Moving Beyond the Checklist: A Three-Tier Model for Integrating Geriatrics Expertise into the CMS Age-Friendly Era.

As the Age-Friendly Health Systems (AFHS) movement enters a new phase of policy integration, hospitals face a pivotal moment. The U.S. Centers for Medicare and Medicaid Services (CMS) Age Friendly Hospital Measure has created regulatory conditions necessary for hospital systems to institute high-quality, personalized care for older adults. As the CMS is expected to transition from pay-for-reporting to pay-for-performance for age-friendly care in the coming years, health systems must urgently move beyond reporting compliance toward true care redesign. The authors highlight three interrelated st

From Pediatrics to Geriatrics: Reviewing Family-Centered Care Interventions and Their Influence on Intensive Care Unit Patient Outcomes.

Family-centered care (FCC) is a hallmark of high-quality care in intensive care units (ICUs), yet consistent implementation remains challenging. This umbrella review evaluated recent literature on FCC interventions and their influence on patient outcomes across ICU settings. FCC interventions improved infant feeding, weight gain, and reduced length of stay (LOS); reduced complications in children; and decreased delirium and LOS in adults. However, evidence strength varied, with some studies reporting mixed or minimal effects. Variability in findings highlights the need for further research usi

Enhancing Advance Directive Completion Among Older Adults in the Geriatrics Clinic in Indiana, USA: A Quality Improvement Initiative.

Background/Objective: Death is an inevitable part of life, and ensuring high-quality end-of-life care is a critical concern. Planning for a "good death" aligns care with patients' preferences, and advance directive (AD) discussions should begin early between patients, physicians, and families. This project aimed to increase AD completion among older adults in the Indiana University Health Primary Care (IUHPC) Geriatrics Clinic, with the goal of meeting or exceeding the national average of 46%. Methods: An intervention was implemented from September to November 2024 and from January to June 202…

Practices, issues and possibilities at the interface between geriatrics and palliative care (InGaP): A case study exemplifying knotworking.

INTRODUCTION: With the recognition of the need for palliative care for people with non-malignant conditions, there is an increasing emphasis on interdisciplinary working between geriatric and palliative care teams. This interdisciplinary work has evolved organically; more needs to be known about current working practices. This is of policy and clinical interest as the older patient population continues to grow. METHODS: A case study based on qualitative interviews was undertaken of end-of-life care for older in-patients in a large London NHS Trust. 30 semi-structured qualitative interviews we

Effects of neuromuscular exercise and dance therapy on physical performance and kinesiophobia in GERIATRICS: A randomized controlled study.

BACKGROUND: Neuromuscular exercise is a comprehensive training approach encompassing functional movements, proprioceptive training, and perturbation exercises aimed at enhancing balance, muscle activation, and joint stability. Dance therapy, defined as the intentional and rhythmic movement of the body to music, serves as a physical and mental modality for promoting well-being. OBJECTIVE: This study aimed to compare the effects of a physiotherapy-based neuromuscular exercise program and dance therapy on physical performance and kinesiophobia in geriatrics. METHODS: Fifty-two participants were…

Hematological and biochemical characterization of aging farm male rat strains in the national center for geriatrics and gerontology.

Laboratory rats, like mice, are a type of animal commonly used in scientific investigations as well as in basic aging and geriatric research. The selection of a rat strain is an important first step in the planning and design of an experiment due to physiological, anatomical, and ethological variations in each strain, which may significantly modify the expected results. In the present study, we characterized age-related changes, from 3 months old (mo) to 24 mo, in three male rat strains commonly used in medical research: RccHan®️:WIST (RccHan:WIST), F344/NSlc (F344), and Slc:SD Rat (SD). The b

Clin-STAR corner: Recent practice-changing studies at the interface of pulmonary and critical care medicine and geriatrics.

Older adults suffering from chronic pulmonary diseases, such as chronic obstructive pulmonary disease and interstitial lung disease, and critical illnesses, such as sepsis and acute respiratory failure, are more vulnerable to adverse outcomes like disability and greater side effects from treatments. In this update, we discuss recent practice-changing clinical trials and observational studies in Pulmonary & Critical Care Medicine that have advanced our understanding of the diagnosis or management of older adults with chronic lung diseases or critical illnesses.

Robotic emergency surgery in geriatric care.

BACKGROUND: Older adults present unique challenges in emergency general surgery (EGS) due to increased comorbidities and frailty. While minimally invasive surgery (MIS), including laparoscopic (lap) and robotic techniques, reduces perioperative morbidity, the role of minimally invasive EGS in geriatric patients remains unclear. This study compares outcomes for geriatric EGS patients following robotic, lap, and open procedures. We hypothesize that MIS approaches would improve perioperative outcomes in geriatric patients. METHODS: We conducted a retrospective analysis of the ACS-NSQIP database

Engineering the geriatric heart to model HFpEF.

Heart failure with preserved ejection fraction (HFpEF) clinical trials suffer from high rates of attrition due to a biological mismatch between preclinical models and patients. While clinical HFpEF is fundamentally driven by advanced age, female sex, systemic inflammation, and progressive myocardial stiffness, preclinical screening still relies predominantly on young male rodents or foetal-like pluripotent stem cell derivatives. In this perspective, we propose a modular and testable geriatric engineering framework for reconstructing the myocardial features of this disease in vitro. The framewo

[Anabolic treatment in geriatric patients with osteoporosis].

Osteoporotic fractures are a major cause of morbidity, functional decline, and mortality in older adults. Despite effective treatment options, a substantial treatment gap persists in primary and secondary fracture prevention. Osteoanabolic agents stimulate bone formation and lead to a rapid improvement in bone mineral density and bone microarchitecture. Clinical trials demonstrate superiority over antiresorptive therapies in patients at very high or imminent risk of fracture; however, treatment in geriatric populations requires a differentiated balancing of the benefit-risk ratio with careful
