Early motion after hand and wrist reconstruction.

There is substantial evidence to support the fact that early motion is beneficial for bones, joints, ligaments, tendons, and muscles. An Early Mobilization Program was designed to provide early motion for all these structures after hand and wrist reconstruction.

[Transpalmar transplantation of a severed hand to the opposite side: a case report].

A man of 20 got both his upper limbs badly injured by an accident on May 28, 1989. Crush of all the 4 fingers of right hand, traumatic amputation through the left arm with severe damage of soft tissue and burst fracture of bones around left elbow. And so, the chance of providing him a useful hand could only be transpalmar transplantation of the undamaged left hand to the opposite side. This was done urgently to reconstruct a new hand, a hand with its fingers in a reversed order. The wound healed up nicely, the transplanted fore-hand was well taken and the new hand functions satisfactorily, tho

Imaging of the Apert syndrome hand using three-dimensional CT and MRI.

Imaging of the Apert syndrome hand is usually done with plain x-rays. This gives only a limited assessment of the grossly abnormal anatomy. Computed tomography (CT) and magnetic resonance imaging (MRI) have the ability to comprehensively demonstrate the spatial arrangement of the bones as well as the anatomy of the soft-tissue structures. This paper demonstrates the use of these two imaging modalities in the complex Apert syndrome hand to improve surgical planning and treatment.

Subcutaneous and osteolytic rhinosporidiosis.

A young man presented with multiple Subcutaneous nodules over scalp, hand, feet and osteolytic lesions of small bones of hand. Clinically and radiologically he was diagnosed as a case of Giant Cell Tumour. Aspiration cytology and biopsy proved it to be rhinosporidiosis. Epidemiological study revealed that he perhaps contracted this infection as an occupational hazard. This is the third reported case of osteolytic lesions due to rhinosporidiosis. Diagnostic dilemmas of subcutaneous and osteolytic rhinosporidiosis are discussed.

Three-dimensional reconstruction of the hand from biplanar X-rays: Assessment of accuracy and reliability.

BACKGROUND: Functional disorders of the hand are generally investigated first using conventional radiographic imaging. However, X-rays (two-dimensional (2D)) provide limited information and the information may be reduced by overlapping bones and projection bias. This work presents a three-dimensional (3D) hand reconstruction method from biplanar X-rays. METHOD: This approach consists of the deformation of a generic hand model on biplanar X-rays by manual and automatic processes. The reference examination being the manual CT segmentation, the precision of the method was evaluated by a comparis

The carpal bones in Poland syndrome.

OBJECTIVE: Classical Poland syndrome is represented by unilateral aplasia of the sternocostal head of the pectoralis major muscle and ipsilateral simple syndactyly and brachydactyly. Various classifications of the severity of hand involvement have been proposed. Since its initial description, numerous studies have been made of the bony, soft tissue, organ, and hematological disturbances. However, carpal bone involvement has been largely overlooked. The purpose of this study was to evaluate the carpal bones in patients with Poland syndrome from a local (Manitoba) cohort as well as those from th

[Free microvascular fibula graft for skeletal reconstruction of the distal radius: 5 years follow-up after en-bloc resection of a giant cell tumour].

A giant cell tumour of the distal radius or of the carpal bones, verified by incisional biopsies, should be approached as a low-grade malignancy. Lesions that arise in the bones of the hand or the wrist have a greater propensity to metastasise, and have a higher risk of local recurrence. In this case report we describe a 54-year-old women with a giant cell tumour of the distal radius (Campanacci grade III lesion), having a follow-up of five years without signs of local recurrence or metastatic disease. The general principles of and options for surgical treatment are discussed.

[Avascular necrosis of the capitate in athletes].

The possibility of osteonecrosis of the carpal bones should always be considered when athletes present with pain of unknown origin in the hand and wrist, in particular, if they are participating in sports such as gymnastics or weight-lifting that involve extreme loading of the wrist with axial compression and microtrauma. This sort of extreme loading of the wrist combined with a constitutionally "weak" blood supply to the individual carpal bones may lead to the formation of osteo-necrotic zones. A treatment method that can produce excellent results, depending on the pathomorphology, is availab

[Roentgen diagnosis of hypo- and hyperparathyroidism].

The internal organs and the bone and joint system of 970 patients with hyperparathyrosis and the bone and joint system of 146 persons with hypoparathyrosis, as well as of patients with other nosology, in whom these diabetes are suspected, underwent roentgenoscopy. It is suggested that indirect thyroid lymphography, combined with esophagoscopy, is an effective method for revealing changes in the parathyroid glands. The characteristic signs of hyperparathyrosis are subperiosteal resorption of the hand bones and specific osteoporosis of the cranial bones, particularly in their combination with sy

Fractures of the carpal bones.

