were included. An elastic net regularized regression model was employed to profile high-risk predi

Biliary anatomic variant and recurrent acute cholecystitis, cholelithiasis in gallbladder remnant in patient with autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited renal disorder. ADPKD is not only associated with progression of renal disease, but also several hepatobiliary manifestations. This report is of a 49-year-old female with recurrent cholelithiasis and cholecystitis following subtotal cholecystectomy in the context of aberrant biliary anatomy and ADPKD. There were significant adhesions obscuring the cystic duct, necessitating the second cholecystectomy be performed open. The right posterior hepatic duct was adhered to the gallbladder wall and was perforated while a
