Neurosurgical advances in the treatment of brain tumors.

Surgery remains an important part of the treatment of primary malignant brain tumors. When surgery is utilized, care must be taken to maximize the safety of the procedures. This article emphasizes advances in lesion localization within the brain and technology used to identify the function of normal tissue around the tumor. Many of the new treatment paradigms involve a surgical procedure. For example, surgery is necessary for biodegradable treatment delivery systems, and for some focal radiation therapy. Neurosurgeons are familiar with implantable catheter systems for other types of disease su

Syndrome of Inappropriate Antidiuretic Hormone Release During Ketamine Infusion in Complex Regional Syndrome Patient Receiving Intrathecal Baclofen: A Case Report.

Complex regional pain syndrome (CRPS) is a severely disabling condition that typically develops after an inciting traumatic event. Ketamine infusion in subanesthetic dose provides sustained analgesia in selected cases of CRPS. In general, ketamine treatment does not significantly affect electrolyte or water balance. Here, we report a case of a CRPS patient on intrathecal baclofen pump developing syndrome of inappropriate antidiuretic hormone release (SIADH) during ketamine infusion. Prophylactic treatment with intravenous loop diuretics was successful in preventing the development of SIADH dur

Externalization of Intrathecal Baclofen Pumps Using a Shoulder Sling: Technique and Clinical Outcomes.

BACKGROUND AND OBJECTIVES: To describe a novel axillary shoulder-sling technique for externalizing intrathecal baclofen (ITB) pumps and report its clinical outcomes in a cohort of patients requiring temporary pump removal for infection management. METHODS: We retrospectively reviewed 14 patients who underwent ITB pump externalization using the axillary sling technique between January 2020 and March 2025. The procedure involved tunneling a new catheter subcutaneously from the lumbar region to the midaxillary line and securing the pump within a sterile orthopedic stockinette loop positioned ben

Indications for pump-assisted enteral feeding.

Of 80 patients fed enterally over nine months, 10 had serious gastrointestinal complications from gravity-controlled continuous enteral feeding. Use of an infusion pump controlled diarrhoea in patients with gastrointestinal disorders, but was less successful in patients with poor gastric emptying or impaired swallowing reflexes. Thus a pump is not indicated for routine use but, in carefully selected patients, it can obviate the necessity for parenteral nutrition.

Ambulatory chemotherapy: Past, present, and future.

Ambulatory chemotherapy allows the delivery of short and extended chemotherapy infusions through a portable pump from the comfort of patients' homes. It is essential to offer it for suitable candidates to ensure both their safety and the success of the treatment session. This requires a delicate balance between clinical assessment and patient acceptance. The two main components of this treatment modality are the pump and the access device. There are several pump designs and mechanisms on the market, with the latest being the portable disposable elastomeric one.Clinicians along with a multidiscip

Development and testing of portable pump for the induction of profound hypothermia in a Swine model of lethal vascular injuries.

UNLABELLED: Rapid induction of a profound hypothermic state (suspended animation) can maintain viability of key organs during repair of lethal injuries. Conventional cardiopulmonary bypass equipment (roller pump) used to induce and reverse hypothermia is expensive, bulky, requires standard electricity, and is not transportable. Development of a small, portable, battery operated, disposable, pump can logistically facilitate induction and maintenance of hypothermia. In this experiment, a portable prototype pump was tested and its performance was compared with the regular roller pump in a swine m

Needle detachment in a slim and physically active child with insulin pump treatment.

Insulin pump therapy (CSII) is well established in pediatric patients with type 1 diabetes. In childhood diabetes, insulin pump treatment shows considerable advantages such as fewer injections, increased flexibility, fewer hypoglycemic events and lower HbA1c levels. Side effects such as catheter obstruction, technical pump failure, and dermatological complications have been observed, but are rarely reported. The reported patient is a physically very active and slim 10-year-old boy with reduced subcutaneous fatty tissue. After strong muscular activity an accidental rupture of the infusion set a

Helical neural implants for intracerebral drug delivery.

Objective.Neurological disorders often arise from specific regions of dysfunction in the brain. One approach to target these pathologic regions is through chemical delivery using intracerebral implants. Previous works have designed implants that are small and flexible, minimizing the mechanical mismatch between inorganic implants and soft organic brain tissue. Most of these implants are simple cylindrical catheters with inflow and outflow ports at either end of the cylinder. This limits the region and volume of tissue that can be dosed. We sought to develop novel catheter designs that permit t

Evaluation of the biocompatibility of novel peptide hydrogel in rabbit eye.

