Bone regeneration using the pouch-and-tunnel technique.

Several surgical techniques, such as the papilla preservation flap, the modified papilla preservation flap, and the sulcular incision flap, have attempted to achieve clot stabilization and graft coverage in an attempt to regenerate lost tissue due to periodontal disease. This case report focuses on soft tissue root coverage and bone regeneration in a one-wall osseous defect using the pouch-and-tunnel surgical procedure. The pouch-and-tunnel surgical technique is a minimally invasive periodontal plastic surgical procedure that uses subepithelial connective tissue as a free graft in a pouch bene

An insight into the possibilities of fibroblast growth factor in periodontal regeneration.

Periodontitis is caused by bacterial biofilms and is modulated by a variety of risk factors. The periodontal ligament comprises heterogeneous cell populations which are lost in the disease process. A variety of regenerative therapies, such as bone grafts, guided tissue regeneration treatment, application of enamel matrix derivative, have been introduced, with some success in periodontal tissue regeneration. Topical application of recombinant cytokines is now one of the most effective methods to stimulate stem cells. Researchers are now exploring the potential applications and uses of fibroblas…

Spontaneous tissue regeneration of the Stensen duct in dogs.

OBJECTIVE: There is a high incidence of wounds in the excretion duct of the salivary parotid gland owing to facial trauma that causes a loss of duct tissue. The aim of this study is to evaluate the ability of the salivary tissue to spontaneously self-regenerate, which is here widely demonstrated. STUDY DESIGN: This work was carried out on 20 adult dogs. A biomaterial catheter was inserted using microsurgical procedures to regenerate the lost duct tissue. This catheter served as an indicator for regenerating tissue growth. The results were assessed through clinical and radiographic exams by me

Barrier membranes in the treatment of periodontal defects.

Efficacious guided tissue regeneration for intrabony, furcation class II, and recession defects can be accomplished with both nonresorbable and bioresorbable barriers. The potential for regeneration of the periodontium is highly dependent on defect morphology and the availability of "progenitor cells." Many factors associated with surgical technique and barrier properties influence the regenerative outcome of guided tissue regeneration. Maintained flap coverage of the barrier minimizes epithelial down-growth as well as the risk of bacterial contamination of the barrier and the healing wound. C…

Efficacy of 3D printed anatomically equivalent thermoplastic polyurethane guide conduits in promoting the regeneration of critical-sized peripheral nerve defects.

Three-dimensional (3D) printing is an emerging tool for creating patient-specific tissue constructs analogous to the native tissue microarchitecture. In this study, anatomically equivalent 3D nerve conduits were developed using thermoplastic polyurethane (TPU) by combining reverse engineering and material extrusion (i.e. fused deposition modeling) technique. Printing parameters were optimized to fabricate nerve-equivalent TPU constructs. The TPU constructs printed with different infill densities supported the adhesion, proliferation, and gene expression of neuronal cells. Subcutaneous implanta

Optimized acellular nerve graft is immunologically tolerated and supports regeneration.

To replace the autologous graft as a clinical treatment of peripheral nerve injuries we developed an optimized acellular (OA) nerve graft that retains the extracellular structure of peripheral nerve tissue via an improved chemical decellularization treatment. The process removes cellular membranes from tissue, thus eliminating the antigens responsible for allograft rejection. In the present study, the immunogenicity and regenerative capacity of the OA grafts were tested. Histological examination of the levels of CD(8+) cells and macrophages that infiltrated the OA grafts suggested that the dec

Use of Melatonin/Decorticotomy and Autogenous Bone Graft in Induced 1-Wall Defect.

PURPOSE: This study was designed to investigate the effect of intramarrow penetration (IMP) and 1% melatonin (MLN) gel on the remodelling process of autogenous bone graft (ABG) in an induced 1-osseous wall defect model. METHODS: Sixty-four intrabony induced mandibular defects were created on the distal side of premolars-P1, P2, P3, and P4 (on each side)-in 8 beagle dogs. A ligature-induced periodontitis was initiated in each defect. Defects were then divided into 4 equal groups. Group I was treated with open-flap debridement (OFD) alone, group II was treated with OFD/ABG, group III was treate

In vitro evaluation of poly (vinyl alcohol)/collagen blended hydrogels for regulating human periodontal ligament fibroblasts and gingival fibroblasts.

Periodontitis is a chronic inflammatory disease that can destroy periodontal tissue. Guided tissue regeneration (GTR) is widely applied to treat periodontitis. However, the challenge is to develop a GTR membrane capable of simultaneously regenerating periodontal tissue and preventing epithelial downgrowth into the defect. Herein, blended hydrogels composed of polyvinyl alcohol (PVA) and fish collagen (Col) were prepared as GTR membranes. The morphology, Col release, and cellular behavior of the blended hydrogels were evaluated. The results showed that the surface porosity and Col release of th…

The intricate anatomy of the periodontal ligament and its development: Lessons for periodontal regeneration.

