Treatment of acute chlorine gas inhalation with nebulized sodium bicarbonate.

Three male patients, 19 to 20 years old, were exposed to chlorine gas secondary to a leak in the chlorination system of an indoor pool. All of the patients were symptomatic with cough, chest pain, and shortness of breath. Physical examinations, arterial blood gases, and chest radiographs were normal. All patients were given a nebulized solution of 3.75% sodium bicarbonate which resulted in prompt relief of their symptoms. None of the patients suffered from prolonged symptomatology. This therapy appears to be useful in treating chlorine gas inhalation; however, it cannot be routinely recommende

Toxicology of fires: an emerging clinical concern.

As a wider variety of synthetic materials is used in buildings, the potential for poisoning from inhalation of products of combustion is increasing greatly. Research during the past few years has shown that the burning of plastics (insulation, furniture, carpeting, electric wiring covering, decorative items) results in the formation of large amounts of highly toxic chemicals. Clinicians treating victims of fires should be aware of the toxicological ramifications of combustion, including delayed pathophysiological sequellae. A case report and a review of some current hypotheses of fire-induced

Successful rescue of a patient with extensive lethal-dose aluminum phosphide poisoning complicated by multiple organ dysfunction syndrome.

Aluminum phosphide poisoning is a highly lethal toxicological emergency associated with an extremely high mortality rate, and no specific antidote is currently available. Aluminum phosphide rapidly releases phosphine gas after coming into contact with gastric acid, which strongly inhibits mitochondrial cytochrome c oxidase, leading to severe cellular metabolic disorder, lactic acidosis, refractory shock, and progressive multiple organ dysfunction syndrome. In severe cases, the mortality rate can approach 100%. We report the case of a man in his early 40s who intentionally ingested 25.2 g of al

Forensic metabolomics: Tracing cyanide-induced metabolic changes in fatalities.

Accurate detection of cyanide exposure is crucial, particularly in forensic science. However, cyanide's high volatility and potential biochemical conversions in biological samples pose challenges for direct detection, complicating the determination of cause of death. Identifying alternative cyanide metabolites as markers may mitigate false negatives and positives, extending the detection window in poisoning cases. This study aimed to evaluate metabolic changes induced by cyanide exposure in forensic cases using a multi-platform approach, including metabolomics and lipidomics analyses via liqui

Formation of highly toxic hydrogen cyanide upon ruby laser irradiation of the tattoo pigment phthalocyanine blue.

Since laser treatment of tattoos is the favored method for the removing of no longer wanted permanent skin paintings, analytical, biokinetics and toxicological data on the fragmentation pattern of commonly used pigments are urgently required for health safety reasons. Applying dynamic headspace-gas chromatography with mass spectrometric detection (DHS-GC/MS) and comprehensive two-dimensional gas chromatography coupled to time-of-flight mass spectrometry (GCxGC-ToF-MS), we identified 1,2-benzene dicarbonitrile, benzonitrile, benzene, and the poisonous gas hydrogen cyanide (HCN) as main fragment

Case Report: Methanol poisoning mimicking acute coronary syndrome-a fatal case of massive intracranial hemorrhage on dual antiplatelet therapy.

This manuscript describes a fatal case of a 45-year-old man who presented with severe chest pain, hypertension (198/115 mmHg), and electrocardiogram changes mimicking acute coronary syndrome (ACS) 30 h after ingesting adulterated alcohol. Uniquely, chest pain was the sole initial manifestation, without visual disturbances, gastrointestinal symptoms, or any other classic methanol features, creating a clinically convincing ACS mimic. Despite negative coronary CT angiography, dual antiplatelet loading therapy (aspirin 300 mg and clopidogrel 300 mg) was administered based on clinical suspicion of

Nrf2-mediated liver protection by 18β-glycyrrhetinic acid against pyrrolizidine alkaloid-induced toxicity through PI3K/Akt/GSK3β pathway.

BACKGROUND: Misusage of pyrrolizidine alkaloid (PA)-containing plants or unaware intake of PA-contaminated foodstuffs causes thousands of PA poisoning cases in humans. PA intoxication is accompanied by oxidative stress and subsequent extensive hepatocellular damage. Our previous study has demonstrated that 18β-glycyrrhetinic acid (GA), a bioactive constituent of liquorice, prevented PA-induced hepatotoxicity in rats, however the underlying mechanisms remain unclear. OBJECTIVE: This study aims to explore the mechanisms underlying the hepato-protective effect of GA in combating retrorsine (RTS,

Investigation of serum concentration and the type of opioid used in patients with opium poisoning.

