Portopulmonary hypertension (PPHTN) refers to the development of pulmonary arterial hypertension in the setting of portal hypertension with or without chronic hepatic failure. This syndrome is characterized by marked alternations of pulmonary vascular tone and obstruction of pulmonary arterial blood flow. An increased pulmonary blood flow, which is a hallmark of the hyperdynamic circulation of cirrhotic patients, seems to be present in almost all patients who develop PPHTN. The elevations of pulmonary artery pressure (PAP) and pulmonary vascular resistance (PVR) along with the transpulmonary g

BACKGROUND AND PURPOSE: The mechanism of stroke in patients with atherosclerotic occlusive disease and hemodynamic failure may be primarily hemodynamic or a combination of hemodynamic and embolic factors. The purpose of this study was to investigate the clinical and imaging features of stroke in these patients.
METHODS: Eleven patients with complete atherosclerotic carotid artery occlusion and increased oxygen extraction fraction measured in the hemisphere distal to the occlusion developed an ischemic stroke during the observation phase of a prospective study of cerebral hemodynamics and stro…

Adrenomedullin, a 52-amino acid residue peptide, has numerous biological actions which are of potential importance to cardiovascular homeostasis, growth and development of cardiovascular tissues and bone, prevention of infection, and regulation of body fluid and electrolyte balance. Studies in man using intravenous infusion of the peptide have demonstrated that, at plasma levels detected after myocardial infarction or in heart failure, adrenomedullin reduces arterial pressure, increases heart rate and cardiac output, and activates the sympathetic and renin-angiotensin systems but suppresses al

The authors' objective was to review previous studies of immediate (first 30 seconds) and stabilized (30 seconds to 20 minutes) hemodynamic responses of healthy adults to the head-up posture, with particular reference to alteration of such responses in the elderly and the usefulness of such data in the diagnosis of orthostatic hypotension. The immediate response in healthy young adults is characterized by a prompt rise in heart rate, which peaks at about 8 to 15 seconds and then tapers; the arterial pressure and total vascular resistance decrease sharply at 5 to 10 seconds, followed by a rapid

BACKGROUND AND PURPOSE: Hemodynamic stroke is a recognized but poorly described entity. The aim of this study was to define the frequency and pathogenic mechanisms of hemodynamic stroke.
METHODS: We prospectively studied 300 consecutive patients with acute ischemic stroke for evidence of a hemodynamic mechanism. All patients with a probable or possible thromboembolic source were excluded.
RESULTS: Twenty-nine patients (9.6%) had documented or presumed hypotension at stroke onset, with 27 of 29 (93%) having watershed infarction on computed tomography (CT). Most (21/29) patients had a slow (ho

Three variants of hemodynamic reaction to massive thromboembolism of the pulmonary artery are encountered--hyperkinetic, eukinetic, and hypokinetic. Their manifestations are determined by the patient's cardiovascular status at the onset of thromboembolism, the volume of the embolic obstruction, the degree of pulmonary hypertension, and the concomitant cardiopulmonary pathology. Effective treatment leading to marked reduction of pulmonary hypertension is conducive to increase in the proportion of the hyperkinetic type; ineffective thrombolysis, thrombosis of the inferior vena cava antiembolic f

The authors reviewed the actions of nifedipine, a blocker of calcium channels, on the systemic and pulmonary circulation. The mechanism of action of the drug on the systemic and pulmonary circulation and on the gas exchange is discussed. The main effects of nifedipine on the coronary circulation and indications for therapeutic use of the drug are discussed on the topics of coronary insufficiency, systemic and pulmonary blood hypertension, acute respiratory failure, and Raynaud's phenomenon.

Cerebral blood flow was measured by the intravenous xenon-133 technique at rest and during cerebral vasodilation with acetazolamide in 32 patients before and after uncomplicated carotid endarterectomy. The results were compared with the internal carotid artery perfusion pressure measured during surgery. A significant improvement in side-to-side cerebral blood flow asymmetry occurred in 6 patients studied at baseline and in 11 patients during provoked cerebral vasodilation. These patients all belonged to a group of 14 patients who, in addition to a severe stenosis of the internal carotid artery

It is reported on a new catheter technique for the determination of the cardiac output by means of the method of thermodilution. It consists of a thin sensorcatheter (3 Ch), which contains a microthermistor as temperature sensor and additionally two electrodes. The insertion of the catheter is carried out via puncture of the subclavian vein by means of a special introducer set, the outer sheath of which can used for injecting the cold solution. The positioning of the sensor catheter into the pulmonary artery may be done by intracardiac ECG monitoring with the help of the two electrodes. By mea

The treatment of cardiovascular collapse and anaphylactic shock is largely empiric. A simple animal model was developed to evaluate the hemodynamic alterations in anaphylaxis. Eight adult New Zealand white rabbits of both sexes were studied. All animals weighed 3.8 kg to 5.3 kg. Sensitization was accomplished with a 2-mL subcutaneous dose of horse serum followed in two days with a 2-mL intravenous (IV) dose. At least 14 days elapsed after the IV dose before a 1-mL challenge dose of horse serum was given. On the day of the challenge dose, a femoral arterial catheter, arterial temperature probe,

When a large left ventricular aneurysm or infarct is excised, a frequent complication is inability of the left ventricle to regain adequate performance. Reduction in ventricular volume and impairment of myocardial contractility, either of which can diminish cardiac output to levels incompatible with life, are probably the main factors that lead to left ventricular failure after such surgery. A prototype prosthetic myocardium, designed to mimic the actions of the left ventricular myocardium, offers a direct means of restoring both ventricular volume and contractility after excision of a large p

The object of this investigation was to demonstrate the hemodynamic features of cardiac rejection. The postoperative functional evolution of a homogeneous group of unmodified homografts was compared with that of a group of autografts studied by the same protocol. On the 4th postoperative day, a significant decrease was observed initially in the maximum systolic output, the maximum systolic rate index, the mean systolic ejection rate and also in the strength, work and, especially, acceleration of the left ventricles of the homografts. In the terminal phase, the abnormalities already present in
