Oncological emergencies: syndrome of inappropriate antidiuretic hormone secretion (SIADH).

Excessive secretion of vasopressin in the course of Syndrome of Inappropriate Antidiuretic Hormone Secretion is a common cause of hyponatremia in cancer patients. Clinical symptoms depend on the cause, rate of change of sodium level and their absolute values. Treatment options include fluid restrictions, intravenous administration of hypertonic sodium chloride solutions, loop diuretics and vaptans. The sodium level should not be adjusted too fast, because it may lead to irreversible brain damage. The article presents pathophysiology, diagnostics and recommendations of management of this oncolo

Lignocaine versus bupivacaine for flank anaesthesia using multiport catheters via a caudal epidural approach in cattle.

OBJECTIVE: To determine the anaesthetic and systemic effects of dorsolumbar epidural anaesthesia using non-stylet multiport catheters via the caudal approach to administer hypertonic 5% lignocaine (HL) or hypertonic 0.5% bupivacaine (HB) to the flank in standing cattle. MATERIALS AND METHODS: Six healthy adult cattle weighing 310-455 kg received 0.2 mg/kg HL or 0.025 mg/kg of HB; control animals received 0.9% saline solution. All drugs were injected into the dorsolumbar epidural space via a caudal approach through a non-stylet multiport catheter. Each animal received each treatment at random.

Cerebral influences of sodium and angiotensin II on cardiovascular function in hypotensive hemorrhage.

During progressive blood loss several mechanisms act in concert to compensate for the reduced intravascular volume with the overall aim to provide sufficient blood supply to vital organs. The hemodynamic responses in this situation follow a characteristic course of events in conscious individuals with an initial phase of largely maintained blood pressure and tachycardia followed by an abrupt fall in pressure, accompanied by bradycardia and widespread inhibition of sympathetic nervous activity when 20-30% of the blood volume is lost. Our research has focussed on Na+ and angiotensin II effects o

Pathophysiology and treatment of exercise-associated hyponatremia.

Exercise associated hyponatremia (EAH) is a medical condition that can occur during physical exertion. Initially, EAH was considered to be restricted to extreme endurance activities, such as ultramarathons and Ironman triathlons. However, it has been more recently recognized in a variety of sports, including team sports and in shorter-duration events. The pathophysiology of EAH is multifactorial and includes excessive fluid intake and non-osmotic arginine vasopressin secretion, which is induced by physical activity. Sodium loss through sweat appears to play a less important role in contributin

Hyponatremia.

Hyponatremia is the most common electrolyte disorder in hospitalized patients. Hospital-associated hyponatremia includes community-acquired (e.g., hyponatremia on admission) and hospital-acquired hyponatremia. Acute-onset hyponatremia requires rapid treatment with hypertonic saline to decrease cerebral edema. In cases of chronic hyponatremia (>48 hours), the brain has time to normalize cell volume by losing solutes. However, even mild chronic cases can have adverse outcomes, such as decreased cognition, osteoporosis, increased risk for falls, and fractures. Hyponatremia is associated with incr

A rapid and efficient method for the extraction and identification of menaquinones from Actinomycetes in wet biomass.

BACKGROUND: Menaquinones are constituents of prokaryote cell membranes where they play important functions during electron transport. Menaquinone profiles are strongly recommended for species classification when proposing a new Actinomycetes taxon. Presently, the most widely used methods to determine menaquinones are based on freeze-dried cells. Taxonomic research in our lab has revealed that menaquinone concentrations are low for some species of the genus Microbacterium, leading to difficulties in identifying menaquinones. RESULTS: Menaquinones extracted using the novel lysozyme-chloroform-m

Limited fluid volume resuscitation.

Volume replacement therapy is crucial to the treatment of hypovolemic shock. In patients with certain conditions, limiting the volume of fluid administered has many potential therapeutic benefits and technical advantages. Hypertonic saline and colloids have characteristics that allow effective treatment of hypovolemic shock using relatively smaller volumes than would be required for isotonic crystalloids alone. This article describes the theory and clinical application of limited fluid volume resuscitation in veterinary medicine.

Enema-induced severe hyperphosphatemia in children.

We report the case of a 13-year-old boy with Costello syndrome and chronic constipation who received phosphate-containing Fleet-pediatric enemas at regular intervals. The day before admission he was given four enemas for severe constipation. Within 24 h the boy had developed severe hyperphosphatemia (phosphate 17.75 mmol/L), hypertonic dehydration (sodium 171 mmol/L) and severe hypocalcemia (calcium 0.56 mmol/L). With early intervention and treatment, the child survived without sequelae. Osmotically acting hypertonic phosphate enemas may result in severe toxicity. Children with developmental d

Hypertonic saline destruction of multiple glomus tumors.

