Finger 'pebbles'. A dermatologic sign of diabetes mellitus.

A 35-year-old obese man presented with a chief complaint of hand dryness of 5 years' duration. He was a store manager and denied exposure to chemicals, repetitive trauma, chronic irritation, and hard manual labor. However, he did admit to frequent hand washing. He had no itching or swelling in his hands, but on occasion he had tenderness in the dry areas. [Differential diagnosis of tinea manus and tinea pedis].

Dermatophytoses of hands and feet may appear under a variety of different clinical pictures which can easily be mistaken for other skin diseases. The degree of acute or chronic inflammation and of a disturbed keratinization determines if vesicle or pustule formation, scaling, hyperkeratosis or maceration are predominant features of a tinea manus or tinea pedis. Several examples of similar clinical manifestations e.g. eczema, psoriasis, lichen planus, are given. It is stressed that direct microscopy and culture of the fungus are prerequisites of a correct mycological diagnosis.

Chromohyphomycosis of the hand due to Exophiala jeanselmei (Phialophora jeanselmei, Phialophora gougerotii)--case report and review.

A case of an unusual cause of chromomycosis of the hand is described. Follow-up at 2 months revealed a complete recovery following removal of the mass, without treatment with antifungal therapy. A review of the different disease entities of the chromomycoses and a brief generic classification of the dematiacious fungi are presented.
