There is a high incidence of gastric carcinoma in the coloured population of the Western Cape. Diagnostic tests other than barium meal examination or gastroscopy were investigated. The short-term results of a new surgical procedure for prevention of duodenogastric reflux and rapid gastric emptying after partial gastric resection are reported. A jejunal segment provided with an intussusception valve was interposed between the gastric remnant and the duodenum in six patients with severe postgastrectomy symptoms. Cell proliferation of human gastric mucosa was studied in 3 healthy subjects. Biopsies were obtained from the antral and fundic part of the stomach through a fiber gastroscope. INTRODUCTION: We are presenting our experiences with combined laparoscopic and endoscopic approaches applied to the minimally invasive intragastric resection of the gastric tumors.
PATIENTS AND METHOD: We performed the above-mentioned combined intragastric resection in 7 patients. The intragastric resection is only used with posterior gastric wall tumors and tumors located near the cardia and the pylorus. For this approach, only 1 trocar is placed into the stomach to intragastrically introduce the endostapler. We used 2 trocars (5 mm) into the abdominal cavity for laparoscopy.

BACKGROUND: To investigate whether endosonography can help in the detection and treatment of Dieulafoy's disease, we examined eight patients with suspicion of Dieulafoy's disease.
METHODS: Between December 1992 and April 1995, eight patients were referred because of suspicion of Dieulafoy's disease. Seven presented with upper gastrointestinal bleeding and one with a tiny ulcer. In all eight patients the stomach was examined with an Olympus GF-UM20 echoendoscope. The stomach was filled with 200 to 400 ml of water after which the body, fundus, and cardia were carefully visualized.

We performed a total gastrectomy in a 16-year-old asymptomatic CDH1 gene mutation carrier in whom two prior gastroscopies with biopsies were normal. Pathological sections of gastrectomized specimens of 74 patients with benign gastric ulcer and 79 with gastric cancer were reviewed.