A peptide containing a RGD (arginine-glycine-aspartic acid) sequence as well as a hydrophobic N-fluorenyl-9-methoxycarbonyl (FMOC) tail was prepared via a standard FMOC solid-phase peptide synthesis technique. The supramolecular self-assembly of such peptide through pi-pi stacking from FMOC tail can transfer the peptide aqueous solution into a three-dimensional hydrogel. The biocompatibility of the peptide hydrogel was evaluated via clinical follow-up and histological analysis. The data obtained demonstrated that the peptide hydrogel exhibited good biocompatibility when injected to the subconj

Implantofix vascular access system: an experience with 19 patients.

The use of implantable vascular access devices has increased rapidly in recent years. We have placed the new Implantofix Implantable Drug Delivery System 7500 in 19 patients. The patients reported only minor discomfort. The majority of the nurses found the Implantofix port easier to access than other ports, and some reported less swelling and soreness at the port site. Some of the physicians had difficulty visualizing the radiopaque catheter on X-ray; Burron is currently developing a more radiopaque system. The Implantofix system offers several important benefits. Its compact size assures mini

[Clinical experience with bacterial contamination of Port-A-Cath systems in tumor patients].

In 33 cancer patients with subcutaneously implanted Port-A-Cath systems (Pharmacia) who developed bacteremia with rigor and high fever, aerobic and anaerobic cultures were prepared from aspirated chamber blood. Organisms were isolated from 19 patients of whom 10 had fever, but none life-threatening. In seven patients the fever was caused by infected chamber blood, while in three it was impossible to prove whether it was due to chamber contamination or the underlying disease. Almost all of the causative organisms were skin saprophytes, most frequently Staph. epidermidis, Acinetobacter Iwoffi an

Subcutaneous administration of foslevodopa/foscarbidopa (PRODUODOPA®) in arm and thigh due to giant epigastric hernia: a case report.

BACKGROUND: PRODUODOPA® is a combination of foscarbidopa and foslevodopa indicated for the treatment of motor fluctuations in patients with advanced Parkinson's disease when available combinations of peroral drugs have not given satisfactory results. The solution is administered via subcutaneous infusion, most frequently in the abdomen, using an infusion pump. The study results showed a significant effect on the reductions in off time and higher control of motor symptoms without troublesome dyskinesia. CASE DESCRIPTION: A 61-year-old man on long-term oral therapy for Parkinson's disease prese

Postoperative analgesia with transversus abdominis plane catheter infusions of levobupivacaine after major gynecological and obstetrical surgery. A case series.

Transversus abdominis plane block has become an important method of postoperative pain management for patients undergoing abdominal surgery but the modest duration is a major limitation. We report the successful use of a novel TAP catheter technique for continuous infusion of levobupivacaine in six gynecologic and obstetric patients. Bilateral TAP catheters were inserted at the end of surgery by ultrasound imaging using a Contiplex® C needle (B. Braun, Melsungen, Germany) in the Triangle of Petit or in a postero-subcostal level based on the location of the surgical incision. Following negative

Intracerebroventricular infusion of RU28318 blocks aldosterone-salt hypertension.

The chronic intracerebroventricular (icv) infusion of aldosterone in rats and dogs elevates the blood pressure within 10-14 days at doses far below those that produce hypertension systemically. The effect in rats is dose dependent and blocked by the concomitant icv infusion of the antimineralocorticoid, prorenone. The effect of the icv infusion of RU28318, another specific spironolactone mineralocorticoid antagonist, on the hypertension produced by chronic subcutaneous (sc) administration of aldosterone in sensitized rats was reported. Miniosmotic pumps were used to deliver 1 micrograms/h aldo

Flexible versus rigid catheters for chronic administration of exogenous agents into central nervous system tissues.

INTRODUCTION: Neuropharmacology studies depend on consistency in drug delivery. Drug infusions into central nervous system (CNS) tissues have been described as unreliable. Speculation has focused on infusion pumps as the source of variation. This report demonstrates that the catheter may be a source of variability. The inconsistency can be significantly reduced by a change in catheter design. METHODS: Normal and tumor cell-challenged (abnormal) brains of Fischer rats were infused with small and large molecular weight cytotoxic drugs via rigid and flexible catheters placed directly into the pa…

[The pharmacokinetics of continuous peridural morphine infusion].

In patients with cancer pain treated by continuous epidural opiate infusion (4.5-24 mg morphine per day) via implanted or portable pumps (n = 40) plasma levels of morphine were determined during the postoperative period and during regular refill of the pump systems. Concentrations were between 2.6 and 18.8 ng/ml depending on daily dosage and body weight. There were no signs of accumulation. Concentrations in lumbar CSF measured in some of the patients were 15-20 higher but decreased by 10-20% (in relation to daily dosage) in the course of long-term treatment. This may be induced by reduction i