Opioid poisoning constitutes a major public health issue in Iran and worldwide, with increasing rates attributed to both traditional opiates and synthetic derivatives such as methadone. Reliable assessment of serum opioid concentrations is essential for clinical management and prognosis. Quantitative measurement of serum opioid concentrations determines local epidemiological patterns and identifies risk factors that guide future prevention and treatment strategies. This descriptive cross-sectional study was conducted at Imam Khomeini Hospital in Kermanshah, Iran, from March to June 2023. Fifty

Epitaxial Oxide Interfaces Create Poison-Resistant CuO Sites for Environmental Catalysis.

Real exhaust streams rarely contain a single pollutant: NOx coexists with volatile organic compounds (VOCs) in flue gas from petrochemical production, chemical manufacturing, and waste incineration, yet catalysts that couple NH3-SCR with VOC oxidation typically suffer competitive adsorption, sulfur poisoning, and HCN byproduct formation. Here we engineer an epitaxially stabilized CuO overlayer on Ti1-xInxO2 that breaks the activity-selectivity-stability constraint by creating electron-poor, high-symmetry Cu-O sites and activating lattice-oxygen redox at the oxide-oxide interface. Interfacial s

Methemoglobinemia Following Indoxacarb Ingestion: A Unique Toxicological Presentation.

Indoxacarb, an oxadiazine insecticide, is known for its selective lethality by blocking neuronal voltage-dependent sodium channels. While primarily developed to target insect populations resistant to other pesticides, its toxicity in humans remains poorly understood. We present a case of methemoglobinemia resulting from indoxacarb ingestion, a rare manifestation of its toxic effects. A 38-year-old farmer attempted suicide by ingesting the insecticide, leading to cyanosis, hypoxemia, and characteristic arterial blood gas findings indicative of methemoglobinemia. Prompt diagnosis was challenging

Visual loss caused by acute cyanide poisoning: a case report.

CONTEXT: Cyanide is considered one of the most lethal poisons, reports on visual changes after acute cyanide poisoning are rare due to a low rate of survival of those having committed suicides. CASE DETAILS: A 30-year-old man developed visual loss in both eyes after inhaling sodium cyanide gas, but nothing abnormal was found in slit lamp microscope and indirect ophthalmoscope examinations 5 months later. In pattern visual evoked potential, the amplitude of P100 wave was apparently decreased. Humphrey perimetry examination and AccuMap multifocal visual evoked potentials objective perimetry sho

Beyond Purification: Highly Efficient and Selective Conversion of NO into Ammonia by Coupling Continuous Absorption and Photoreduction under Ambient Conditions.

Although the selective catalytic reduction technology has been confirmed to be effective for nitrogen oxide (NOx) removal, green and sustainable NOx re-utilization under ambient conditions is still a great challenge. Herein, we develop an on-site system by coupling the continuous chemical absorption and photocatalytic reduction of NO in simulated flue gas (CNO = 500 ppm, GHSV = 18,000 h-1), which accomplishes an exceptional NO conversion into value-added ammonia with competitive conversion efficiency (89.05 ± 0.71%), ammonia production selectivity (95.58 ± 0.95%), and ammonia recovery efficien

Catalytic destruction of pentachlorobenzene in simulated flue gas by a V2O5-WO3/TiO2 catalyst.

Pentachlorobenzene (PeCB) in simulated flue gas was destructed by a commercial V(2)O(5)-WO(3)/TiO(2) catalyst in this study. The effects of reaction temperature, oxygen concentration, space velocity and some co-existing pollutants on PeCB conversion were investigated. Furthermore, a possible mechanism for the oxidation of PeCB over the vanadium oxide on the catalysts was proposed. Results show that the increase of gas hourly space velocity (GHSV) and the decrease of operating temperature both resulted in the decrease of PeCB removal over the catalyst, while the effect of the oxygen content in

Intoxication by cyanide in fires: a study in monkeys using polyacrylonitrile.

It is suspected that hydrogen cyanide (HCN) may be an important factor in incapacitating fire victims, but the effects of sublethal exposures are not well characterized. Also, the incapacitating effects of fire atmospheres result from exposure to a mixture of toxic products so that the contribution from each component is difficult to determine. The mechanisms of incapacitation in monkeys exposed to the pyrolysis products of polyacrylonitrile (PAN) were compared to those resulting from low level HCN gas exposures. The physiological effects of the PAN atmospheres were almost identical to those o

[Fatal calcium polysulfide overdose presenting corrosive chemical injury of esophagus and sulf-hemoglobinemia].

A-79-year-old woman ingested a cup of unknown violet agricultural solution intentionally. She was vomiting and smelt of sulfur. Arterial blood gas showed metabolic acidosis and marked cyanosis regardless of relatively high PaO2, caused by sulfhemoglobinemia. A nasogastric tube could not be inserted because of marked stenosis caused by endoscopically proven severe corrosive chemical injury (burn) of esophagus. Considering the smell and the clinical presentation, we concluded that the causative agent was calcium polysulfide or lime-sulphur solution, a common agricultural product used as a fungic
