UK prevalence of underlying conditions which increase the risk of severe COVID-19 disease: a point prevalence study using electronic health records.

BACKGROUND: Characterising the size and distribution of the population at risk of severe COVID-19 is vital for effective policy and planning. Older age, and underlying health conditions, are associated with higher risk of death from COVID-19. This study aimed to describe the population at risk of severe COVID-19 due to underlying health conditions across the United Kingdom. METHODS: We used anonymised electronic health records from the Clinical Practice Research Datalink GOLD to estimate the point prevalence on 5 March 2019 of the at-risk population following national guidance. Prevalence for

Improving Health and Cancer Services in Low-Resource Countries to Attain the Sustainable Development Goals Target 3.4 for Noncommunicable Diseases.

The United Nations Sustainable Development Goals 2015 to 2030 includes a specific goal for health (Sustainable Development Goal [SDG] 3) with 13 targets, including SDG3.4 for the control and treatment of noncommunicable diseases (NCDs), namely, cardiovascular diseases, cancer, diabetes, and chronic lung disease. There is considerable concern that SDG3.4 may not be achieved. The WHO Best Buys for NCDs has emphasized prevention, and although crucial, it alone will not achieve the 30% reduction in NCDs by 2030. Likewise, a strengthened health system is required as all NCDs are likely to require h

Heat-health action plans overlook Europe's most important health system resource: the health and care workforce.

Europe is warming at more than twice the global average, and heatwaves increasingly coincide with extreme wildfires, rainfall, floods and storms, creating growing health security risks while intensifying an ongoing health and care workforce crisis. Governments across the European Region have scaled-up action to establish heat-health action plans, supported by comprehensive WHO guidance. However, health and care workers remain largely absent from these strategies, which continue to place their focus on protecting populations rather than the workforce responsible for delivering care. Workforce m

Dyslexia in higher education: implications for maths anxiety, statistics anxiety and psychological well-being.

This study examined levels of mathematics and statistics anxiety, as well as general mental health amongst undergraduate students with dyslexia (n = 28) and those without dyslexia (n = 71). Students with dyslexia had higher levels of mathematics anxiety relative to those without dyslexia, while statistics anxiety and general mental health were comparable for both reading ability groups. In terms of coping strategies, undergraduates with dyslexia tended to use planning-based strategies and seek instrumental support more frequently than those without dyslexia. Higher mathematics anxiety was asso

Gender differences in retirement planning: A longitudinal study among Spanish Registered Nurses.

BACKGROUND: Consistent patterns of gender differences in retirement planning behaviours have been shown but little is known about these behaviours among nurses. AIMS: To analyse the antecedents of the behaviours to prepare for retirement in nurses older than 55 and to identify differences as a function of gender. METHODS: A two-wave longitudinal study with Spanish nurses (n = 132). RESULTS: Statistically significant gender differences were revealed. Specifically, paths from financial knowledge to public protection and self-insurance as well as paths from goals clarity to public protection a

Clinical trial protocol of PODRACING: A randomized controlled trial evaluating 3D perfusion zone models for selective arterial clamping during robot-assisted partial nephrectomy.

BACKGROUND: Performing partial nephrectomy (PN) for suspected renal cell carcinoma (RCC) requires a careful clamping strategy to balance blood loss and postoperative kidney function. For better prediction of individual kidney perfusion, the proposed DIPLANN-tool visualizes arterial perfusion zones using 3D models from CT scans to support surgeons in planning selective clamping (SC). We hypothesize that using 3D perfusion zone (3DPZ) models allows for more frequent and more accurate SC during robot-assisted PN (RAPN). Furthermore, it might also benefit patients' health and insight. STUDY DESIG

If fear of infertility restricts contraception use, what do we know about this fear? An examination in rural Ethiopia.

BACKGROUND: Ethiopia has made great progress toward reducing unmet need for family planning and increasing contraception use over the last decade. However, almost one-quarter of women still have an unmet need. The primary reason for non-use is "method-related health concerns" and, within this broad category, the belief that using contraception will cause infertility is common. This belief extends beyond Ethiopia to low-, middle-, and high-income countries across the world. The objective of this paper is to examine associations with the belief that contraception use causes infertility and to su

Promoting Family/Friend Involvement in Care Planning in Care Homes: A Qualitative Exploration of the Usefulness and Relevance of an Information Resource.

BACKGROUND: The involvement of family/friends in the care planning of an older adult residing in a care home can help to foster person-centred care. This study explored and incorporated family/friends' views on the relevance and usefulness of a draft information resource focusing on care planning to address and support older adults' needs. METHODS: The research team drafted an information resource to help family/friends better understand care planning. The contents of the draft resource were informed by previous research on care planning in older adult care homes, which included a scoping rev

Hospital admissions among adolescents with local authority care experience or special educational needs in England: a population-based cohort study using linked administrative data from health, education and social care services.

INTRODUCTION: Children and young people in state care or with special educational needs (SEN) experience disproportionately higher rates of long-term physical and mental illness compared with their peers. However, few large-scale studies have explored the intersection of social care, education systems and healthcare services during adolescence. We aimed to quantify planned and unplanned hospital admissions during adolescence for pupils with SEN and/or experience of state care in England. METHODS: We analysed linked administrative records from hospital, education and social care systems for ad

Discharge planning and resource utilization for minimizing readmissions in acute exacerbation of chronic heart failure: Insights from an observational study in a community hospital.

