Can tissue engineering applications replace cardiovascular medical devices? Searching for the key to clinical translation.

Cardiovascular diseases are the number one killer worldwide. Malfunction of the conduction system, heart valves, the myocardial muscle or the vessels irrigating the tissue have been traditionally addressed with medical devices such as pacemakers, left ventricular assist devices, heart valves and vascular grafts. These positively impact critical outcomes such as functionality and mortality. However, by their non-biological nature, they cannot operate with the same efficiency as their original natural counterpart. Conversely, tissue engineering is an interdisciplinary field aimed at fabricating

A membrane model of electrically remodelled atrial myocardium derived from in vivo measurements.

AIMS: Contemporary ionic-based membrane models are computationally expensive and are not intended to match the properties of a given experimental preparation. The aim of this work was to use measured restitution properties of electrically remodelled atrial tissue to develop a simplified membrane model based on the Fenton-Karma (FK) equations amenable to large-scale simulation of chronic atrial fibrillation (CAF). METHODS: Two membrane models, the FK-CAF and FK-CNTRL parameter sets, were developed to match action potential duration (APD) and conduction velocity (CV) restitution properties of r

The evolving state of cardiac resynchronization therapy and conduction system pacing: 25 years of research at EP Europace journal.

Cardiac resynchronization therapy (CRT) was proposed in the 1990s as a new therapy for patients with heart failure and wide QRS with depressed left ventricular ejection fraction despite optimal medical treatment. This review is aimed first to describe the rationale and the physiologic effects of CRT. The journey of the landmark randomized trials leading to the adoption of CRT in the guidelines since 2005 is also reported showing the high level of evidence for CRT. Different alternative pacing modalities of CRT to conventional left ventricular pacing through the coronary sinus have been propose

Cardiac electrophysiological substrate underlying the ECG phenotype and electrogram abnormalities in Brugada syndrome patients.

BACKGROUND: Brugada syndrome (BrS) is a highly arrhythmogenic cardiac disorder, associated with an increased incidence of sudden death. Its arrhythmogenic substrate in the intact human heart remains ill-defined. METHODS AND RESULTS: Using noninvasive ECG imaging, we studied 25 BrS patients to characterize the electrophysiological substrate and 6 patients with right bundle-branch block for comparison. Seven healthy subjects provided control data. Abnormal substrate was observed exclusively in the right ventricular outflow tract with the following properties (in comparison with healthy controls

Unusual conduction disorder: left posterior fascicular block + left septal fascicular block.

A 44-year-old man with aortic valve insufficiency and stenosis underwent aortic valve replacement depicting interesting ECG changes. This unique case is discussed, contributing to the understanding of a trifascicular left intraventricular conduction system.

Optimizing conduction system pacing lead placement utilizing the image overlay technique.

The His-bundle has several locations from which conduction system pacing can be achieved. Some locations offer better sensing, thresholds and paced QRS durations. Existing techniques to aid repositioning of an already deployed, but sub-optimally placed lead, include either simple memory of the initial lead position combined with its observation on an x-ray review screen or utilizing an additional vascular access and pacing lead with the first lead serving as a real-time marker (Two-lead technique). We describe a novel, readily available, cost-efficient, imaging-based approach to assist in the

Comparison of Pacing Performance and Clinical Outcomes Between Left Bundle Branch and His Bundle Pacing.

BACKGROUND: Left bundle branch (LBBP) and His-bundle pacing (HBP) provide physiological ventricular activation. OBJECTIVES: This study investigated differences in feasibility, device performance, and clinical outcomes between LBBP and HBP. METHODS: Consecutive patients with LBBP and HBP from 2018 to 2021 in 2 centers were prospectively studied. The primary endpoint was optimal device performance during follow-up, defined as the presence of pacing thresholds <2.5 V, R-wave amplitude ≥5 V, and absence of conduction system pacing (CSP)-related complications. The secondary endpoint was the compo

Incidence, Predictors, and Implications of Permanent Pacemaker Requirement After Transcatheter Aortic Valve Replacement.

Transcatheter aortic valve replacement (TAVR) is a safe and feasible alternative to surgery in patients with symptomatic severe aortic stenosis regardless of the surgical risk. Conduction abnormalities requiring permanent pacemaker (PPM) implantation remain a common finding after TAVR due to the close proximity of the atrioventricular conduction system to the aortic root. High-grade atrioventricular block and new onset left bundle branch block (LBBB) are the most commonly reported conduction abnormalities after TAVR. The overall rate of PPM implantation after TAVR varies and is related to pre-

Injury to the conduction system: management of life-threatening arrhythmias after penetrating cardiac trauma.

BACKGROUND: Life-threatening conduction abnormalities after penetrating cardiac injuries (PCIs) are rare, and rapid identification and treatment of these arrhythmias are critical to survival. This study highlights diagnosis and management strategies for conduction abnormalities after PCI. METHODS: Patients with life-threatening arrhythmias after PCI were identified at an urban, level I trauma center registry. RESULTS: Seventy-one patients survived to reach the hospital after PCI. Of these, 3 (4%) survivors (male = 3, mean age 41.3, median injury severity score = 25) had critical conduction a
