Cardiac intensive care for the neonate and child after cardiac surgery.

PURPOSE OF REVIEW: The focus of postoperative care in the pediatric patient with congenital heart disease has become a reduction in length of stay and morbidity. This review will discuss strategies to achieve this goal and recent studies to support current practices. RECENT FINDINGS: Most agree that prolongation of the length of stay following a cardiac surgery contributes to morbidity. Postoperative feeding difficulty, hyperglycemia, acute kidney injury, fluid overload, and prolonged intubation contribute significantly to length of stay. SUMMARY: Postoperative care of the neonate and child

Is nCPAP of value in extreme preterms with no access to neonatal intensive care?

This prospective study was undertaken to investigate the efficacy of nasal continuous positive airway pressure (nCPAP) in a group of extremely small infants denied access to a neonatal intensive care unit (NICU) in South Africa. Consecutive infants weighing less than 1200 g and/or of a gestational age below 28 weeks admitted to the neonatal ward with respiratory distress syndrome (RDS), and who were refused admission to the NICU, received either nCPAP (Infant Flow System E.M.E., UK) of headbox oxygen. Of 22 infants, 11 infants were included in the treatment group (nCPAP) and 10 in the control

The influence of education and experience on ethical attitudes in neonatal intensive care.

A cross-sectional survey of medical students, pediatrics residents, and neonatologists attempted to identify the combined influence of education and experience on ethical attitudes regarding the management of infants weighing less than 750 g at birth. Of the 120 people surveyed, 94 (78%) returned usable surveys. All groups rated quality of life the most important criterion in treatment decisions. Conversely, cost was rated least important by each group. In addition, cost was rated relatively more important by the clinical medical students than by the preclinical students, residents, or neonato

Bacteria Pathogen Distribution in Late-Onset Neonatal Sepsis: Trends from a South African Neonatal Intensive Care Unit.

Late-onset sepsis (LOS) in neonates is defined as a bloodstream infection occurring after 72 hours of life and is primarily caused by bacterial pathogens, often associated with catheter use and prolonged hospitalization. Despite its clinical and policy relevance, limited data exist on the etiology of neonatal infections in sub-Saharan Africa. This 8-year retrospective study investigated microbial patterns and antibiotic trends of LOS in a South African neonatal unit to inform infection control and antibiotic stewardship strategies. Data from neonates weighing 500 to 2,500 g admitted to the neo

Neonatal jaundice: magnitude of the problem in Cairo University's neonatal intensive Care unit as a referral center.

BACKGROUND: Neonatal jaundice is one of the most common physiologic problems requiring medical attention in newborns. It is benign in most cases; however, high levels of bilirubin are neurotoxic and can lead to serious brain damage. OBJECTIVES: This study aimed at assessment of magnitude of neonatal jaundice in cases of neonatal hyperbilirubinemia admitted into neonatal intensive care unit (NICU), Cairo University Pediatric Hospital and to detect possible etiologies, management and outcome. METHODS: The present work is a retrospective study, included 789 neonates suffered from hyperbilirubin

Epidemiology of neonatal sepsis in two neonatal intensive care units in Krakow, Poland in 2016-2017 years.

BACKGROUND: Sepsis in low-birth-weight neonates remains one of the most significant causes of neonatal morbidity and mortality. Approximately 3 million newborns suffer from sepsis globally every year. The aim of this study was to compare demographic and clinical features, as well as etiology and antibiotic susceptibility, of the main pathogens related to neonatal sepsis in two neonatal intensive units during a two-year period. METHODS: We observed early-onset (EO-BSI) and late-onset bloodstream infections (LO-BSI) cases in two high-reference neonatal intensive care units (NICU) over a 24-mont

Epidemiology of neonatal sepsis caused by multidrug resistant pathogens in a neonatal intensive care unit level 3.

Aim Steady progress in intensive treatment worldwide has increased the survival of immature neonates, but with multiple invasive procedures, which have increased the risk of infection, thus the bacterial resistance to antibiotics. The aim of this study was to analyse the epidemiology of multidrug resistance pathogens as causative agents of neonatal sepsis in the neonatal intensive care unit. Methods A retrospective cohort study conducted at the Intensive care unit of the Paediatric Clinic of Tuzla over a three-year period (2016-2018) analysed epidemiology of neonatal sepsis caused by multidrug

Postpartum Care for Parents of Neonatal Intensive Care Unit Infants before and after Adoption of Telemedicine.

OBJECTIVE: Despite higher morbidity and mortality risks, parents of preterm infants miss postpartum visits more often than parents of full-term infants. Whether the introduction of telemedicine improved access to postpartum care among parents of infants in the neonatal intensive care unit (NICU) is unknown. We aimed to compare postpartum visit attendance and care comprehensiveness for NICU parents before and after the option of telemedicine. STUDY DESIGN: We conducted a retrospective cohort study of postpartum parents without a history of hypertension who gave birth <32 weeks at two Philadelp

Alberta family integrated careTM: a review of a family-integrated care model in neonatal intensive care.

Alberta Family Integrated CareTM (AB-FICareTM) is a standardized, theory-driven model that integrates parents as primary caregivers in neonatal intensive care units (NICUs); however, its clinical effectiveness, implementation determinants, and cross-setting applicability require systematic synthesis. This narrative review aims to evaluate the evidence on AB-FICareTM in Level II NICUs, focusing on neonatal and parental outcomes, implementation barriers and facilitators, and transferability to healthcare systems with distinct organizational and cultural contexts. Following SANRA guidelines, we s

Parental satisfaction with care given in neonatal intensive care units, multicentre referral hospitals in eastern Ethiopia.

BACKGROUND: Parental satisfaction is an important dimension of the quality of a child's care and has been shown to improve health outcomes, including adherence to therapeutic regimens and understanding of medical information. Parental satisfaction is low in resource-limited countries like Ethiopia, with the paucity of evidence. This study aimed to determine the magnitude of parental satisfaction with care given in neonatal intensive care and its associated factors among parents of neonates admitted to neonatal intensive care units (NICUs) at referral hospitals in eastern Ethiopia from June to

The Response of the Infant to the Father's Voice in the Neonatal Intensive Care Unit.

BACKGROUND: The formation of the family is interrupted following a Neonatal Intensive Care Unit (NICU) admission, and fathers report experiencing delayed infant bonding due to unit barriers and separation. Fathers state comfort with early infant bonding through language, fearing physical contact with the sick newborn. During hospitalization, active engagement supports ongoing infant/parent vocalization and infant stabilization. PURPOSE: This prospective descriptive pilot study explored the infant's physiological response to the father's voice during a live reading activity. METHODS: After In

Intensive care and perioperative management of neonates with functionally univentricular hearts.

Although mortality rates for patients with univentricular hearts continue to fall, the course in the intensive care unit is remarkable for significant morbidity and utilization of significant resources. Preoperative patient management focuses on balancing competing circulations, pulmonary and systemic, which exist in parallel rather than in series, as in the normal circulation. Postoperative patient management focuses on optimizing systemic output, respiratory status, and mitigating the effects of cardiopulmonary bypass. In this article, we review pre- and postoperative intensive care manageme