This cross-sectional study analyzed discharge disposition in 1,584 readmitted patients aged 65 or older with acute exacerbation of chronic heart failure (AECHF) in a large community hospital from April 2021 to April 2022. The study aimed to explore the relationship between age (65-74, 75-85, and 85 or older) and gender (male, female) with discharge disposition. Results revealed that 55.6 % were discharged for home self-care, 27.3 % with external home health support, and 17.1 % to skilled nursing facilities. Logistic regression showed no significant differences in discharge between age groups.

Provision of health care services related to substance use disorder in southern U.S. jails.

INTRODUCTION: The U.S. jail population has more than tripled since the 1980s, and today, one out of every three incarcerated individuals is being held in a county or city jail. Substance use disorders (SUD) are overrepresented in incarcerated populations; however, little recent research has examined the availability and quality of SUD-related health care services in jail settings. Incarcerated individuals may engage with a variety of SUD-related health care services, including: screening and withdrawal management at entry, SUD treatment or other brief health care interventions while they are b

From Earthly companions to extraterrestrial countermeasures: rethinking AI-mediated intimacy for long-duration spaceflight through modular autonomous intimacy companion architecture.

The prospect of multi-year missions to the Moon and Mars raises unresolved questions about intimacy, attachment, loneliness and sexual expression in isolated, confined and increasingly autonomous habitats. While the physiological, psychological and operational risks of spaceflight have been extensively studied, sexual intimacy remains a taboo domain, leaving little policy, design guidance or behavioural health planning. Terrestrial research on AI-mediated companionship and sex-tech shows that emotionally meaningful and erotic interaction is possible through chatbots, affective agents, haptic d

Advancing equitable postpartum depression screening in a level-IV NICU: a quality improvement approach shaped by planned experimentation.

BACKGROUND: Given the incidence and impact of postpartum depression (PPD) on families in the NICU, we aimed to increase the percentage of birthing parents screened for PPD from 0 to 90% over 18-months and provide psychosocial support. METHODS: Planned experimentation was used to design an effective screening framework in a level IV NICU. Interventions included process standardization, electronic health record (EHR) support, and data visualization for feedback. Data were displayed on a control chart and stratified for race and language to ensure equitable delivery. RESULTS: Provider screening

Leading With LARCs in Nigeria: The Stars Are Aligned to Expand Effective Family Planning Services Decisively.

Despite years of family planning effort in Nigeria, the modern contraceptive prevalence (mCPR) has reached only 10%. Yet a few recent seminal, well-executed programs have been outstandingly successful providing long-acting reversible contraceptives (LARCs)—both in the public and private sector, and in the North and South. Remarkably, the LARCs they provided were equivalent to 2% mCPR in 2015 alone. Accordingly, we advocate markedly increased support for: (1) private-sector approaches such as social franchising, particularly in the South, (2) mobile outreach, and (3) support to public clinical

Implementation of a Modified Early Screening for Discharge Tool to Optimize Case Manager Efficiency and Impact Length of Stay.

PURPOSE OF STUDY: The postacute landscape has been challenged since the onset of the COVID-19 pandemic by staffing shortages and a decline in postacute bed availability. As a result, patients in acute care hospitals are experiencing longer lengths of stay (LOS) and case managers (CMs) are managing increasingly complex discharge plans. This project involved the design and implementation of a modified Early Screen for Discharge Planning (ESDP) tool to support prioritizing patients with complex discharge needs, with the primary outcome of decreasing LOS. PRIMARY PRACTICE SETTING: The project too

Perspectives of Clients and Health Care Professionals on the Opportunities for Digital Health Interventions in Cerebrovascular Disease Care: Qualitative Descriptive Study.

BACKGROUND: Cerebrovascular diseases (CVDs) are a major and potentially increasing burden to public health. Digital health interventions (DHIs) could support access to and provision of high-quality health care (eg, outcomes, safety, and satisfaction), but the design and development of digital solutions and technologies lack the assessment of user needs. Research is needed to identify opportunities to address health system challenges and improve CVD care with primary users of services as the key informants of everyday requirements. OBJECTIVE: This study aims to identify opportunities for DHIs

Declining fertility and increasing use of traditional methods of family planning: a paradox in Uttar Pradesh, India?

Uttar Pradesh (UP), with more than 220 million people, is the most populous state in India. Despite a high unmet need for modern family planning methods, the state has experienced a substantial decline in fertility. India has also seen a decline during this period which can be attributed to the increased prevalence of modern methods of family planning, particularly female sterilisation, but in UP, the corresponding increase was marginal. At the same time, Traditional Family Planning Methods (TMs) increased significantly in UP in contrast to India, where it was marginal. The trends in UP raise

Interactive Visualization Applications in Population Health and Health Services Research: Systematic Scoping Review.

BACKGROUND: Simple visualizations in health research data, such as scatter plots, heat maps, and bar charts, typically present relationships between 2 variables. Interactive visualization methods allow for multiple related facets such as numerous risk factors to be studied simultaneously, leading to data insights through exploring trends and patterns from complex big health care data. The technique presents a powerful tool that can be used in combination with statistical analysis for knowledge discovery, hypothesis generation and testing, and decision support. OBJECTIVE: The primary objective