BACKGROUND: Multiple glomus tumors are uncommon cutaneous neoplasms. Therapeutic destructive alternatives include excision, argon or CO2 laser treatment, and sclerotherapy with sodium tetradecyl sulphate. OBJECTIVE: A case report is presented using a previously unreported technique of therapy, specifically hypertonic saline. METHODS: 23.4% NaCl was injected intraluminally in four sessions over a 6-month period. CONCLUSION: Serial hypertonic saline injections should be added to the list of effective treatment alternatives for multiple glomus tumors.

Advantages of BACTEC hypertonic culture medium for detection of Haemophilus influenzae bacteremia in children.

The hypertonic and aerobic culture media in the BACTEC system were compared for the detection of Haemophilus influenzae bacteremia in children. Of 1,611 blood cultures, 30 were positive for this pathogen. The aerobic and hypertonic media gave positive results in 28 and 29 cultures, respectively. Within the first 12 h, H. influenzae was detected in the hypertonic medium in 48.5% of the positive cultures as compared to 35% for the aerobic medium. Importantly, after the first 12 h, the hypertonic medium yielded positive results sooner than did the aerobic medium, the difference being statisticall

[Morphometric study of the blood-carrying capillaries during the administration of a liquid into the salivary gland].

The physiological saline was injected into the excretory duct of the cat parotid gland under the pressure of 30, 70 and 120 cm H2O. It was found with the aid of transmission electronic microscopy, morphometry and statistical analysis that the liquid injection into the gland produces compression of the blood capillaries weaving the acinus. The compression of the capillary tubes is shown by a significant reduction in space given to the lumen of the capillaries and their endothelial layer. The compression of the capillary tubes is combined with a two-fold lessening suggest that the additional blo

Reduction of water permeability by anisotonic solutions in frog corneal epithelium.

PURPOSE: To study the effects of bathing solution osmolarity and Cl- secretagogues on the diffusional water permeability (Pdw) of the isolated frog corneal epithelium. METHODS: Isolated frog corneas, with the endothelium scraped off, were mounted as a partition between Ussing-type hemichambers. Unidirectional diffusional water fluxes (Jdw) were measured by adding 3H2O to one hemichamber and sampling from the other. Electrical parameters were measured simultaneously. Jdw was determined in control isosmotic conditions and after either changes in osmolarity of the bathing solutions or the additi

Efficacy of Adjuvant 10% Hypertonic Saline in Transforaminal Epidural Steroid Injection: A Retrospective Analysis.

BACKGROUND: Chronic lower back pain with or without radiculopathy represents an important medical, social, and economic problem. Many treatment modalities and techniques, including surgery and epidural administration of steroids, have been used to manage this pain. Hypertonic saline, which has been used as an adjunct to percutaneous epidural adhesiolysis, can also be injected via a transforaminal approach in expectation of longer-lasting effects. OBJECTIVES: This study aimed to determine the effect of adding hypertonic saline to conventional transforaminal epidural steroid injections (TFEI) t

Hypertonicity, but not hypothermia, elicits substance P release from rat C-fiber neurons in primary culture.

Isocapnic dry gas hyperventilation provokes hyperpnea-induced bronchoconstriction in guinea pigs by releasing tachykinins from airway sensory C-fiber neurons. It is unknown whether dry gas hyperpnea directly stimulates C-fibers to release tachykinins, or whether this physical stimulus initiates a mediator cascade that indirectly stimulates C-fiber tachykinin release. We tested the hypotheses that mucosal hypothermia and/or hyperosmolarity--physical consequences of airway heat and water loss imposed by dry gas hyperpnea--can directly stimulate C-fiber tachykinin release. Neurons isolated from n

Hypertonic induction of aquaporin-5 expression through an ERK-dependent pathway.

Aquaporin-5 (AQP5) is a water channel protein expressed in lung, salivary gland, and lacrimal gland epithelia. Each of these sites may experience fluctuations in surface liquid osmolarity; however, osmotic regulation of AQP5 expression has not been reported. This study demonstrates that AQP5 is induced by hypertonic stress and that induction requires activation of extracellular signal-regulated kinase (ERK). Incubation of mouse lung epithelial cells (MLE-15) in hypertonic medium produced a dose-dependent increase in AQP5 expression; AQP5 protein peaked by 24 h and returned to baseline levels w

Administration of 3% hypertonic saline via peripheral route: Is it really safe?

BACKGROUND: Administration of 3% sodiumchloride through a peripheral venous catheter is associated with risk of infusion-related adverse events (IRAE) due to its high osmolarity. Given this concern and the paucity of data regarding these events, many hospitals have policies that require central line administration of 3% sodiumchloride. OBJECTIVE: The objective of this analysis was to evaluate the incidence of IRAE associated with peripheral administration of 3% sodium chloride. METHODS: This analysis included patients who received 3% sodium chloride via a peripheral venous catheter between Ma
