[Partial testicular feminization syndrome].

OBJECTIVES: To report one case of partial androgen insensitivity syndrome. METHODS: The patient underwent history and physical examination, blood tests, hormone determinations, androgen stimulation and suppression tests, nuclear chromatin and karyotype, imaging and endoscopic tests, exploratory laparoscopy with gonadal biopsy and bilateral orchyectomy, and subsequent genital plasty. RESULTS: We report the case of a phenotypically female patient with genital ambiguity, with a dorsally curved peniform clitoris and a urogenital sinus with partial labia fusion. Blood tests, hormonal determinatio

Applications of intraoperative ultrasound in gynecological surgery.

PURPOSE OF REVIEW: Gynecologists are well trained in office-based ultrasound, but are less experienced in the field of intraoperative ultrasound. Many gynecologic procedures may benefit from the use of real-time ultrasonography. The purpose of this review is to summarize the current use of intraoperative ultrasound in gynecologic procedures. RECENT FINDINGS: Evaluation and assessment of the value of intraoperative ultrasound in gynecological procedures is essentially non-existent. The role of intraoperative ultrasound in gynecology is in its infancy, with anecdotal experience and literature i

Vaginal repair of vesico-vaginal fistulas: our experience.

Vesico-vaginal fistula repair is feasible both with abdominal and vaginal approach. We report our experience in vaginal repair of 34 vesico vaginal fistulas. Twenty-eight out of 34 fistulas have been treated by vaginal repair (82.3%) (in 2 cases after an attempt of endoscopic repair), 4/34 (11.8%) by abdominal repair and 4/34 (11.8%) by endoscopic repair with good outcome in 2 cases. In 96% of fistulas treated by vaginal repair (27/28) we obtained a complete recovery; in one patient we observed fistula recurrence after 2 months from the operation. Four patients were treated by abdominal approa

[Genital prolapse and colpocele. Coelioscopic sacrocolpopexy].

Genito-urinary prolapse is a common disease that requires a global approach which includes all the compartments. Sacrocolpopexy with anterior and posterior mesh allows long-lasting anatomical restoration with a preservation of the sexual function. The laparoscopic approach is a modern and efficient answer for this functional surgery.

Transvaginal hysterectomy: rationale and surgical approach.

Abdominal hysterectomy is performed in the United States at a 3:1 ratio over vaginal hysterectomy, despite evidence that vaginal hysterectomy offers advantages over abdominal hysterectomy with regard to operative time, complication rates, recovery, return to daily activities, and overall costs of treatment. In fact, the predominance of the abdominal approach may be based on factors other than clinical considerations, including resident training, use of limited or obsolete guidelines, greater third-party compensation for abdominal procedures, a presumption rather than a confirmation that pathol

Techniques for pelvic surgery in subfertility.

BACKGROUND: Since the advent of in-vitro fertilisation (IVF) the role of infertility surgery has diminished. This type of surgery is still however widely performed.and there are many different surgical techniques that can be used to repair blocked or damaged Fallopian tubes. Most evidence in this area comes from uncontrolled series. OBJECTIVES: To evaluate surgical techniques for the treatment of tubal infertility (adhesiolysis, salpingostomy, surgery for proximal tubal occlusion, reversal of sterilisation). SEARCH STRATEGY: This review has drawn on the search strategy developed for the Subf

Endoscopic septotomy: a new surgical approach to infantile hydrometrocolpos with imperforate hemivagina and ipsilateral renal agenesis.

The authors propose a new surgical approach for neonates and infants with hydrometrocolpos caused by double vagina and imperforate hemivagina. Usually for these patients, laparotomy is a common approach used to decompress the obstructed hemivagina. The authors compared the characteristics of three options used to relieve obstruction of the hemivagina using endoscopic, transvaginal, and laparotomy approaches. An endoscopic septotomy (colposcopic approach) using the Storz neonatal resectoscope is less invasive and less expensive than other methods. We emphasize that endoscopic septotomy is a fea

Laparoscopic surgery of unicornuate uterus with rudimentary uterine horn.

This report describes a new procedure for laparoscopic treatment of non-communicating rudimentary uterine horn when attached to the contralateral unicornuate uterus by a band of tissue. A retrograde dissection with primary bipolar coagulation and section of the band of tissue enables primary occlusion of the main blood supply. In our opinion, this new approach may prevent bleeding during laparoscopic dissection of the rudimentary horn and may avoid myometrial injury of the resting unicornuate uterus.

Development of a Realistic Physical Phantom for Laparoscopic and Robotic-Assisted Sacrocolpopexy Training and Associated.

OBJECTIVE: Robotic-assisted sacrocolpopexy is the gold standard for treating advanced pelvic organ prolapse, but its most critical step, presacral dissection, carries a significant risk of vascular and ureteral injury. Traditional training relies heavily on intraoperative exposure, yet no simulator currently provides an anatomically realistic, reusable, and cost-effective platform for practicing this high-risk procedure. METHOD: To address this gap, we developed a modular phantom designed for training in laparoscopic and robotic sacrocolpopexy procedures. A hierarchical task analysis was cond

Outcomes of concomitant midurethral sling and anterior colporrhaphy in managing stress urinary incontinence associated with cystocele: a systematic review and single-arm analysis.

BACKGROUND: Multiple surgical approaches have been provided for treating Stress urinary incontinence (SUI) combined with anterior vaginal wall prolapse (cystocele). However the optimal treatment remains a topic of ongoing debate. This systematic review and single-arm analysis aimed to evaluate the efficacy and safety of midurethral sling (MUS) with concomitant anterior colporrhaphy (AC) for surgical treatment of stress urinary incontinence associated with grade 1-3 anterior vaginal wall prolapse. METHODS: We searched through multiple databases including, PubMed, Cochrane, Scopus, and Web of S

Robot-Assisted Laparoscopic Sacrohysteropexy with Autologous Fascia Lata.

INTRODUCTION AND HYPOTHESIS: Autologous fascia lata has been increasingly utilised in pelvic floor reconstructive surgeries such as sacrocolpopexy and sacrohysteropexy. This case highlights sacrohysteropexy with autologous fascia lata as a promising option for women with advanced uterovaginal prolapse who wish to preserve their uterus and avoid synthetic mesh. METHODS: We report the case of a 65-year-old woman with stage 3 pelvic organ prolapse following one forceps and one spontaneous vaginal delivery. She presented with cervical-dominant prolapse (6 cm beyond the hymen) and a widened genita

Preoperative low-residue diet in gynecological surgery.

OBJECTIVE: To evaluate the impact of preoperative low-residue diet on intra- and postoperative outcomes among gynecological surgical patients. METHODS: This is a surgeon-blind, randomized controlled trial enrolling patients undergoing elective surgery for either benign disease or endometrial carcinoma. Patients were preoperatively randomized to receive either low-residue diet (arm A) or free diet (arm B) starting from three days before surgery. The primary outcome was the quality of the surgical field (scored using a 5-point scale, from poor to excellent). Secondary outcomes were postoperativ
