Single Stage Repair of #30 Facial Cleft with Bone Morphogenic Protein.

Tessier #30 clefts (median mandibular clefts) represent a spectrum of deformities ranging from a minor cleft in the lower lip to complete clefts of the mandible involving the tongue, lower lip, hyoid bone, thyroid cartilages, and manubrium. Various techniques have been used to address these problems; the most common procedure involving 2 stages: an initial correction of the soft tissue followed by closure of the mandibular cleft at a later date using bone grafting. This approach was subsequently reduced to a single operation, but still required harvesting of autologous bone graft. Here, we des…

The thyrohyoid membrane as a target for ultrasonography-guided block of the internal branch of the superior laryngeal nerve.

STUDY OBJECTIVE: The objective was to present a proof of concept for a simple and consistently successful ultrasonograpy (US)-guided techniq … t of iSLN penetration through the thyrohyoid membrane. The distance between the greater horn of the hyoid bone and the nerve entry point into the thyrohyoid membrane was measured. Ultrasonography-guided in-plane injections were performed unilaterally with 22-gauge 50-mm nerve block needles in 3 fresh cadavers with 2-mL lidocaine/methylene blue mixture deposi …

Human hyolaryngeal movements show adaptive motor learning during swallowing.

The hyoid bone and larynx elevate to protect the airway during swallowing. However, it is unknown whether hyolaryngeal movements during swallowing can adjust and adapt to predict the presence of a persistent perturbation in a feed-forward manner (adaptive motor learning). We investigated adaptive motor learning in nine healthy adults. Electrical stimulation was administered to the anterior neck to reduce hyolaryngeal elevation, requiring more strength to swallow during the perturbation period of this study. We assessed peak hyoid bone and laryngeal movements using videofluoroscopy across thirt…

Absence of facial type differences among preschool children with sleep-related breathing disorder.

The aim of this study was to find out whether there are specific facial types and a specific hyoid bone position in preschool children with sleep-related breathing disorder (SBD). A total of 69 children were divided into 4 groups based on the mandibular line/Frankfurt horizontal angle and apnea index. There were 19 children with hyperdivergent facial type and SBD and 19 children with neutral facial type and SBD, all of them with documented 0 < AI < 5. Ten children had hyperdivergent facial type and non-SBD, and 21 children neutral facial type and non-SBD. The present findings show that SBD may…

Comparing Hyoid Suspension Approaches in Multilevel Sleep Surgery: A Systematic Review and Meta-Analysis.

OBJECTIVE: Hyoid suspension is a method used to address hypopharyngeal obstruction in multilevel surgery for obstructive sleep apnea (OSA). The procedure involves suspending the hyoid bone to either the mandible or thyroid cartilage, determined by surgeon preference. This meta-analysis aims to compare the effectiveness of these two approaches in multi-level sleep surgery. DATA SOURCES: PubMed, Embase, Scopus, and The Cochrane Library. REVIEW METHODS: All studies underwent a 2-stage blinded screening, extraction, and evaluation process. Primary outcomes of the study included pre- and postoper…

Gross anatomical observations of attachments of the middle pharyngeal constrictor.

The pharyngeal muscles overlap each other and some of their parts have different areas of origin. Such arrangements make the interrelationships among pharyngeal muscles complicated. This study investigated the attachments of the middle constrictor to clarify its configuration and re-examine its functions. The gross anatomies of the pharyngeal and neighboring muscles were examined in 41 cadavers. The middle constrictor arose from the stylohyoid ligament and the hyoid bone, and its fibers were divided into three overlapping groups. The anterosuperior group ascended posterosuperiorly from the lig…

Double Thyroglossal Duct Cysts in an Adult.

Double thyroglossal duct cysts (TDCs) are a very rare congenital malformation. Herein, we report a rare case of double TDCs, one below the hyoid bone and the other at the base of the tongue. Clinicians should be aware that TDCs can occur as multiple cystic lesions. Preoperative radiological examinations should be performed for an accurate diagnosis of double TDCs.

Thyroglossal duct cyst with hoarseness as the sole symptom and an intralaryngeal extension masquerading as a laryngeal mass: Clinical experience and literature review.

A thyroglossal duct cyst is the most common congenital disease in the neck. There are two age groups usually associated with thyroglossal duct cysts: 1-11 years in children and 30-60 years in adults. These midline neck masses are typically located anteriorly in the neck, inferior to the hyoid bone. We report an extremely rare case of an intralaryngeal thyroglossal duct cyst without a neck mass, presenting with hoarseness as the sole symptom. A 64-year-old man presented with a 3-month history of hoarseness. On physical examination, no neck mass or swelling was observed during cervical palpation…

Role of vertical larynx movement and cervical lordosis in F0 control.

The role of vertical larynx movement in vocal frequency (F0) change has attracted the attention of many researchers. Recently, Hirai, Honda, … midsagittal MRI data using three male subjects with evident cervical lordosis. Tracings of the jaw, hyoid bone, laryngeal cartilage, and cervical spine were compared in high and low F0 ranges. In the high F0 range, the hyoid bone moved horizontally while the larynx height remained relatively constant. In the low F0 range, the entire larynx moved vertically, …

Avascular midline oropharyngeal anatomy allows for expanded indications for transoral robotic surgery in pediatric patients.

Transoral robotic surgery (TORS) in children is in its infancy, and indications have been primarily limited to lingual tonsillar hypertrophy … cision. Four of the seven patients also underwent transoral resection of the central portion of the hyoid bone, while three had central hyoid resection during prior surgery. Two minor complications occurred with no evidence of lesion recurrence after mean follow-up of 19.7 mo. The midline avascular channel of the tongue allows for relatively bloodless surgic …
