BACKGROUND: Although the oral manifestations of Crohn disease are well-established, there is little specific documentation of the gingival involvement.
CASE DESCRIPTION: The authors describe four patients with significant gingival involvement and identify clinical signs and symptoms of the disease involving the gingivae, along with other oral manifestations. Patients had persistent gingival lesions manifesting as pustular ulcerations, erythema, swelling and cobblestoning. The authors also discuss the differential diagnosis, treatment options and prognostic factors.

Drug-induced gingival hyperplasia is a serious concern both for the patient and the clinician. A 45 year-old Caucasian male patient with hypertension, who received amlodipine (10 mg/day, single dose orally) for two months, sought medical attention because of the new-onset gingival enlargement. BACKGROUND, AIMS: Orofacial granulomatosis (OFG) is a descriptive term used for granulomatous disorders of the face and oral cavity that may occur for a variety of reasons, some of which result in significant morbidity and mortality. Although rarely, a granular enlargements of the gingiva may be the first clinical manifestation of OFG, preceding other local or systemic manifestations.

Gingival hyperplasia or gingival overgrowth is a common occurrence in patients taking phenytoin, cyclosporine, or calcium channel blockers. Speech, mastication, tooth eruption, and aesthetics may be altered. This paper discusses a technique for root coverage using autogenous free gingival grafts. Twenty-two patients who had gingival recession on facial tooth surfaces were studied. The patients underwent surgical treatment, and pretreatment and posttreatment average probing depths for the distal, facial, and mesial tooth surfaces were compared. There was a general decrease in recession for all surfaces. Recession levels tended to remain stable for the duration of the study. There was a slight increase in recession on midfacial surfaces at 4 years that was not statistically significant.

In keeping with the concept of rapid epithelialization of close wound edges, the strip technique was developed to maximize the area of gingival grafting with less trauma to the donor site or the recipient site. An incision is made and a partial-thickness flap is reflected so that stable periosteum is left. The apical mucosal border of the recipient site is sutured to the periosteum. This paper demonstrates the clinical variety of gingival enlargement. An increase of the number of individuals who suffer from local or general enlargement caused by microbial gingival inflammation or different drugs can be demonstrated. The muco-gingival surgery represents one of the facet of periodontal surgery. The gingival recessions, characterized by a total attachmentless uncovering the root surface, become an esthetic and painful problem for our patients. The recessions can be treated by different surgical procedures. After a review of the etiologic factors and of the different type of wound healing, the authors describe and illustrate respectively the gingival graft for root coverage and the laterally positioned flap.