The periodontal ligament (PDL) connects the tooth root and alveolar bone. It is an aligned fibrous network that is interposed between, and anchored to, both mineralized surfaces. Periodontal disease is common and reduces the ability of the PDL to act as a shock absorber, a barrier for pathogens and a sensor of mastication. Although disease progression can be stopped, current therapies do not primarily focus on tissue regeneration. Functional regeneration of PDL may be achieved using innovative techniques, such as tissue engineering. However, the complex fibrillar architecture of the PDL, essen

Reconstructive strategies for dermatofibrosarcomas of the face: role of regenerative dermal templates.

BACKGROUND: Dermatofibrosarcomas protuberans is a challenging cutaneous tumor from an oncologic and reconstructive surgical point of view. Involvement of functionally and aesthetically sensitive areas, such as facial units, in young patients accounts for more demanding cases. An updated evaluation of most beneficial excisional/reconstructive strategies in these cases is still lacking. METHODS: We investigated the potential of regenerative dermal templates in staged postoncologic reconstructive management of a young woman affected by a dermatofibrosarcomas protuberans of the forehead involving

Applications of basic fibroblastic growth factor (FGF-2, bFGF) in dentistry.

Recent developments in research have been based on the maintenance and regeneration of natural organs and tissues; among such developments is the use of growth factors (GFs). The use of basic fibroblastic growth factors (bFGF) may be indicated in different disciplines of dentistry such as periodontics and dental traumatology. These cells' ability to induce proliferation and differentiation of cells may make GFs a useful source for the development of natural structures. This mini-review will discuss how bFGF can be beneficial to dentistry in relation to 1) re-implantation/autotransplantation of

Gingival recession treatment using a bilayer collagen membrane.

Traditional periodontal plastic surgery techniques for the management of gingival recession have typically used soft tissue grafts to obtain defect coverage with great clinical success. Clinicians using guided tissue regeneration (GTR) techniques are also enjoying significant success in periodontal plastic surgery procedures. GTR therapy utilizing bioabsorbable membranes offers the advantages of preventing a donor site surgery and a second surgical procedure for barrier removal. A new bioabsorbable bilayer collagen membrane that readily adapts to bone and tooth surfaces by a gel formation of c…

Hierarchically Patterned Polydopamine-Containing Membranes for Periodontal Tissue Engineering.

Periodontitis is a common chronic inflammatory disease that affects tooth-supporting tissues. We engineer a multifunctional periodontal membrane for the guided tissue regeneration of lost periodontal tissues. The major drawback of current periodontal membranes is the lack of tissue regeneration properties. Here, a series of nanofibrous membranes based on poly(ε-caprolactone) with tunable biochemical and biophysical properties were developed for periodontal tissue regeneration. The engineered membranes were surface coated using biomimetic polydopamine to promote the adhesion of therapeutic prot…

Local delivery of prostaglandin E1 induces periodontal regeneration in adult dogs.

Formation of cementum, alveolar bone and periodontal ligament was produced in 18 sites on buccal surfaces of mandibular premolars and molars of 11 adult dogs near to sites of local delivery of prostaglandin E1 (PGE) for three weeks. Mineralizing bone and cementum were labelled with fluorescent dyes and polarizing microscopy showed periodontal ligament fibers between these new mineralized tissues. These observations extend recent demonstrations that local application of PGE causes formation of new bone on the mandible and suggest the potential for predictable, site-directed periodontal regenera

Peripheral nerve regeneration using a bioresorbable silk fibroin-based artificial nerve conduit fabricated via a novel freeze-thaw process.

While silk fibroin (SF) obtained from silkworm cocoons is expected to become a next-generation natural polymer, a fabrication method for SF-based artificial nerve conduits (SFCs) has not yet been established. Here, we report a bioresorbable SFC, fabricated using a novel freeze-thaw process, which ensures biosafety by avoiding any harmful chemical additives. The SFC demonstrated favorable biocompatibility (high hydrophilicity and porosity with a water content of > 90%), structural stability (stiffness, toughness, and elasticity), and biodegradability, making it an ideal candidate for nerve rege

4D-Printed MXene-Based Artificial Nerve Guidance Conduit for Enhanced Regeneration of Peripheral Nerve Injuries.

Repairing larger defects (>5 mm) in peripheral nerve injuries (PNIs) remains a significant challenge when using traditional artificial nerve guidance conduits (NGCs). A novel approach that combines 4D printing technology with poly(L-lactide-co-trimethylene carbonate) (PLATMC) and Ti3C2Tx MXene nanosheets is proposed, thereby imparting shape memory properties to the NGCs. Upon body temperature activation, the printed sheet-like structure can quickly self-roll into a conduit-like structure, enabling optimal wrapping around nerve stumps. This design enhances nerve fixation and simplifies surgical

Unrestricted Somatic Stem Cells Loaded in Nanofibrous Conduit as Potential Candidate for Sciatic Nerve Regeneration.

Motor and sensory recovery following critical size peripheral nerve defects is often incomplete. Although nerve grafting has been proposed as the gold standard, it is associated with several disadvantages. Here we report a novel approach to peripheral nerve repair using Human Unrestricted Somatic Stem Cells (USSC) delivered through an electrospun neural guidance conduit. Conduits were produced from PCL and gelatin blend. Several in vitro methods were utilized to investigate the conduit's physicochemical and biological characteristics. Nerve regeneration was studied across a 10-mm sciatic nerve
