Meningitis is a condition characterized by inflammation of the meninges that can lead to severe neurological dysfunction and mortality. Epidural steroid injection is a common procedure performed for lumbar radiculopathy and the procedure has its risk and complications such as pneumocephalus, postural headache, meningitis, arachnoiditis, hematoma, abscess, and conus medullaris syndrome. Here, we discuss a case of complicated bacterial meningitis following epidural steroid injection.

Horner's syndrome is a clinically significant condition caused by disruption of the oculosympathetic nerve pathway, typically presenting with ptosis, miosis, and facial anhidrosis. It can result from serious, life-threatening conditions such as trauma, carotid artery dissection, or malignancy; however, it may also arise from iatrogenic reasons, including stellate ganglion blocks, by other surgical interventions of the head and neck. While exceedingly rare, Horner's syndrome has been reported to occur after a cervical epidural steroid injection. Given the various potential etiologies of Horner'

Bertolotti's syndrome, also known as lumbosacral transitional vertebrae, is a relatively rare spinal condition characterized by an anatomical variation in the lower spine. Conservative approaches such as physical therapy, anti-inflammatory medications and lifestyle modifications may be recommended for mild cases. In more severe cases or when conservative measures fail to provide relief, injections such as may be considered to alleviate pain. This case is unique in that we document a challenging technique of ipsilateral transforaminal epidural steroid injection in a patient with Bertolotti's sy

Epidural steroid injections (ESI) are an effective nonsurgical option for the management of chronic lower back pain. Despite the increased frequency of ESI for low back pain management, there is a paucity in the literature surrounding systemic long-term effects. Reported adverse effects after fluoroscopically guided ESI with dexamethasone are low. This case report describes the development of anorgasmia in a 49-year-old man with chronic low back pain and bilateral leg radiculopathy, after a fluoroscopically guided L5/S1 interlaminar ESI with dexamethasone. This adverse effect highlights, to th

Pneumorrhachis (PR) is an uncommon condition characterized by the accumulation of air within the spinal canal. This finding may occur due to various causes, mostly after trauma and medical procedures. It can manifest with various features depending on the underlying cause, the location, and the extent of the air trapped. Clinically, the symptoms in affected patients can span a spectrum, ranging from being asymptomatic to manifesting as radiculopathies resulting from compression. The pneumorrhachis-related cauda equina syndrome is incredibly rare and typically appears as a large volume of air c

Epidural injections are routinely used for short-term management of radicular pain and chronic low back pain. Prescription of this intervention, in the presence of intracranial abnormalities, is a topic of debate. Intracranial arachnoid cysts are cerebrospinal fluid-filled spaces, which are usually asymptomatic despite being a formidable size. As far as the authors know, there have been no cases depicted in indexed literature regarding asymptomatic supratentorial arachnoid cysts becoming symptomatic post undertaking of spinal epidural injections. We depict this phenomenon in a 53-year-old woma

BACKGROUND: Chronic anal fissure is called the reaching the muscle layer under the skin by deepening of the fissure in the breech skin. Our purpose is to enable the tissue to heal by relaxing the anal sphincter with caudal epidural injection.
METHODS: One hundred twenty cases were included in this randomized, controlled study. The cases were categorized into 2 groups; Under ultrasound guidance, the first group, caudal epidural injection group (Group CE, n = 60) was treated with 5 cc bupivakain + 5 cc isotonic from a caudal epidural for a total of 3 sessions at the beginning, 1 week and 2 week

The psychiatric risks of epidural steroid injections for chronic pain in a geriatric patient with no prior psychiatric history are presented here. A 76-year old Caucasian female presented to the emergency department with her family for an inability to sleep, confusion, and behavioral outbursts. The mood instability and psychosis were reported as having started a week after her third epidural steroid injection for low-back pain associated with a prior fall. After 12 days of mixed treatment outcomes and increasing paranoia without any localized neurological findings, the patient was transferred

In clinical practice, cervical transforaminal epidural steroid injection is commonly used for alleviating pain. We report a case of segmental spinal myoclonus that developed after cervical transforaminal local anesthetic and steroid injection. A 39-yr-old man presented involuntary movements of the bilateral upper trapezius muscles after cervical transforaminal epidural steroid injection on the right C6 and C7 roots with a 5 mg of dexamethasone injectate mixed with 0.5 ml of 1% lidocaine and 0.5 ml of normal saline at each level. His myoclonus was semirhythmic and continuously persisted at a fr

INTRODUCTION: Neuropsychiatric symptoms are a well-described side effect of systemic corticosteroid therapy and can range from mild to severe.
CASE PRESENTATION: We describe a case of substance-induced psychosis following epidural injection of 10 mg dexamethasone. Three days after the procedure, the patient developed symptoms including anger, hostility, insomnia, paranoia, and delusions. Symptoms resolved between 7 and 17 days.
DISCUSSION: In the past 50 years, there have been several case reports of severe neuropsychiatric effects following intraarticular or other interventional pain inject

Caudal epidural injections and facet joint injections using steroids and local anaesthetic are widely used methods of pain control in patients suffering from radicular leg pain. In the vast majority of cases this is low risk. We present an interesting case of a patient who suffered from symptomatic adrenal suppression following a caudal epidural injection, and thus wish to draw this rare but significant complication to the attention of orthopaedic practitioners.

AIM: To investigate the impact of statin use on response to fluoroscopy-assisted transforaminal anterior epidural steroid injection (TAESI).
METHODS: Patients undergoing TAESI for low back pain were recruited and stratified according to statin use. Pain was evaluated with a visual analogue scale (VAS) before and at 1, 3, and 6 months after TAESI. Health-related quality-of-life was evaluated using the Short Form 36 (SF-36) questionnaire 6 months after TAESI.
RESULTS: There were no significant differences in VAS scores after TAESI between statin users (n = 40) and statin nonusers (n = 253). Th

PURPOSE: To investigate the effectiveness and safety of epidural steroid injections in patients with lumbar spinal stenosis (LSS).
METHODS: We performed a search on the CENTRAL, Pubmed, Embase and Cochrane databases up to September 2014. We recovered 17 original articles, of which only 10 were in full compliance with the randomized controlled trial (RCT) criteria. These articles were reviewed in an independent and blinded way by two reviewers who were previously trained to extract data and score their quality by the criteria of the Cochrane Handbook (5.1.0).
RESULTS: We accepted ten studies

A 64-year-old male patient with a history of herpes zoster exposure presented with severe, constant, burning pain in the left T10 dermatome consistent with postherpetic neuralgia. Previous treatment included oral and topical medications as well as an intercostal nerve block; however, these treatment options did not provide significant relief. The patient was treated with a single-level T10 thoracic transforaminal epidural steroid injection for refractory postherpetic neuralgia. He reported complete resolution of his symptoms at 2- and 12-week follow-ups. This case illustrates transforaminal ep

In 45 newborns, operated on for the inborn failures of gut, the markers of stress and indices of central hemodynamics were studied up. General anaesthesia was applied together with caudal-epidural injection of local anaesthetics in 15 newborns, in 15--the adjuvants--klophelline and promedol--were added to local anaesthetics, in 15--a standard atharalgesia was used. The best and most durable effect was noted while application of anesthesia in combination with adjuvants.
