How to do it? Practical guidance on organising mass antibiotic prophylaxis for a large school after a cluster of meningococcal disease.

After a second meningitis case occurred in a school, we undertook mass prophylaxis of 2095 pupils and staff of three institutions on the site. This incident, early in the life of the new primary care organisations, presented several challenges and provided opportunities to learn about successful co-management of future major incidents. This paper aims to identify and address problems in planning such prophylaxis, with particular reference to the new NHS arrangements. It also provides a practical checklist, with commentary on potential issues based on our local experience. This may help other h

Joint human and animal vaccination strategies to reach mobile pastoralists and their livestock in Africa: A scoping review.

Africa's pastoralist population exceeds 268 million and occupies over 40% of the continent's land. Mobile pastoralists connected to their livestock are underrepresented in data and face barriers to accessing fixed vaccination services. We aimed to identify and describe joint human-animal vaccination strategies to improve immunisation coverage among mobile pastoralists and their animals. We conducted a scoping review and followed Joanna Briggs Institute and PRISMA-ScR guidelines. We screened 2469 records from 12 databases. Five studies (2004-2016) from Chad, Nigeria, and Somalia met the inclusi

Comparison of AI-generated renal diets by different large language models: a guideline-based evaluation with expert input.

BACKGROUND: Chronic kidney disease (CKD) represents a major public health concern due to its increasing prevalence worldwide and the substantial burden it places on healthcare systems. Nutritional therapy plays a critical role in slowing the progression of the disease. Recently, large language models (LLMs) such as ChatGPT, Copilot, and Gemini have been introduced for dietary planning in patients with kidney disease. However, the accuracy, reliability, and guideline adherence of the diets generated by these models remain uncertain. This study aimed to evaluate AI-based dietary recommendations

Adnexal tumors in menopausal women: surgery or surveillance?

Asymptomatic ovarian tumors in menopausal women do not always require surgical removal. Experts recommend an initial evaluation with transvaginal ultrasound to help characterize the tumor's malignant risk. Low-risk tumors can be monitored with ultrasound, whereas high-risk tumors should be referred to a gynecologic oncologist. Indeterminate tumors require secondary testing with the strategies outlined in this Practice Pearl.

Trends of resistance to antimicrobials recommended currently and in the past for management of gonorrhea in the Apex STD center in India and comparison of antimicrobial resistance profile between 2002-2006 and 2007-2012.

BACKGROUND: Antimicrobial resistance in Neisseria gonorrhoeae jeopardizes public health and continues to spread out to currently recommended and older antimicrobial agents. Antimicrobial resistance (AMR) surveillance provides essential clues toward the modification of treatment guidelines. The aim of the study was to determine gonococcal AMR profile and trends between 2007 and 2012 and to evaluate any change in AMR profile in comparison with published trends in 2002 to 2006. METHODS: Antimicrobial susceptibility testing of 261 N. gonorrhoeae isolates from consecutive patients between 2007 and

Healthcare utilization and expenditures for low income children with sickle cell disease.

BACKGROUND: While multiple studies have examined the healthcare burden of sickle cell disease (SCD) in adults, few have specifically focused on healthcare utilization and expenditures in children. The objective of this study was to characterize the healthcare utilization and costs associated with the care of low-income children with SCD in comparison to other children of similar socioeconomic status. PROCEDURE: For the study period, 2004-2007, we conducted a retrospective, cross-sectional descriptive analysis of administrative claims data from a managed care plan exclusively serving low-incom

Evaluation of Stillbirth.

Stillbirth remains a significant and poorly understood pregnancy complication in the United States. Despite medical advancements, the US stillbirth rate exceeds that of other high-income countries. A key challenge is the high proportion of unexplained cases, which hinders prevention efforts. However, systematic evaluation can significantly reduce unexplained stillbirths. Identifying causes is crucial for families and health care providers, aiding in grief processing, future pregnancy planning and stillbirth prevention efforts. Best practices in pain assessment and management for children.

Pain assessment and management are essential components of paediatric care. Developmentally appropriate pain assessment is an important first step in optimizing pain management. Self-reported pain should be prioritized. Alternatively, developmentally appropriate behavioural tools should be used. Acute pain management and prevention guidelines and strategies that combine physical, psychological, and pharmacological approaches should be accessible in all health care settings. Chronic pain is best managed using combined treatment modalities and counselling, with the primary goal of attaining func

Intimate partner violence victimization: identification and response in primary care.

In the United States more than 1 out of 3 women experiences lifetime intimate partner violence (IPV) victimization. Short screening instruments such as HITS or the AAS can identify IPV victimization. Nonjudgmental statements that validate an IPV victim's experience should be followed by safety assessment and planning. Intervention includes referral to services, treatment of associated health conditions, mandatory reporting if required, and documentation. Counseling has been shown to reduce IPV victimization. Clinical guidelines recommend IPV screening for all or most women, and providing or re

Dental considerations in the management of the cardiac transplant patient.

Every year, more than 25,000 transplantation procedures are performed in the United States to replace solid organs, including the heart, intestine, kidney, liver, lung, and pancreas. Cardiac transplant patients need specialized dental care. The compromised health and immune systems of these patients place them at increased risk for systemic and oral infections, which must be considered when planning dental treatment before and after cardiac transplantation. This article reviews the current status of dental care practices that have been recommended and presents a rationale that can be applied a

Infection rislks associated with urinary catheters.

Urinary catheterisation can kill. It is one of the most common healthcare interventions but carries the risk of 'significant danger' to patients from infection (Pratt et al 2001). While essential for some patients, this invasive procedure is sometimes unjustified and even forgotten once in place (Saint et al 2005). Accountability for the safe management of urinary catheters by using evidence-based guidelines is emphasised. New ways of examining practice and planning for improvement (Department of Health 2005), and new technology (Tambyah 2004) will contribute to better outcomes for those patie

Diagnosing neurofibromatosis type I in children.

Neurofibromatosis is the single most common genetic disease of the neurologic system. Because neurofibromatosis type 1 (NF-1) is often diagnosed early in life, it is essential that health care providers deepen their understanding of this common genetic disorder. Children who meet particular criteria, as formulated by the National Institutes of Health, must be worked-up for NF-1 in order to initiate appropriate treatment implementation and evaluation. This article provides a review of the literature regarding the pathophysiology, clinical manifestations, and plan of treatment related to NF-1, s

Guidelines for the documentation and verification of measles, rubella, and congenital rubella syndrome elimination in the region of the Americas.

In the region of the Americas, goals for the elimination of endemic measles and rubella/congenital rubella syndrome (CRS) by the year 2000 and 2010, respectively were established. The successful implementation of measles elimination strategies in the region of the Americas resulted in the interruption of endemic measles transmission in 2002 and tremendous progress toward rubella and CRS elimination. In October 2007, the 27th Pan American Sanitary Conference adopted Resolution CSP27.R2 urging member states to begin documenting and verifying the interruption of endemic transmission of the measle
