Early innovations in opioid use disorder treatment and harm reduction during the COVID-19 pandemic: a scoping review.

BACKGROUND: The COVID-19 pandemic has exerted a significant toll on the lives of people who use opioids (PWUOs). At the same time, more flexible regulations around provision of opioid use disorder (OUD) services have led to new opportunities for facilitating access to services for PWUOs. In the current scoping review, we describe new services and service modifications implemented by treatment and harm reduction programs serving PWUO, and discuss implications for policy and practice. METHODS: Literature searches were conducted within PubMed, LitCovid, Embase, and PsycInfo for English-language …

Reducing toxic reactive carbonyl species in e-cigarette emissions: testing a harm-reduction strategy based on dicarbonyl trapping.

Reducing the concentration of reactive carbonyl species (RCS) in e-cigarette emissions represents a major goal to control their potentially harmful effects. Here, we adopted a novel strategy of trapping carbonyls present in e-cigarette emissions by adding polyphenols in e-liquid formulations. Our work showed that the addition of gallic acid, hydroxytyrosol and epigallocatechin gallate reduced the levels of carbonyls formed in the aerosols of vaped e-cigarettes, including formaldehyde, methylglyoxal and glyoxal. Liquid chromatography mass spectrometry analysis highlighted the formation of coval

Addressing excess risk of overdose among recently incarcerated people in the USA: harm reduction interventions in correctional settings.

Purpose The purpose of this paper is to discuss overdose among those with criminal justice experience and recommend harm reduction strategies to lessen overdose risk among this vulnerable population. Design/methodology/approach Strategies are needed to reduce overdose deaths among those with recent incarceration. Jails and prisons are at the epicenter of the opioid epidemic but are a largely untapped setting for implementing overdose education, risk assessment, medication assisted treatment, and naloxone distribution programs. Federal, state, and local plans commonly lack corrections as an ing…

Young people who use drugs engaged in harm reduction programs in New York City: Overdose and other risks.

BACKGROUND: Little is known about the engagement of young people who use drugs (PWUD) in harm reduction programs (HRPs), and few studies have included non-opioid users and non-injectors. While HRPs have effectively engaged PWUD, young people are under-represented in their services. METHODS: The Injection Drug Users Health Alliance Citywide Study (IDUCS) is the largest community-based study of PWUD in HRPs in the US. From 2014-2015, 2421 HRP participants across New York City (NYC) completed a cross-sectional survey. We investigated differences in socio-demographics, service utilization, and ri…

From Screen-Time Control to Digital Resilience: A Harm-Reduction Framework for Adolescent Internet Gaming Disorder in China.

Internet Gaming Disorder (IGD) among adolescents is a mental health issue associated with multiple levels of influence, including individual development, game design, self-regulation, and the broader offline environment. Drawing on a narrative synthesis of relevant psychological evidence, together with conceptual analysis and normative policy analysis, this paper examines the extent to which China's existing anti-addiction measures correspond to the risk structure of IGD. Existing evidence indicates that a range of individual, environmental, and game-design factors are associated with problema

Current Tobacco Product Use and Cessation Behaviors Among People Who Use Opioids: Implications for Integrated Harm Reduction Services.

INTRODUCTION: Tobacco use, particularly cigarette smoking, is disproportionately high among people who use opioids (PWUO). Although harm reduction programs provide trusted, low-barrier, and repeated contact with PWUO, tobacco cessation support is not routinely integrated into these settings. We assessed current use of different forms of tobacco products and smoking cessation behaviors among PWUO and considered implications for integrating tobacco cessation into harm reduction services. METHODS: We conducted a cross-sectional survey among PWUO attending a syringe service program (SSP) in New H…

Smoking Cessation in Primary Care: A Narrative Review of Evidence-Based Treatment, Emerging Technology, and Harm Reduction Approaches.

Conventional cigarette smoking remains a leading preventable cause of morbidity and mortality despite declining prevalence, yet evidence-based treatments remain underused. This narrative review examines guideline-supported therapies and emerging approaches, including digital tools, neuromodulation, and harm-reduction strategies. Standard care typically combines FDA-approved pharmacotherapy with behavioral counseling. Digital interventions can extend access and support adherence, particularly when integrated with pharmacotherapy and follow-up. Repetitive transcranial magnetic stimulation (rTMS)

Reshaping addiction harm reduction program in Iran: The Ministry of Health's journey from reactive measures to strategic optimization.

Iran's national harm reduction program, scaled up during a period of substantial decline in HIV prevalence among people who inject drugs, later faced growing operational challenges as client numbers declined at many service sites. This Comment describes the evidence-based restructuring undertaken by the Ministry of Health and Medical Education (MoHME) after approximately two decades of implementation. National studies on population size estimation, geospatial distribution of injecting drug use, and service quality assessment informed a major reconfiguration of the harm reduction network. Under…

Harm reduction self-efficacy and motivations for contactless supply access among a sample of syringe services program participants.

BACKGROUND: Contactless harm reduction supply methods (e.g., vending machines, mail order, mobile delivery) have become prevalent in the United States. However, this approach has faced some criticisms, including the notion that, unlike staffed syringe services programs, contactless methods do not provide face-to-face support, education, or referrals to treatment, potentially limiting their overall impact. METHODS: We collected self-reported data from a sample of people who inject drugs who accessed a syringe services program (N = 50), including their demographics, harm reduction self-efficacy…

Balancing abstinence and harm reduction across the continuum of care for liver transplantation in alcohol-associated liver disease.

Alcohol use following liver transplantation (LT) is associated with worse outcomes. Strategies for predicting, preventing, and managing alcohol consumption among candidates and recipients of LT with alcohol-associated liver disease (ALD) have historically emphasized complete abstinence. We propose a harm reduction approach as a possible complementary and realistic strategy for managing alcohol use in the context of LT for ALD. In this article, we outline the ethical and clinical foundations for applying harm reduction across the continuum of care for LT in ALD. Although complete alcohol abstin…
