Four patients with active Paget's disease were treated with the RNA inhibitor actinomycin D. Three were clinically improved after treatment; the fourth had multiple collapsed vertebrae and showed no symptomatic improvement. Striking changes took place in urinary calcium and hydroxyproline, in serum alkaline phosphatase, and to a less extent in serum calcium and phosphate. These studies are continuing and are being compared with the effects of mithramycin.

METHODS: Blood and urine samples were collected from rats fed with a methionine-enriched diet (HHCY, n = 18) or an isocaloric control diet (control, n = 10) for 12 weeks. Primary dissecting aneurysms of peripheral arteries without involvement of the aorta are rare. The present report deals with a case of dissecting aneurysm of the external iliac artery in a 39-old man treated by resection and prosthetic graft. Although the exact cause of the dissection was not apparent, it seems likely that it was related to disorder of connective tissue because of the presence of a pectus excavatum, a deep palate and an increased hydroxyproline urinary excretion.

Delta 1-pyrroline-5-carboxylate dehydrogenase extracted from the combined ciliary body and iris of bovine eyes was studied biochemically. The enzyme was purified 120-fold. The partially purified enzyme had broad optimum at pH 8.0. Apparent Km values for DL-delta 1-pyrroline-5-carboxylate and NAD were 0.14 and 0.18 mM, respectively. The enzyme activity was strongly inhibited by GABA, proline, hydroxyproline, and glutamine.

Two acute quadriplegic adolescent boys with hypercalcemia had large increases in urine calcium during calcium-free, or nearly free, intravenous hyperalimentation. The calcium which spilled in the urine proved to be of endogenous origin since it was accompanied by proportional amounts of hydroxyproline. To limit the endogenous bone losses during hyperalimentation, it is recommended that calcium in an amount equal to that loss be added to the perfusate so that only exogenous calcium is spilled in the urine.

The hypothesis that Paget's disease of bone is a low grade neoplastic process led us to use the cytotoxic antibiotic mithramycin in its treatment. The dramatic effects observed on the serum calcium and alkaline phosphatase, and urinary hydroxyproline are compatible with the concept that mithramycin is primarily toxic to osteoclasts. Subjective and objective clinical effects establish this agent as useful in the treatment of Paget's disease despite its observed toxicity to other organ systems.
