Surgical Outcomes of Dysphagia Provoked by Diffuse Idiopathic Skeletal Hyperostosis in the Cervical Spine.

PURPOSE: This study aimed to predict the surgical outcomes of diffuse idiopathic skeletal hyperostosis (DISH)-related dysphagia (DISH-phagia) and to evaluate the importance of prevertebral soft tissue thickness (PVST). MATERIALS AND METHODS: In total, 21 surgeries (anterior osteophytectomy or anterior cervical decompression and fixation) were included in this study for DISH-phagia from 2003 to 2019. Clinical outcomes were assessed using the Dysphagia Outcome and Severity Scale (DOSS) preoperatively, at 1 month postoperatively, and last follow up (mean 29.5 months). PVST was measured using lat…

The History of Arachne Through Historic Descriptions of Meningiomas with Hyperostosis: From Prehistory to the Present.

BACKGROUND: Intracranial meningiomas are brain tumors that have probably been known the longest, largely because of the occasional productio … first meningioma monograph and the first explanations of Harvey Cushing regarding the mechanism of hyperostosis, this review aims to weave again the story of Arachne. We identify the main contributors who have tried to understand and explain the tendency of some of these tumors to cause hyperostosis or other skull bone involvements. CONCLUSIONS: The contribu …

Respiratory presentation of diffuse idiopathic skeletal hyperostosis (DISH): Management and review of the literature.

Background: Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by the ossification of entheses. When localized in the cervical paravertebral region, typical signs and symptoms include stiffness, pain, loss of range of motion, and difficulty swallowing.Clinical Presentation: The authors present two less typical respiratory manifestations of DISH due to cervical osteophytes protrusion and obstruction of the upper airway. The first patient was treated conservatively (application of CPAP during nighttime), while the second required emergency intubation and a combined ENT-neurosurgica…

A rare case of Eagle syndrome and diffuse idiopathic skeletal hyperostosis in the cervical spine.

Published case studies involving diffuse idiopathic skeletal hyperostosis (DISH) along with symptomatic ossification of the stylohyoid ligament, or "Eagle syndrome," are rare. In this report, we document a case of bilateral Eagle syndrome and advanced DISH in the cervical spine of an 80-year-old man who presented with severe neck stiffness and intermittent asphyxia. The key imaging and clinical features of this disorder are also described.

Decompressive surgery in a patient with hyperostosis corticalis generalisata for relief of cognitive disability and dysaesthesia.

A 28-year-old man with genetically confirmed hyperostosis corticalis generalisata (Van Buchem disease) suffered from headache and progressive cognitive and sensibility disorders. Bone formation of the skull was ongoing, leading to narrowing of the intracranial space and foramen magnum. A large bilateral frontoparietal craniotomy and decompression of the foramen magnum resulted in almost complete relief of his symptoms. This is the first report on successful decompressive surgery as a treatment of cognitive impairment and dysaesthesia.

Imaging diagnosis-an atypical presentation of diffuse idiopathic skeletal hyperostosis (dish) in a dog.

A 10-year-old female spayed Dalmatian was evaluated for progressive cervical scoliosis and stiffness. This imaging report describes the imaging and postmortem findings for this patient. A diagnosis of an atypical manifestation of diffuse idiopathic skeletal hyperostosis (DISH) was made based on imaging and additional diagnostics. This report serves to increase awareness of DISH in the veterinary community, as well as to describe a unique presentation of the disease with atypical lesion distribution and severity. In addition, this report contrasts the pathophysiology and imaging characteristics…

Feline intracranial meningioma with skull erosion and tumour extension into an area of skull hyperostosis.

Skull hyperostosis is a frequently recognised feature of meningioma in feline and human patients, occurring at a frequency of around 4.5% of human cases. Evidence of osteolysis with extension of meningioma into, and in some cases through, the region of skull hyperostosis is much less commonly described in human patients. Here we present a 12-year-old cat with marked skull hyperostosis secondary to an intracranial meningioma, with magnetic resonance imaging and computed tomography evidence of tumour extension into the skull, centrally within the region of hyperostosis. Only a thin layer of bone…

[Giant cervical hyperostosis of the prevertebral space: presentation of two cases and review of the literature].