Fractures of the carpal bones, excluding the scaphoid, are less common and are often missed on standard, plain radiographs. The diagnosis requires knowledge of the anatomy and common fracture patterns of the bones and the specialized radiographic views necessary to image them. Although the hamate hook, trapezial ridge, and pisiform often fracture from direct trauma, other fractures of the carpus may indicate more widespread injury and require a detailed evaluation to rule out associated perilunate or carpometacarpal fracture-subluxations. Fortunately, if treated appropriately, the vast majorit

[The late results of observing and treating patients with histiocytosis X and involvement of the jaw bones].

Eighty-one patients with histiocytosis X involving the jaw bones were followed up for 30 years; the patients presented with involvement of the lungs, skin, mucosa, other bones of the skeleton, and diabetes insipidus signs. In 58 of the 81 patients eosinophilic granuloma eventuated in a generalized process: chronic xanthomatosis or Hand-Schüller-Christian's disease. This fact permits a conclusion that eosinophilic granuloma is an initial stage of the same systemic disease, histiocytosis X, and that subdivision of histiocytosis X into 3 forms, accepted in literature, is undesirable, for it erron

Osteoblastoma of the sternum--case report and review of the literature.

Osteoblastoma is an extremely rare entity that represents less than 1% of all bone tumors, and affects twice as many males as females with peak incidence between 15 and 20 years. Osteoblastomas commonly affect axial bones, long bones, bones of the foot and hand, and less commonly the pelvis, scapula, ribs, and clavicle. Osteoblastoma does not have a classic presentation, but can vary with the location and size of the tumor. The main complaint is often progressive pain localized at the tumor site. Osteoblastoma is a benign tumor with an aggressive behavior. The treatment is wide surgical resect

Scaphoid numerical simulation of the critical loading until fracture.

The numerical study of the scaphoid fracture, although it is relatively unexplored, can be of great clinical interest since it is highly common and can result in temporary or persistent disability. In this manuscript, seven combinations of boundary conditions and contacts between adjacent bones, together with four different loads, simulating real hand movements, are assessed. Three different fracture criteria for bones are employed to study the failure of the scaphoid with the aforementioned combination of interaction conditions. The results offer an interesting view of the accuracy of the pos

Hand fractures and return to play in elite Australian cricketers.

INTRODUCTION: Hand fractures are one of the most common injuries sustained whilst playing cricket. Further research is required to inform future clinical management and risk-reduction strategies. METHODS: This retrospective cohort study reviewed all cases of hand fractures in elite Australian cricket players over a three-year period. Data included specific activity when injury occurred, location of injury, management (non-surgical or surgical) and days to return to play. RESULTS: Seventy (17%, 95% CI 14-21 of players; 43 male, 27 female) players sustained 90 hand fractures. Seventy-three (81

Percutaneous treatment of carpal, metacarpal, and phalangeal injuries.

Percutaneous fixation of hand fractures is a common technique that takes advantage of the subcutaneous nature of hand bones, their small size, and their limited loading potential for stress placed on hardware. Percutaneous wire fixation supplements cast fixation when plaster cannot hold particular reductions, and allow surgical fixation with limited postoperative swelling. In the first part of the current study, the types of wires that are used for hand fixation, fluoroscopy, helpful instruments, and the basic techniques used for this type of surgery are discussed. In the second part of the st

[Fractures of the hand skeleton].

The complex anatomical conditions of the hand pose special problems for fracture treatment. On the one hand length and axes as well as rotation of the fractured bones have to be reestablished in order to prevent functional impairment. On the other hand operative possibilities are limited by the overlying vessels, nerves and tendons. Certain types of fractures are successfully treated by conservative means. Disadvantages are the long time of immobilisation and sometimes clumsy bandages. Other types of fractures require operative treatment in order to accomplish correct reduction. In addition st

Paediatric Hand Fractures - A Review.

Paediatric hand fractures account for approximately 25% of hand-related emergency department visits, making them the second most common childhood fractures after distal forearm fractures. These injuries are more prevalent in boys and peak ages between 9 and 14 years. Hand fractures in children are primarily caused by crushing injuries and, increasingly in older children, by sports-related activities. The little finger (52%) and thumb (23%) are the most frequently fractured digits, with phalangeal fractures, especially of the proximal phalanx, being common. Metacarpal fractures are more prevale
