Stroke is less common in children than in adults, yet equally devastating. Nevertheless, children with symptoms of acute stroke commonly present to community hospitals or adult stroke centers where experience and institutional protocols for pediatric stroke diagnosis and management may be limited. Despite advances in pediatric stroke awareness and the implementation of stroke pathways in many children's hospitals, significant gaps remain in the timely recognition, diagnosis, and management of pediatric stroke. A key contributing factor is the lack of system-level infrastructure to guide pediat

The Home Hospitalization Programme was initiated in Jerusalem in 1991 to provide intensive medical care at home in order to prevent or shorten hospitalizations. The programme was based upon regular home visits by physicians, and nursing assessment to determine the need for regular nursing care. Primary-care physicians and nurses were renumerated by a global monthly fee, and were on 24-h call in addition to their periodic visits. Patients were recruited by senior geriatric physicians from acute hospital wards, as well as from the community, at the family doctor's request. Ancillary services ava

BACKGROUND: Optimal maternal infant and young child nutrition is a cornerstone of child survival and development. Kenya adapted the Baby Friendly Hospital Initiative (BFHI), to strengthen maternal, infant, and young child nutrition through community level interventions, multisectoral engagement, and integration within the Community Health Strategy and Primary Health Care. Despite strong policy support, scale-up and sustainability remain uneven. This study examined enablers, barriers, and gaps influencing the implementation of Baby Friendly Community Initiative (BFCI) in Kenya.

BACKGROUND: Italy can be viewed as a laboratory to assess the quality of mental healthcare delivered in a community-oriented system, especially for severe mental disorders, such as personality disorders. Although initiatives based on clinical indicators for assessing the quality of mental healthcare have been developed by transnational-organisations, there is still no widespread practice of measuring the quality of care pathways delivered to patients with severe mental disorders in a community-oriented system, especially using administrative healthcare databases. The aim of the study is to eva

BACKGROUND: Seamlessly providing appropriate drug therapy to patients recovering from stroke is essential for preventing recurrence, regardless of changes in their medical care environment. To address this, several initiatives have been undertaken to improve communication between hospital and community pharmacists, including the use of discharge medication summaries. However, we previously revealed discrepancies between the contents of prescriptions upon discharge and outpatient prescriptions from other institutions. Therefore, in the present study, we aimed to examine the effect of bidirectio

We examined risks for first hospitalization and the rate, risk factors, costs, and 1-year outcome of 30-day readmission among patients admitted for complications of cirrhosis. Data were retrospectively analyzed for adult patients with cirrhosis residing in Minnesota, Iowa, or Wisconsin and admitted from 2010 through 2013 at both campuses of the Mayo Clinic Hospital in Rochester, MN. Readmission was captured at the two hospitals as well as at community hospitals in the tristate area within the Mayo Clinic Health System. The incidence of hospitalization for complications of cirrhosis was 100/100

PURPOSE: People living with dementia and their care partners identify interactions with the healthcare system as among their greatest challenges. Many hospital staff do not feel prepared to care for people living with dementia. This contributes to poor outcomes for patients living with dementia, frustration and confusion for care partners, and distress for hospital staff.
PATIENTS AND METHODS: An academic project team with expertise in education, geriatrics, simulation, and community engagement, who had previously developed dementia-friendly training materials for classroom and community use,

This essay argues for the urgent need to redefine the role of geriatricians, as medical speciality, beyond hospital settings, emphasizing the importance of community geriatrics (CG) in Latin America. Despite global discussions on integrating CG in geriatric practice, its presence remains marginal in low-income countries, where older adults face systemic barriers to health care access. The essay calls for a discussion of CG within geriatric training model, which often prioritizes hospital-based care, and advocates for a community-based approaches to aging. Using a geriatric program in Colombia

This communication identifies several incongruities in a recent article published in the journal. Key concerns include: a lack of context provided in relation to the background issues with the hospital water system, misapplication of research findings, and a lack of clarity in the discussion section regarding enteric pathogens.

Low-income and minority communities are disproportionately affected by incarceration, which exacerbates health disparities. Through enhanced primary care, the Transitions Clinic Network (TCN) seeks to improve the health of people with chronic conditions who return to their communities from prison. Using TCN data, we assessed the impact of early engagement in primary care and referral from correctional systems to TCN on the use of acute care (emergency department visits and hospitalizations) and recidivism. Of 751 patients, 29.8 percent were referred by correctional partners, and 52.7 percent w

BACKGROUND AND AIMS: Learning Health Systems (LHS) integrate research, clinical expertise, and patient data to continuously improve health care. Although community engagement is central to LHS, existing literature has largely focused on data infrastructure, clinical improvement, and system learning, with limited attention to meaningful patient and public involvement (PPI) in community-based care. This review aimed to identify facilitators and barriers to such involvement in the development of LHS in community health services.

BACKGROUND: Mental health disorders are more prevalent in urban areas; urban living has been linked to increased risk of anxiety, mood, and schizophrenia disorders. These associations may be partially explained by the social and physical urban environment. While traffic-related exposures such as air pollution and noise have been linked to adverse mental health outcomes, the role of road infrastructure and traffic in severing communities and breaking down the social fabric-termed community severance-remains understudied.
METHODS: We conducted ZIP code-level analyses (2011-2014) to investigate

BACKGROUND: Dementia prevalence is rising, and it will double in the next 20 years. This study sought to understand the prevalence of dementia in hospitalized patients with ischemic stroke, and its impact on outcomes.
METHODS: Using the Canadian Institute of Health Information's (CIHI) Discharge Abstract Database (DAD), all acute ischemic stroke admissions from April 2003 to March 2015 in Canada (excluding Quebec) were analyzed. Concurrent dementia at the time of admission was assessed based on hospital diagnostic codes. Characteristics and in-hospital outcomes were compared in patients with

OBJECTIVE: This study was designed to describe our experience in implementing the American Academy of Pediatrics (AAP) recommendation to test infants in a car bed if they fail a car seat challenge test.
STUDY DESIGN: Car seat challenge testing was performed for 90 minutes or the anticipated duration of the trip home, whichever was longer. Based on the pulse oximetry saturation values (90-92% or < 90%) two categories of test failure were used. Infants who failed in the car seat were tested in the car bed. Testing took place in both the normal newborn nursery (NBN) and special care nursery (SCN

When the epidemic of Marburg hemorrhagic fever occurred in Uige, Angola, during 2005, the international response included systems of case detection and isolation, community education, the burial of the dead, and disinfection. However, despite large investments of staff and money by the organizations involved, only a fraction of the reported number of cases were isolated, and many cases were detected only after death. This article describes the response of Medecins Sans Frontieres Spain within the provincial hospital in Uige, as well as the lessons they learned during the epidemic. Diagnosis, m

Community-based palliative care is defined as palliative care delivered outside of the hospital and outpatient clinics. These settings include the home, nursing homes, day programs, volunteer organizations, and support groups. There is strong evidence outside of the neuropalliative context that community-based palliative care can reduce hospital costs and admissions at the end of life. Research that focuses on specialized community-based palliative care for neurologic disease have similar findings, although with significant variability across conditions and geographic locations.