Swellings of the retropharyngeal area are quite rare and all cases have common symptoms deriving from the site of origin, size, interaction with the surrounding anatomical structures and the presence, or lack thereof, of any associated inflammation. The main symptoms are dysphagia, the feeling of a foreign body in the pharynx and dyspnea. The present work describes two clinical cases of hyperostosis of the cervical column both of which presented discrete dysphagia, intermittent dyspnea and swelling in the area located between the pharynx and the spinal column thus leading to the feeling of hav…

Idiopathic periosteal hyperostosis with dysproteinemia (Goldbloom's syndrome): case report and review of the literature.

Idiopathic subperiosteal new bone formation is uncommon in childhood. We report a case of diffuse periostitis in association with hypergammaglobulinemia and hypoalbuminemia in a 12-year-old boy. In 1966, 2 cases of idiopathic periosteal hyperostosis with accompanying dysproteinemia were documented. This report serves as a review of this unique and perplexing problem in clinical diagnosis.

Autosomal dominant type of endosteal hyperostosis with unusual manifestations of sclerosis of the jaw bones.

We report three cases of autosomal dominant type endosteal hyperostosis which occurred in one Japanese family. A new pattern of sclerotic changes in the jaw bones is evident. In all members of the family there was a symmetrical thickening of the diaphyseal cortices of the long bones. The affected bones were only minimally widened and the epiphyses and metaphyses were spared. Endosteal sclerosis of the neurocranium was present with loss of the diploë. The sclerotic changes included enlargement and mottled sclerosis of both the maxilla and mandible, with multiple embedded teeth and odontomas. Th…

Cranial Distraction Osteogenesis Combined With Bone Thinning for Nonsynostotic Cranial Hyperostosis Following Ventriculoperitoneal Shunt.

Cranial distraction osteogenesis is widely used for craniosynostosis. However, it is rarely applied in nonsynostotic cranial deformities, such as cranial hyperostosis and asymmetry. The authors report the case of a 17-year-old patient with open-lip schizencephaly that developed cranial hyperostosis and skull asymmetry due to chronic over-drainage from a fixed-pressure ventriculoperitoneal shunt. He presented with neurological deficits and cranial deformation. The authors performed cranial distraction osteogenesis combined with bone thinning and conversion to a variable-pressure ventriculoperit…

Investigating hyperostosis frontalis interna: a computed tomography analysis and predictive model for Hershkovitz classification.

BackgroundAxial computed tomography (CT) cross-sections offer an accessible model for assessing diverse pathologies associated with hyperostosis frontalis interna (HFI) based on the Hershkovitz classification.PurposeTo delineate the CT characteristics of HFI, emphasizing a radiological description using the Hershkovitz classification. It investigated whether the Hershkovitz classification can be predicted using density, a variable unexplored in the literature. HFI thickness and the presence of concurrent hyperostosis beyond the frontal bone (HBFB) were evaluated across varying degrees of the H…

Use of ultrasonic bone dissector in diffuse idiopathic skeletal hyperostosis involving the cervical spine.

INTRODUCTION AND IMPORTANCE: Diffuse idiopathic skeletal hyperostosis (DISH), or Forestier's disease, is characterized by ligament and enthesis calcification, mainly in the thoracic spine. Cervical involvement can cause dysphagia due to soft tissue compression, necessitating surgical intervention when conservative measures fail. We assess the efficacy of anterior cervical osteophytectomy with an ultrasonic bone dissector in relieving symptoms and improving outcomes. CASE PRESENTATION: A 73-year-old male presented with progressive neck pain and dysphagia caused by DISH in the cervical spine. C…

Distinguishing bone marrow edema patterns on MRI between spondyloarthritis and diffuse idiopathic skeletal hyperostosis.

BACKGROUND: Bone marrow edema (BME) in the vertebral body, a key feature of spondyloarthritis (SpA), is not uncommon in diffuse idiopathic skeletal hyperostosis (DISH). Our study aims to compare the vertebral BME patterns on MRI between DISH and SpA. METHODS: This retrospective study included 273 patients with DISH and 132 patients with SpA who underwent whole-spine MRI. Two radiologists jointly assessed primary imaging features on radiographs and MRI of the spines, followed by independent evaluations of BME characteristics including the presence, location, signal intensity and associated fin…
